Vasomotor Rhinitis
Q:
I was recently diagnosed with vasomotor rhinitis by an allergist. Also, I am allergic to mold and have food allergies from onions, rice and white potatoes, which cause my nose to swell and get stuffed up. Could you explain what he means by vasomotor rhinitis?
A: Rhinitis is an inflammation of the nose ("rhin" meaning nose, as in rhinosceros) which can be divided into a few basic types: infectious (as in a cold), allergic, and vasomotor. Allergic rhinitis can be seasonal and occur only when pollen counts are high, such as ragweed in late summer, or it can be perennial and last pretty much year round because of sensitivity to molds or dusts. Incidentally the mold counts are especially high right now because of the never-ending rain we've had recently. Vasomotor rhinitis is a poorly understood "wastebasket" term for people who have a stuffy or draining nose, but don't have evidence of specific allergies. The blood vessels (hence the term "vaso") in the linings of the nose appear to be extrasensitive and swell up for no good reason. Changes in the temperature or humidity, scents in the air, eating cold food, cigarette smoke, cooking odors, or emotional stress may all be triggers. Some medications, like reserpine and prazosin, have also been implicated. Sexual arousal or hormonal changes can also result in nasal congestion. Sneezing, which is common with allergic rhinitis, is usually not a prominent symptom with vasomotor rhinitis.

Pollens that are carried by bees from plant to plant are seldom responsible for hay fever because the grains are large and have a waxy coating. Rhinitis
Edema and vasodilation of the nasal mucous membrane, nasal discharge, and obstruction. (See also Allergic Rhinitis in Ch. 148.) Acute rhinitis is the usual manifestation of a common cold (see The Common Cold in Ch. 162); it may also be caused by streptococcal, pneumococcal, or staphylococcal infections. Chronic rhinitis is generally a prolongation of subacute inflammatory or infectious rhinitis but may also occur in syphilis, TB, rhinoscleroma, rhinosporidiosis, leishmaniasis, blastomycosis, histoplasmosis, and leprosyall of which are characterized by granuloma formation and destruction of soft tissue, cartilage, and bone. These conditions produce nasal obstruction, purulent rhinorrhea, and frequent bleeding. Rhinoscleroma causes progressive nasal obstruction from indurated inflammatory tissue in the lamina propria. Rhinosporidiosis is characterized by bleeding polyps.
Diagnosis and Treatment
Diagnosis and treatment of acute bacterial rhinitis are based on pathogen identification and antibiotic sensitivities. Topical vasoconstriction with a sympathomimetic amine, such as phenylephrine 0.25%, given q 3 to 4 h for not more than 7 days relieves stuffiness, as do systemic sympathomimetic amines, such as pseudoephedrine 30 mg po q 4 to 6 h.

What is hay fever?
Frequent sneezing is characteristic of hay fever. Hay fever, otherwise known as seasonal allergic rhinitis, is an allergic reaction to airborne substances that attack the upper respiratory passages - the nose, sinus, throat and frequently also the eyes. Allergic perennial rhinitis is caused by house dust, mites and pets for example.
It is the most common of all the allergic diseases - about 15 per cent of the population in industrialised countries suffer from hay fever. The symptoms usually appear first in childhood and then lessen by the age of 30 or 40.
The name hay fever is misleading because symptoms don't just occur in the autumn when hay is traditionally gathered and never include fever.

Rhinitis Rhinitis is a common disorder, that has the potential to cause significant morbidity, impairing quality of life, and work/school performance.
It is also responsible for considerable costs to the econovmy through treatment expenditure, and revenue lost through absence from work. Nasal problems will often hae co-morbid effects on the sinuses, throat, middle-ear, and lower respiratory tract. Rhinitis frequently co-exists with asthma, and there is evidence to suggest that untreated rhinitis may influence the development and control of asthma. Despite these observations, there is a common misconception that rhinitis is an unimportant condition, and as a result is frequently trivialised by patients and their doctors.

The Nasal Passages and Daily Congestion
The nose is separated into two passages by a wall of cartilage called the septum. The nasal passages are lined with a membrane that produces mucus. Mucus is one of the body's defense systems:
  • The mucus, a thin clear liquid, traps small particles and bacteria that are drawn into the nose as a person breathes.
  • The trapped bacteria usually remain harmless in healthy individuals.
  • Even under normal circumstances, this produces a cycle of congestion and decongestion that occurs continuously throughout the day.
  • When one side of the nose is congested, air passes through the open, or decongested, side. The sides alternate between being wide open and being narrowed.
