Schizencephaly is an extremely rare developmental disorder characterized by abnormal slits, or clefts, in the brain's cerebral hemispheres. Schizencephaly is a form of porencephaly in which there is a cyst or cavity in the cerebral hemispheres. Individuals with clefts in both hemispheres (bilateral clefts) are commonly developmentally delayed and have delayed speech and language skills and corticospinal dysfunction. Individuals with smaller, unilateral clefts (clefts in only one hemisphere) are often paralyzed on one side of the body and may have normal intelligence. Patients with schizencephaly may also have varying degrees of microcephaly (abnormally small head), mental retardation, hemiparesis or quadriparesis (partial or complete paralysis), and reduced muscle tone (hypotonicity). We took her to the neurologist and her diagnosis was 1. cortical dysplasia involving the right per-sylvian region with broad gyri which are diminished in number associated with thickened cortex. There is deep enfolding of the cortex with a deep sulcus which nearly reaches the lateral ventricle. While this is reminiscent of closed lip schizencephaly, a thin band of white matter separates the enfolded gray matter from the ventricular body. 2. Absence of the septum pellucidum. 3. The optic chiasm and nerves appear to be within normal limits for size. She is now 5 years old and only has a lack of motor skills involving her left arm and leg. She has not shown any other problems but with this diagnosis should I be concerned about any other problems that may come up in the future. And is this hereditary.

It may be related to a vascular event. The resulting abnormality is a seam of grey matter extending from the pial surface of the brain down towards the ependyma of the lateral ventricle. Radiology Resident Case of the Week Schizencephaly April 26, 1996 Lorenzo Carson Peer Review Status: Not reviewed Clinical Sx: Patients may present with seizure disorders, mild to moderate developmental delay and mental retardation. Etiology/Pathophysiology Failure of the normal migration of neurons from the germinal matrix zone at 1-5 months of gestation. A gray matter lined parenchymal cleft develops which extends from the subarachnoid space to the subependyma of the lateral ventricles. Pathology: The cleft may be narrow or closed (type 1) or wide and open (type 2). It is frequently associated with complete or partial absence of the cavum septum pellucidum in approximately 80-90% of cases. the cleft is usually located near the pre- or postcentral gyri. The clefts can be unilateral or bilateral. Polymicrogyria, microcephaly, and gray matter heterotopia have also been noted. Optic nerve hypoplasia has been noted in 33% of cases, with possibility of blindness. Prognosis is poor, with severe intellectual impairment, spastic tetraplegia and blindness. Miscellaneous Most brain malformation can be produced by a variety of injuries during gestation. This includes infections, X-radiation, certain drugs, metabolic and genetic abnormalities.

We have been given very limited answers to our questions, because they are not sure of long term outcomes. My granddaugher is now 2 years old, she is having speed, physical and occupational therapy once a week. She is coming along very well she is about 3 months behind in development so far. Transaxial MRI of the brain Full history/Diagnosis is available below Diagnosis: Schizencephaly Full history: 26 year old man with left frontoparietal open-lip schizencephaly with overlying polymicrogyria presents with refractory complex partial seizures. Clinical evaluation suggests seizures most likely arising from the left frontal region. Radiopharmaceutical: 9.0 mCi F-18 FDG, i.v. Findings: A defect in the left frontoparietal cortex is noted corresponding to the open-lip schizencephaly on MRI. The left hemisphere is smaller than that of the right and there is enlargement of the left lateral ventricle as showed on MRI. Discussion: Schizencephaly, open- or close-lip, is associated with polymicrogyria/agyria in the adjacent cortex. Patients present with seizure, developmental delay, mental retardation and possible blindness if associated with optic nerve hypoplasia.
