BECHET'S SYNDROME
is a relapsing, multi-system inflammatory disease in which there are oral/genital ulcers. There may be inflammation of the eyes, joints, blood vessels, central nervous system and gastrointestinal tract involvement. Attacks last about a week to a month and recur spontaneously. Onset is usually between 20-30 years of age with symptoms ocurring up to several years after the onset. Twice as many men as women are affected. There is a genetic predisposition, with autoimmune mechansism and viral infection which may all play a part.
Related disorders are Reiters Syndrome, Stevens Johnson Syndrome, and Ulcerative Colitis.

Familial Mediterranean Fever (FMF) is a rare (I am not so sure about the rare.) inflammatory disease characterized by recurrent attacks of fever and acute inflammation of the membranes that line the abdominal cavity (peritonitis) and/or the lungs (pleuritis); pain and swelling of the joints (arthritis); and/or in some cases, skin rashes. In addition, some affected individuals may experience a serious complication known as amyloidosis, which is characterized by abnormal accumulation of a fatty-like substance (amyloid in various parts of the body. If amyloid accumulates in the kidneys (renal amyloidosis), kidney function may be impaired and life-threatening complications may occur. HOW IS RHEUMATOID ARTHRITIS DIAGNOSED?
Rheumatoid arthritis may be difficult to diagnose. Many other conditions can resemble it and its symptoms can develop insidiously. Blood tests and x-rays may show normal results for months after the onset of joint pain. Even after rheumatoid arthritis has been diagnosed, it is extremely important to determine whether the course of the disease is benign (type 1) or aggressive (type 2) in order to treat the problem appropriately.
Blood Tests
Various blood tests may be used to help diagnose RA, determine its severity, and detect complications of the disease.
Rheumatoid Factor. In RA, antibodies that collect in the synovium of the joint are known as rheumatoid factor . In about 80% of cases of rheumatoid arthritis, blood tests reveal rheumatoid factor. It can also show up in blood tests of people with other diseases. However, when it appears in patients with arthritic pain on both sides of the body, it is a strong indicator of type 2 RA. The presence of rheumatoid factor plus evidence of bone damage on x-rays also suggests a significant chance for progressive joint damage.

URETHRITIS DEFINITION Urethral inflammation most often manifested by urethral discharge, dysuria, or meatal pruritis confirmed by the laboratory finding of increased number of polymorphonuclear leukocytes (PMNs) on Gram-stained urethral smear or in the sediment of a first voided urine. I. Etiology Gonococcal urethritis: Neisseria gonorrhoeae. Nongonococcal urethritis: Chlamydia trachomatis Ureaplasma urealyticum Trichomonas vaginalis Herpes Simplex Virus Candida albicans Miscellaneous bacteria Mycoplasma genitalium Bacteroides urealyticum Other ( E. Gouty arthritis refers to painful monosodium urate crystal deposits in soft tissues, causing extreme pain, especially in the great toe or hand.  Ligaments bursa , etc. and other tissues with poor blood supply are subject to these crystals being deposited.  There are several chronic diseases that predispose to gout including coronary artery disease, obesity, alcoholism, high blood pressure, and chronic thiazide diuretic usage.  The condition initially completely resolves, then recurrences begin occurring with increasing frequency.  Initially, the joint spaces are normal on x-ray.  Treatment is avoidance of certain foods, colchicine and steroids for acute attacks, and allopurinol for prevention.
Spondyloarthropathies refer to other non-inflammatory sources of arthritis.  These most often involve the sacroiliac joints and are negative for rheumatoid arthritis blood tests.  However, except for psoriatic arthritis, HLA B27 is positive in 90% (normal populations have a positive test in only 10%). Ankylosing spondylitis   results in stiffening of the spine and spine pain from spontaneous spinal fusion of multiple spine segments and swelling of ligament insertions into bone, and one sided sacroiliitis. The spine eventually develops a typical bamboo appearance on x-ray.

    There is another form of arthritis call Reiter"s syndrome that can come from certain bacteria or Salmonella found in food or water. People who develop this seem to have an abnormal response to certain infections, in that their immune systems attack their joints while fighting off the infection.
