PROBABLE CAUSES
  • tight quadriceps muscles, tight calf muscles (hamstrings may also be tight)
  • biomechanical faults (usually excessive or prolonged pronation), worn or improper shoes
  • training errors (too much hill work too soon)
TREATMENT STRATEGIES
  • stretch tight muscles (quadriceps and calf muscles)

Runners Knee: The Number One Injury Effecting The Runner
by John W. Zandler, D.C., C.C.S.P. In the classification of athletic-running injuries two main categories exist. The first, referred to as over use injuries, comprise approximately 95% of all athletic injuries and are the result of the body's inability to accommodate physical demands placed upon it by one's training/competition thus resulting in over strain of the body's muscles, ligaments and tendons. These injuries in contrast to traumatic injuries are the result of repeated insult-stress to the body's tissues where as traumatic injuries are due to one traumatic incident (i.e. spraining one's ankle due to stepping off a curb). Due to the prevalence of over use injuries, this article will briefly review common injuries specific to running as well, common factors predisposing one to such injury. Additionally, future articles in the Columbus Roadrunner Newsletter will highlight various running injuries and common precipitating factors of such injuries as well as, prevention treatment methods. In researching 10,271 athletic injuries treated by a large sports medicine facility running accounted for the largest percentage (37%) of injuries treated. Upon further review of such running injuries, the knee joint was the most commonly involved part of the body. Furthermore, it was found that training errors including a sudden change in training, endurance/intensity, or miles was the most common factor precipitating over use injuries thus resulting in time off from training. Additional factors of injury include structural-anatomical problems resulting in altered body biomechanics and over use of the knee joints, muscles, ligaments and bony elements. Thus, due to the prevalence of knee injuries, the following article will focus on common injuries affecting the knee joint.

The most common long-term running injury is runners knee, pain behind the knee cap during running. You probably have runner's knee, if your knee cap hurts when you walk or run, particularly when you walk down stairs. It usually does not hurt to pedal a bicycle and it hurts a lot when you push the kneecap against the bone behind it. The back of the kneecap is shaped like a triangle with the point fitting in a grove in the lower part of the bone behind it. During running, the knee cap is supposed to move up and down and not side to side/ and touch the front of the bone behind it. If it does, the back of the kneecap will rub against the front of the bone behind it and hurt. Treatment is to stop the kneecap from touching the bone behind it, which is usually caused by the knee cap being pulled toward the outside laterally/ while the lower leg twists the bone behind it inward medially. When you run, you land on the lateral bottom of your foot and roll inward, causing the lower leg to twist inward. At the same time, three of the four quad muscles attached to the kneecap pull the kneecap outward and cause the knee cap to rub against the bone behind it. You can keep the knee cap from rubbing by wearing orthotics, special insets in your shoes, doing exercises to strengthen the muscle that pulls your knee cap inward and by running backwards rather than forward. 1. With my right knee i have a sharp pain on the inside of the actual kneecap, like the bone is bruised(?). Running causes discomfort behind and below the kneecap. The joint clicks frequently but painlessly and has done since i broke it when i was 12 (I'm now 33).
2. With my left knee joint i have recently been suffering with 'dull' pain to the outside of the actual joint. There is a tender area just above the joint. I am due to do LONDON MARATHON in April. Any help with injury prevention, rehab. etc would be appreciated.

Chrondromalacia causes knee tenderness, knee pain in front of the knee, and a grating sensation in the knee. The knee pain is worst after sitting for a prolonged period of time or when getting out of a chair. The knee cartilage is affected probably from overuse, trauma and/or abnormal forces on the knee, such as a misaligned patella. Chondromalacia is treated with rest or immobilization and nonsteroidal anti-inflammatory drugs for pain. Physical therapy, especially quadriceps strengthening and hamstring stretching may also be helpful. Surgery is beneficial if there is a problem with the alignment of the patella that cannot be corrected with therapy.

Usual symptoms of this running injury include pain directly under the kneecap, or the surrounding area. Climbing or descending stairs may be painful. Usually the injury responds well with biomechanical evaluation and an orthotic, when indicated. Heel/Arch Pain
Varies from a slight tightness in the bottom of the foot to an intense heel pain that may worsen when getting out of bed in the morning. An X-ray will sometimes reveal the running injury as a "heel spur" on the bottom of the heel. This is usually due to an overpronation problem that will respond well to the use of an orthotic. Ilio-Tibial Band Syndrome This running injury can manifest itself anywhere along the entire length of this highly fibrous connective tissue. It runs from along the hip to the knee. At its worst, pain can be very intense at either the outside of the hip to the outside of the knee. A complete biomechanical evaluation, foot to hip, is needed to diagnose the problem and prevent further running injury. Stretching can aggravate a running injury of this type.

Shin Splints This running injury often causes pain along either side of the lower leg . Symptoms vary greatly from dull tightness to a painful area along the lower leg. Injuries have a higher incidence of occurrence in athletes starting a new running or aerobic program. Many cases of this type of running injury will resolve on their own. If not, consult a Sports Medicine Injury Specialist. Orthotics may be necessary.
Achilles Tendonitis Inflammation of the tendon that connects the calf muscle to the heel. Can be stiff at the beginning of a run and feel better during the workout. Pain varies from dull ache to acute, knife-like pain. Treatment should include sports massage to the calf muscle. The worst running injuries may require surgery. Check running shoes for excessive wear. Stress Fractures Usually located in either the lower leg (tibia) or one of the metatarsals (toes), the 2nd and 3rd toes being the most common. This type of running injury can be difficult to diagnose in its early stages and can be confused with shin splints. Healing usually requires six to eight weeks of rest.

Runner's Knee (Peripatellar pain syndrome/Patello-femoral Pain syndrome)
The symptoms of runners knee include pain near the knee cap usually at the medial (inner) portion and below it. Pain is usually also felt after sitting for a long period of time with the knees bent. Running downhill and sometimes even walking down stairs can be followed by pain. The condition is caused by the kneecap not tracking smoothly in its femoral groove. When the knee is bent there is increased pressure between the joint surface of the knee cap and the femur (thigh bone). This stresses the injured area and leads to pain. Factors that increase what is known as the "Q" (Quadriceps) angle increases the chance of having runners knee. The Q angle is an estimate of the effective angle at which the quadriceps averages its pull. It is determined by drawing a line from the Anterior Superior Iliac Spine (bump above and in front of your hip joint) to the center of your knee cap and a second line from the center of your knee cap to the insertion of the patellar tendon (where the tendon below your knee cap inserts). Normal is below 12 degrees, definitely abnormal is above 15 degrees. Many times adding to the strong lateral pull of the bulk of the quadriceps is a weak vastus medialis. This is the portion of the quadriceps that helps medially stabilize the patella. It runs along the inside portion of the thigh bone to join at the knee cap with the other three muscles making up the quadriceps. Some of the mechanical conditions that may contribute to this include:
