  • Originally defined by a Swedish neurologist in the 1940s, restless leg syndrome is now thought to affect up to 15% of the population, particularly older people. Four basic elements must be present to make the diagnosis:
  • A desire to move the limbs, often associated with paresthesia or dysaesthesia Symptoms exacerbated by rest and relieved by activity Motor restlessness Nocturnal worsening of the symptoms

  • What helps? L-dopa and benzodiazepines have been tried, but there are now three high-quality randomised studies of pergolide (a dopamine receptor agonist) which show it to be highly effective. Details of the studies are shown in the Table. All of the studies were well-designed and highly detailed in the outcomes they examined, and all of them allowed a period of dose-escalation with both pergolide and comparator with drug formulations of identical appearance.

    Up to 15 percent of pregnant women develop restless leg syndrome (RSL) during their third trimester, when sensations such as crawling, creeping, or tingling inside the foot, calf, or upper leg begin to disrupt sleep. Some women may feel cramping, burning, or pain, but nearly all report an overpowering urge to move their legs. Unfortunately, stretching, bending, rubbing your legs, and walking around offer only temporary relief; as soon as you sit or lie down, the symptoms return. The condition usually gets worse in the afternoon or evening and at night. No one knows for sure what causes RSL. Some researchers suggest the condition may be inherited, while others think it may be the result of nerve damage in the legs from diabetes, kidney problems, or alcoholism. Mary O'Malley, a psychiatrist and consultant for sleep disorders at Norwalk Hospital in Connecticut, says pregnancy-induced iron deficiency also can cause or aggravate RSL. She bases another theory on personal experience: "I had RSL throughout my entire pregnancy. For me, the symptoms got worse if I ate sugary foods before bed or if my legs got cold." Although O'Malley gave birth nearly a year ago, she says that even today she occasionally feels as if she has ants in her pants.

    R estless legs syndrome (RLS) is a common, underdiagnosed and treatable condition. A neurologic movement disorder, RLS is often associated with a sleep complaint. Patients with RLS may suffer an almost irresistible urge to move the legs, usually due to disagreeable leg sensations that are worse during inactivity and often interfere with sleep. RLS may be described as an agitated inability to rest that can have a negative impact on quality of life by causing waking discomfort, chronic sleep deprivation and stress. This article provides science-based information about RLS and its assessment and management in the primary care setting. Consequences of RLS Direct adverse effects of RLS include discomfort, sleep disturbances and fatigue. These consequences have a secondary impact on functioning by affecting occupational activities, social activities and family life. Disrupted sleep and an inability to tolerate sedentary activities can lead to job loss, a compromised ability to enjoy life and problems with relationships.

    Darkness to Light Home Page Dealing with Restless Leg Syndrome by Gary F. Zeolla I woke up early in the morning on February 22, 2001, too early to get up. So I rolled over on my side and curled my legs up to try to fall back asleep. But just as I was falling asleep, both legs just kicked out on their own and woke me up. I shook my head wondering what happened and curled up again. But as I was falling asleep, one leg kicked out. I tried again, and this time the other leg kicked out. Now I was getting worried as I didn’t know what had happened. So I rolled over on my back to try again. I figured my legs couldn’t “kick” if they were straight. But this time my arm jerked up into the air. Now I was getting really worried. So I tried again, and now my one of my hands flipped up. Then my fingers would snap out own their own. Now I was really worried. I had heard of restless leg syndrome, but all of my limbs were jerking. That night when I went to bed the same scenario repeated itself. I barely slept at all the entire night. The same was true for every night for the next week. After a week of hardly any sleep, my resistance was probably very depleted, and I came down with the flu. I had the flu back in the fall, but I got over it in a couple of days. This time, I only had about a 99 degree temperate. But after not sleeping hardly at all the first night I was sick, I got up the next day with a 101-degree temperature. And it stayed between 100 and 101 degrees for over a week. Without hardly any sleep, my body simply wasn’t able to fight the virus very well.

Periodic limb movement was formally described first in the 1950s, and, by the 1970s, it was listed as a potential cause of insomnia. In addition to producing similar symptoms, PLMD and RLS are treated similarly. Periodic Limb Movement Disorder Periodic limb movement disorder affects people only during sleep. The condition is characterized by behavior ranging from shallow, continual movement of the ankle or toes, to wild and strenuous kicking and flailing of the legs and arms. Furthermore, abdominal, oral, and nasal movement sometimes accompanies PLMD. Movement of the legs is more typical than movement of the arms in cases of PLMD. Movements typically occur for 0.5 to 10 seconds, in intervals separated by five to 90 seconds. In 1979, the Association of Sleep Disorder Centers (ASDC) set the parameters for determining the presence of PLMD:
