People with PLMD may be completely asymptomatic, or they may complain of either excessive daytime sleepiness or insomnia, or both. Limb movements can be severe enough to wake an individual from sleep, making it difficult to stay asleep for a significant duration and leading to excessive daytime sleepiness. Many patients who suffer from excessive daytime sleepiness do not know they are being aroused from sleep by periodic limb movements because they do not actually wake up. Rather, they will feel as though they have not slept well. These arousals can occur anywhere from five times an hour up to more than 50 times an hour, depending on the severity of movement. As mentioned earlier, leg movements in PLMD are typically an upwardly flexed big toe and ankle. Sometimes the hip and knee are flexed and tightened as well.

restless leg syndrome By James Broomfield, MD Restless leg syndrome involves unusual sensations in the legs that cause frequent leg movements. What is going on in the body? No one knows exactly why restless leg syndrome occurs. People who suffer from restless leg syndrome have uncontrollable urges to move their legs. A feeling of discomfort in the leg prompts the need for movement. For some people, these sensations may be the result of nerve damage. The sensations may also be caused by decreased blood flow through some vessels in the legs. These leg movements often occur after a person has gone to bed. This can make sleeping difficult. When the urge to move the leg happens during the day, a person may have to get up and walk around to relieve the discomfort. What are the signs and symptoms of the condition? A person with restless leg syndrome experiences unusual sensations that usually occur in the legs but sometimes are present in the arms and trunk. The sensations: are described as burning, itching, crawling, pulling, or tugging but are rarely painful occur when the person is inactive and usually get worse at night cause an irresistible urge to move the legs are relieved by leg movements cause jerking movements of the legs when the person is sleeping and occasionally when the person is awake can occasionally cause restless movements of the feet or toes in the evening, when a person is sitting or lying down

Chorea and Huntington's Disease Restless leg syndrome (RLS) is a neurological disorder that is fairly common but often unrecognized or misdiagnosed. It may affect up to 12,000,000 people in the United States. It is characterized primarily by sensory symptoms and a movement disorder. The 4 primary features of restless syndrome, as defined by the International Restless Leg Syndrome Study Group in 1995, are:
  • The desire to move the legs in association with unusual or uncomfortable sensations deep within the legs, usually in the calves; in some cases the arms may also be affected. The sensations are usually described as creeping, burning, tingling, cramping, aching, pulling, crawling or water flowing deep within the affected extremities.
Motor restlessness in a response to or an effort to relieve unusual sensations or discomfort. Such movements are often repetitive and may include pacing, rocking, shaking, tossing and turning in bed, stretching, bending, marching in place or repetitive exercises such as walking on a treadmill.
Symptoms become worse while at rest and may be temporarily diminished by voluntary movements of the affected limb.

Tourette's Syndrome RESTLESS LEG SYNDROME What is restless leg syndrome? Restless leg syndrome (RLS) is a condition in which the person develops an urge to move in order to relieve discomforting symptoms or sensation, usually arising in the legs. Although physicians use the term "restless", most people with the condition describe it more as a tenseness, tightness or creepy - crawly or burning feeling. Usually the sensation is most severe in the evenings and brought on by rest or relaxation, and relieved by movement including stretching, massaging, bicycling the legs or walking about. The symptoms usually come on gradually after age 20 and may fluctuate over time, becoming more noticeable or aggravating in the 50's and 60's. They usually interfere with falling asleep or may waken the sufferer at night. They may be accompanied by periodic leg movements in sleep, usually consisting of extension of the big toe and flexion of the ankle, knee and hip.

    Restless Legs Syndrome (RLS)
People experience restless legs in different ways, but all describe a very unpleasant "creepy, crawly" sensation that occurs in the legs when they are sitting or lying still, especially at bedtime. It is also different from the feeling of a limb having "fallen asleep" when the blood supply is cut off and from the "pins and needles" or burning sensation sometimes experienced by diabetics. The pain and unpleasant feelings of RLS appear most often in the calves and can be temporarily relieved by stretching and moving the legs. If you suffer from RLS, you will find that it occurs primarily when you are awake, but that it can also affect your ability to sleep. Five to 10 of every 100 people experience the discomfort or pain of RLS at some time in their lives. How do I know if I may have Restless Legs Syndrome?

    • Home remedies are effective for some people. These include a hot bath, leg massage, use of a heating pad, ice packs, aspirin or other pain relievers, regular exercise and elimination of caffeine. Prescription medications may help depending on the severity of the condition, other medical problems or other medications that the patient is taking.

