    Rett syndrome
    Contents of this page: Definition Rett syndrome is an inherited disorder that affects only females. The syndrome causes mental retardation and developmental degeneration. Causes, incidence, and risk factors Rett Syndrome is a severe progressive neurological disorder that causes marked developmental regression, especially in the areas of expressive language and hand use. It occurs only in girls. The gene associated with this disorder was recently identified on the X-chromosome. This gene encodes methl-CpG-binding protein 2 (MeCP2), which regulates synthesis (transcription) of various other proteins. Synthesis of incorrect proteins, due to mutations in the gene, causes damage in the maturing brain. The severity of the disease is linked to specific types of mutation. Rett Syndrome has an estimated prevalence of seven to ten cases per 100,000 females. Females have two X chromosomes, so even when one has this significant mutation, the other X chromosome provides enough normal protein for the child to survive. Males conceived with this mutation are miscarried (spontaneously aborted) during pregnancy because they do not have a second X chromosome to compensate for the problem.

Back September 30, 1999 NICHD Funded Researchers Discover Gene for Rett Syndrome Researchers at the Howard Hughes Medical Institute at Baylor College of Medicine in Houston and Stanford University have discovered the gene for Rett syndrome, a heartbreaking disorder which gradually robs healthy infant girls of their language, mental functioning, and ability to interact with others. The finding has immediate implications for both the development of a test to diagnose the disorder before birth, as well as new strategies to prevent the debilitating effects of the disease. The research team, funded by the National Institute of Child Health and Human Development (NICHD), published its results in the October 1999 Nature Genetics. "Rett syndrome affects girls," said Huda Zoghbi, M.D., the study's senior author. "At about 6 to 18 months of age, otherwise normal children gradually stop responding to and interacting with their parents." Eventually, the girls lose the ability to speak, walk, and purposefully use their hands. Children with Rett syndrome develop repetitive hand motions, such as hand wringing or hand "washing." They may also develop breathing abnormalities and experience sudden, unexpected heart attacks. "Rett syndrome has long frustrated researchers and clinicians because of its unusual pattern of inheritance," said NICHD Director Duane Alexander, M.D. "The discovery of Dr. Zoghbi and her coworkers is a definite turning point, which we hope will soon lead to better diagnosis, and, ultimately, treatment, for this disorder."

    One of the most important things about me is that although I have Rett Syndrome and can no longer talk, I still have plenty to say. Most folks think that because I can't talk, I must not understand what they're saying, or that I must be deaf, and so they shout at me. What's frustrating for me is that they're the ones who don't understand! My mom says that I have a pure spirit, and she tells me that I'm an angel sent to teach everybody what life is really all about. To describe me to someone who doesn't know me is difficult. Like all girls with this weird syndrome, and only girls get it, I have involuntary hand movements. Sometimes I wring my hands, sometimes I clap, and sometimes it looks like I'm trying to wash them. I'm not, it's just something that happens. I also have a unique movement. I cover my mouth with my right hand and flick the fingers of my left hand on my shoulder. I can't help that, but I can use my left hand in another way, too, a way that makes a lot more sense. I can use it to say "yes"! The best thing about me is my spirit and my smile. I have a smile that my mom says knocks people out, and I have this indomitable spirit. One time I almost died because of an infection in my lungs, but I fought hard to live and well, here I am today.

But within six to 18 months of her birth, symptoms of Rett syndrome will present themselves, and she will endure countless hardships, and her parents will face boundless challenges. What is Rett Syndrome?
    According to Kathy Hunter, founder and president of The International Rett Syndrome Association , Rett syndrome (RS) is a neurological disorder seen almost exclusively in females and found in a variety of racial and ethnic groups worldwide. The child with RS usually shows an early period of apparently normal development until 6 to 18 months of life. From that point on, the child gradually loses communication skills and purposeful use of the hands. Stereotyped hand movements, gait disturbances and slowing of the rate of head growth become apparent. Other problems may include seizures and disorganized breathing patterns that occur when awake. This syndrome is often misdiagnosed as autism, cerebral palsy or non-specific developmental delay. In the case of Connie Coughlin of Bartlett, Tenn., her daughter, Dani, was diagnosed with cerebral palsy. The family believed that for 11 years until she was officially diagnosed with RS.

It primarily occurs in females and it can strike all racial and ethnic groups. Development appears to be on target until toddlers reach six to eighteen months of age. Symptoms range from loss of mobility to the inability to retain acquired speech. The head may stop growing and there is a great deal of hand activity, such as hand washing, clapping, wringing of hands and putting one's hands in her mouth.

    What is Rett Syndrome?
    Rett syndrome is a progressive neurological disorder in which individuals exhibit reduced muscle tone, autistic-like behavior, hand movements consisting mainly of wringing and waving, loss of purposeful use of the hands, diminished ability to express feelings, avoidance of eye contact, a lag in brain and head growth, gait abnormalities, and seizures. Hypotonia (loss of muscle tone) is usually the first symptom. The syndrome affects approximately 1 in every 10,000-15,000 live female births, with symptoms usually appearing in early childhood — between ages 6 and 18 months. Rett syndrome is caused by mutations (structural alterations or defects) in the MECP2 (pronounced meck-pea-two) gene, which is found on the X chromosome. Scientists identified the gene — which is believed to control the functions of several other genes — in 1999.

    What is Rett syndrome?
    Rett syndrome is a childhood neurodevelopmental disorder characterized by normal early development followed by loss of purposeful use of the hands, distinctive hand movements, slowed brain and head growth, gait abnormalities, seizures, and mental retardation. It affects females almost exclusively. The disorder was identified by Dr. Andreas Rett, an Austrian physician who first described it in a journal article in 1966.
