When children get it, it is usually a mild disease. Rubella also strikes adults, and outbreaks can occur among teenagers and young adults who have not been immunized. Rubella usually occurs in the winter and spring and spreads very easily. People catch it through contact with other people who are infected. It is spread through coughing, sneezing, or talking. Rubella Immunization Rubella vaccine can be given by itself, but it is usually given together with measles and mumps vaccines in a shot called MMR. You can read about MMR vaccine on page . In unvaccinated populations, rubella is primarily a childhood disease. Where children are well immunized, adolescent and adult infections become more evident. Rubella occurs more frequently in winter and spring. How is rubella spread? Rubella is spread by direct contact with nasal or throat secretions of infected individuals. What are the symptoms of rubella? Rubella is a mild illness which may present few or no symptoms. Symptoms may include a rash, slight fever, joint aches, headache, discomfort, runny nose and reddened eyes. The Iymph nodes just behind the ears and at the back of the neck may swell causing some soreness and/or pain. The rash, which may be itchy, first appears on the face and progresses from head to foot, lasting about three days. As many as half of all rubella cases occur without a rash. How soon do symptoms appear?

Rubella Fact Sheet Rubella (German measles) is caused by a virus Rubella virus is spread by contact with discharges from nose or throat The virus is usually shed in the discharges from about 7 days before the rash until at least 4 days after the rash begins. Rubella virus is passed to the next person by direct contact with the secretions. Babies with rubella acquired at or before birth can continue to shed virus in nose and throat secretions and in urine for 1 year or more. Anyone can get rubella except those who have had rubella or rubella vaccine. Symptoms to look for, Low fever, Rash (lasts 3 days or less), Joint aches, Swollen glands, especially behind the ears and the back of the neck, Symptoms start within 16-18 days after exposure, with a range of 14-23 days. Up to half of all people who get rubella don't have any symptoms. Unborn babies are at high risk of rubella complications, Most children and adults fully recover from rubella with few complications. More than 20,000 babies were born with defects during an outbreak of rubella in 1964-65. The same outbreak also resulted in at least 10,000 miscarriages and stillbirths. About 25 percent of babies whose mothers get rubella in the first trimester of pregnancy are born with one or more birth defects (congenital rubella syndrome) including eye defects, hearing loss, heart defects, mental retardation and, less frequently, movement disorders. With the widespread use of the rubella vaccine, major outbreaks of rubella no longer occur in the United States. Still, small outbreaks do occur. As many as 20 percent of childbearing women are susceptible to rubella infection. What you can do: There is a simple blood test that can determine if you are immune to rubella. The March of Dimes recommends that all women be tested for immunity to rubella before becoming pregnant. If you are not immune, consider being vaccinated at least 28 days before trying to get pregnant. Pregnant women are routinely tested for rubella immunity at an early prenatal visit. If a pregnant woman finds out she is not immune, she should not be vaccinated during pregnancy. All she can do is try to avoid exposure to anyone with the illness. She can also get vaccinated soon after delivery so she is immune during any future pregnancies.

Ocular involvement is typically cataracts (bilateral in 75% of the cases) but also may include uveal colobomas, searching nystagmus, microphthalmus, strabismus, retinopathy, and infantile glaucoma. TREATMENT: Cataract surgery is usually delayed until at least age 2, since the live rubella virus remains in ocular tissues for many months after birth. Unfortunately, this preferred delay also results in a poor prognosis for visual functioning following cataract removal. Monitoring of ocular status (for complications) is recommended. Educational adjustments will vary, according to functional vision. Optical aids and/or illumination levels should be according to individual needs. Glare may be a factor. IMPLICATIONS: Some physicians are recommending therapeutic abortion in cases of maternal rubella, since the probability of serious congenital anomalies is so high.