    Restless Leg Syndrome
    Q:
I have a big problem with restless leg syndrome, actually not only legs but in the shoulder and elbows too. I understand that there is not a great deal that can be done for it. The pharmacist in our drug store has the problem and he is at a loss as to what to do for it. While taking Roxicet for some discomfort for cracked ribs, I discovered that it helps the spasms considerably. I only take it at night. Hot baths help too, but it is annoying when I have to take hot baths at 2 or 3 o'clock in the morning. Do you have any information on this condition?
A: Restless leg syndrome, (RLS), is one of those very annoying problems that is not life threatening but drives a person crazy. (It seems like many of the questions I get for the column fall under that category!) Although fairly common, affecting about 10-15% of people, RLS is poorly understood. Patients describe very unpleasant sensations as being creepy-crawly, jittery, tingly, or burning pain. These feelings occur mainly in the legs, especially the calves, but occasionally the arms may also be involved. Lying in bed or sitting in a car or theater can bring the symptoms on, and moving around, such as stretching, pacing the floor, or cycling seems to provide some relief. Massaging the legs or hot baths also may be soothing.

    More on Restless Leg Syndrome
    Q:
Recently I spent the month of August visiting my children and friends in Madison and Hartland (I live in Littleton, North Carolina), and I read the Milwaukee Journal-Sentinel everyday. In doing so, I found your column that discusses restless leg syndrome. That troubles me about one night a week. I read another article about this syndrome that told of a tonic called Dr. Johnís for Quiet Leg. I asked my son-in-law, a pharmacist, and my doctor, previously a surgeon in Philadelphia now practicing General Medicine at the Littleton Clinic, their opinions and both advised me to send for it as the ingredients are not harmful. Thought this information might be of interest to you and your column. I have ordered some and will try it. Sorry this letter is so long.
A: Hope you enjoyed your time in Wisconsin. Thank you for passing on a potential remedy for restless leg syndrome. It is a problem that has no effective standard medical treatment. I'm a little skeptical when a product advertises that it has been around for 20 years and is 100% guaranteed to work. Why hasn't its fame spread, if it's really that good, by now? Good luck with it!

RLS, Periodic Limb Movement Disorder , PLMD, Nocturnal Myoclonus
    • Restless Leg Syndrome (RLS) is a common condition in which there may be a restless feeling in the legs, with muscle twitching, pins-and-needles, pulling, ants-crawling-under-the-skin sensation, or cramps and muscle aches. This occurs when the patient is lying in bed or sleeping and makes it impossible for the patient, or his bed partner, to get a good night's sleep.
    • This condition is related to PLMD in which one or both legs or arms may undergo a variety of jerky movements from twitching to kicking or hitting the partner in bed.

Periodic Limb Movement/Restless Leg Syndrome (RLS)
    General Information
Periodic Limb Movement condition is characterized by the jerking of a persons leg (or arm) during sleep. Each twitch may last from 1-3 seconds; about every 10-60 seconds. This may occur for only a few minutes or last for hours. In severe cases, the twitches may last all night. The movements themselves seem to do no damage, and some good sleepers experience them without problem. However, if the twitches are strong, or the person is a light sleeper, they can wake a person up. Because they occur before the wakening, you don't know what woke you up. If the awakenings are frequent, it can result in excessive daytime sleepiness. No one really knows the cause of these movements; there are likely to be a variety of causes. Sometimes they are related to certain medications, poor circulation, a metabolic disease, kidney disease or folic acid defficiency. There are some medications which have been used to successfully treat this condition.
    Restless Legs Syndrome
Many people with periodic movements also have Restless Legs Syndrome (RLS), and almost all those with RLS have Periodic Movements. With RLS. there are strong sensations deep within the leg muscles and knees that cause a powerful urge ot move. "It feels like bugs crawling!" This causes problems falling asleep. There are a variety of causes - poor circulation; changes associated with pregnancy and vitamin deficiencies. There are some medications which seem to help. All stimulants such as coffee, tea and chocolate should be avoided.

: I have been diagnosed with ankylosing sponylitis and MS. Over several years I have been getting twitching in my legs when I start to relax. I had an exacerbation last October, and the twitching increasingly got worse. Then my legs twitching at night would wake me up. So, my neurologist put me on clonazepam, before I go to bed, which has relieved it at night. But, it does come back if I lie down for a nap. I have several questions about this First, is restless leg syndrome the same as myoclonus and which do I have.

Contributing risk factors Recommendations Restless leg syndrome (RLS) is a metabolic sleep disorder that causes feelings in the legs that sufferers have described using words such as: tingling, numbness, pins and needles, tiredness, itching, uneasiness, pain, cramping, aching, burning, creeping, crawling, simply indescribable...
These feelings happen during rest or sleep, especially in the lower legs, and create an urge to move the legs about as a way of relieving or completely getting rid of these feelings. If a person with restless leg syndrome tries to hold the legs still and not move them, the feelings only seem to get worse. Restless leg syndrome does not cause any damage to one's health, but it very often leaves the sufferer feeling drowsy during the daytime from not getting a good night's sleep. People who have severe restless leg syndrome may feel a kind of spastic flexing of their hips, knees and ankles when they lie down, as well as leg pains in the morning after they get out of bed. While restless leg syndrome can be a problem at any age, it seems to be most serious in people who are middle-aged and older.
