  • presence of psychotic symptoms in the absence of mood changes for at least two weeks in a patient who has a mood disorder relatively rare disorder compared to the rate of schizophrenia. an approproate diagnosis when the person does not fit diagnostic standard for either schizophrenia or "affective" (mood) disorders such as depression or major depression often originally diagnosed as bipolar affective disorder (manic depression) like schizophrenia, it usually begins in early adulthood (generally earlier than mood disorders), but like mood disorders, it is much more common in women than in men, and women are much more likely than men to have their diagnosis switched from schizophrenia to schizoaffective patients diagnosed as having schizoaffective disorder, when compared with former patients diagnosed as having either schizophrenia or mood disorder one year after discharge from the hospital, were intermediate in their capacity for work, social functioning, and rate of rehospitalization. Their social life and rehospitalization rate were similar to those of patients with schizophrenia; in capacity to work they were more similar to patients with mood disorder.

A. An uninterrupted period of illness during which, at some time, there is either a Major Depressive Episode, a Manic Episoe, or a Mixed Episode concurrent with symptoms that meet Criterion A for Schizophrenia.
Note: The Major Depressive Episode must include Criterion A1: depressed mood
B. During the same period of illness, there have been delusions or hallucinations for at least 2 weeks in the absence of prominent mood symptom
C. Symptoms that meet criteria for a mood episode are present for substantial portion of the total duration of the active and residual periods of the illness.

GENETICS OF SCHIZOPHRENIA AND AFFECTIVE DISORDER - AN OVERLAP
Shashjit Lal Varma , MBBS, MD, PhD, Consultant Psychiatrist, Grampians Psychiatric Services, Ballarat Base Hospital, PO Box - 577, Ballarat 3353, Victoria, Australia Introduction Numerous studies have established the familial aggregation of schizophrenia and that familial factors that predispose to schizophrenia also predispose to other schizophrenia related disorders. Kraepelin proposed that schizophrenia and manic-depressive psychosis were two distinct disorders but since then with abundance of literature on this subject pouring in, the theory is being challenged quite frequently. The number of studies which support the Kraepelinian view of psychosis exceed those which do not. The proponents of continuum theory propose a continuum from unipolar to bipolar disorder to schizoaffective psychosis and right up to schizophrenia. Another hypothesis proposed is that schizophrenia and bipolar affective disorder are two distinct entities and that unipolar and schizoaffective disorders are expressed in the relatives of patients in both the categories (Gershon et al, 1988). Family Studies Rudin (1916) reported that although the risk for schizophrenia for an individual was high (6.2%) if they had a close relative with schizophrenia, it was higher if the ill relative had a non-schizophrenic (mostly affective) psychosis (8.2%) or alcoholism (7.8%). He also found almost equal numbers of relatives with affective disorder and relatives with schizophrenia in the families of his schizophrenic probands. On the other hand, among the psychotic relatives of Odegaard's (1972) manic depressive probands, 19% were diagnosed as schizophrenic, indicating that the co-occurrence may run in both directions.

Study participants will include either 1) persons with Schizophrenia or Schizoaffective Disorder, and their two biological parents OR 2) persons with Schizophrenia or Schizoaffective Disorder, and a sibling (brother or sister) with the same or similar disorder. Brief description of study: We are studying the genetic material of family members with and without a history of Schizophrenia or Schizoaffective Disorder. Relevant family members (e.g., parents of the affected person and/or an affected sibling) are first recruited through the family member expressing an interest in this study. When the willingness of the full family trio (affected child and their biological parents) or sibling pair to participate is ascertained, affected family members will be asked questions regarding their physical and mental health history, and past and current psychiatric symptoms. Interviews can be conducted over the telephone or in person. Kits for obtaining blood samples can be mailed. The study will pay all reasonable expenses associated with obtaining blood samples (doctor fees, shipping expenses).

    Schizoaffective Disorder
    Schizoaffective Disorder is a disturbance in which a mood episode and the active phase symptoms of schizophrenia occur together and were preceded or are followed by at least two weeks of delusions or hallucinations without prominent mood symptoms. The mood episode may be depressive, manic, or mixed. The duration of the depressive episode must last at least two weeks; however the manic or mixed episode must be of at least one week in duration.
    The symptoms of this disorder may occur in a variety of patterns. The following is an example: an individual may have pronounced auditory hallucinations and persecutory delusions for two months before the onset of a prominent major depressive episode. The psychotic symptoms and the full major depressive episode are then present for three months. Then, the individual recovers completely from the depressive episode, but the psychotic symptoms persist for another month.
    Individuals with this disorder may exhibit poor occupational or scholastic functioning, a restricted range of social contact, difficulties with activities of daily living, and an increased risk of suicide.     What is schizoaffective disorder?
    Schizoaffective disorder is a mental illness that produces symptoms of both mood disorders and schizophrenia. Mood disorders include conditions such as depression, in which you feel very sad, and bipolar disorder, in which you may feel depressed at times and very happy or irritable at other times. Schizophrenia is a serious mental illness in which you may hear, see, or feel things that aren't there (hallucinations) and have false beliefs (delusions), like believing that you have special powers nobody else has. About one person in 100 has schizoaffective disorder.
    How does it occur?
    The cause of schizoaffective disorder is unknown. It seems to occur more in some families, and stress may trigger an episode.
    What are the symptoms?
    Symptoms are those of both schizophrenia and mood disorders and may include:
    • hallucinations
    • delusions
    • not taking care of yourself (for example, not bathing or grooming)
    • speaking in a way that makes no sense to others
    • thoughts that race from one idea to the next
    • feelings of sadness, guilt, hopelessness, and anxiety

    Schizoaffective Disorder Schizoaffective disorder is a condition in which there are symptoms of mood (affect) disorders, like depression or mania, and symptoms of schizophrenia present at the same time, or within a few days of each other. Usually the diagnosis of Schizoaffective disorder is given when the schizophrenic symptoms are more pronounced; if the mood disorder is more pronounced the condition is more often called depressive or manic psychosis. There are many different definitions of these conditions, and it can be difficult for the doctors to settle on a diagnosis. Schizoaffective disorder is quite a rare diagnosis. In one study of 936 people, who had received psychiatric treatment in hospital on several occasions, 51 (5%) were diagnosed with Schizoaffective disorder. Some people believe that this low level of diagnosis does not represent the true level of the condition, and that many people are given a mis-diagnosis instead of a diagnosis of Schizoaffective disorder. More recently, in another study, a quarter of all psychotic patients were eventually diagnosed with Schizoaffective disorder.
