The development of a spondyloarthropathy phenotype is the result of a complex interaction between environmental triggers acting on a genetically susceptible individual Reiter's syndrome, writer's syndrome, Riter's syndrome, writers syndrome, Reiters syndrome, reiter syndrome. The striking specificity of these diseases for individuals with B-27 remains the single most striking genetic clue to the etiology and the pathogenesis, however it is also quite clear that there are many other genes involved in determining susceptibility Reiter's syndrome, writer's syndrome, Riter's syndrome, writers syndrome, Reiters syndrome, reiter syndrome. For example, twin studies have shown that HLA-B 27 positive monozygotic twins have a concordance rate of With regard to environmental triggers, there is the obvious well known triggering action of bacterial pathogens, particularly JRAM negative organisms. But there are some rather recent variance modeling studies to suggest that the majority of the susceptibility is actually genetic, implying that whatever the environmental triggers are they are probably relatively ubiquitous. The predominant hypothesis in the last decade or so to explain the role that HLA-B 27 plays in disease pathogenesis argues that it is a result of its peptide binding specificity. So it is hypothesized that B 27 can select and present arthritogenic peptides and these peptides presumably are arthritogenic because they resemble in some way a pathogen derived peptide and thus can stimulate the proliferation of auto-reactive B 27 restricted T cells. This hypothesis has really not been proven, in fact to date there are no arthritogenic peptides that have been identified and studies looking for auto-reactive CD-8 positive B 27 restricted T cells are fairly limited.

I am a lay person, who because of my own personal health problems, has done extensive research on some Mediterranean illnesses which seem to run throughout my Melungeon connected families. MEDITERRANEAN DISEASES WHICH

SYNDROME is a relapsing, multi-system inflammatory disease in which there are oral/genital ulcers. There may be inflammation of the eyes, joints, blood vessels, central nervous system and gastrointestinal tract involvement. Attacks last about a week to a month and reoccur spontaneously. Onset is usually between 20-30 years of age with symptoms ocurring up to several years after the onset. Twice as many men as women are affected. Raynaud's Disease, Phenomenon
Raynaud's syndrome, first described in 1862 by the French physician Maurice Raynaud, has no known cause or cure. The syndrome is a condition in which small arteries, most commonly in the fingers and toes, spasm and cause the skin to turn pale or a patchy red to blue. Estimates of its prevalence range from 5 to 30% of the population. It is 5 times more common in women, often striking females between the ages of 18 and 30. I've experienced iritis attacks for over 20 yrs. I have also experienced an arhritis conditon . I was diagnosed with reiters syndrome , and apparantly the arthritis and iritis are symptoms of this disease. I'm currently taking cortisone, dilating, and acular drops. My vision is greatly impaired. Each irits bout lasts approx. 2mos. than goes away. I'm also taking injections of "enbrel" for the arthritis Next Article

Autoimmune Diseases Autoimmune diseases are the result of the body's defense system going awry. Humans have developed a very complex immune system to protect the body from invading infections, such as viruses, bacteria, fungi, and parasites. When functioning normally the immune system is important in maintaining good health and preventing serious illness from infection. Diseases such as AIDS are the result of immune deficiency and develop when the immune system is inhibited and underactive. Autoimmunity, on the other hand, results when the immune system mistakenly is activated and produces self-injury. Autoimmune diseases include those that can affect the endocrine system and produce diabetes or attack the nervous system and produce diseases such as multiple sclerosis and myasthenia gravis. Autoimmune diseases also occur when the joints, muscles and internal organs of the body are injured by a misdirected immune system and produce rheumatic disease. The rheumatic diseases that result form autoimmunity that we treat at the Center for Arthritis and autoimmunity include rheumatoid arthritis, systemic lupus erythematosus or lupus, Sjogrens syndrome, mixed connective tissue disease, polymyositis, dermatomyositis, scleroderma and vasculitis. A number of people (male and female) suffer from painful joints of such severity that they take painkillers e.g. paracetamol for relief or pay a visit to their general practitioners. In some of these cases the joint pain (arthralgia) is due to inflammation of the joint (arthritis) following an infection in some other part of the body. This is known as reactive arthritis. On some occasions the infection would have been acquired following sexual activities. In sexually acquired reactive arthritis (SARA), asymmetric polyarthralgia which may be migrating is the worst common presenting symptom. Some people may have one joint involved while others have several joints involved, and the common joints are the elbow, wrist, knee and ankles. Hip joint involvement is uncommon and the condition may present acutely of sub-acutely. Reiters syndrome consists of urethritis, conjunctivitis and arthritis.
Clinical features
The history of men seen recently in our department are shown in table 1. More men than women are diagnosed with reactive arthritis. The male:female ratio shown in table 2 may be due to underdiagnosis in women. Inflammation of the joint usually occurs within four weeks after the infection. This may be accompanied by low back pain, muscular pains and low grade fever. Other parts of the body that may be involved are the urethra, the eyes and the heart - carditis. Skin manifestations include keratoderma blennorrhagica and erythema nodosum. In some patients there may be frequent recurrence of the joint pains while in others it may become chronic.

RHEUMATIC DISEASES
