WHAT YOU SHOULD KNOW
Schizoaffective disorder is a hybrid form of mental illness that combines the problems of schizophrenia with those of a mood disorder. As with schizophrenia, victims lose touch with reality. However, schizoaffective disorder is more likely to wax and wane, much as depression and mania tend to run in cycles. The condition affects more women than men.
Causes
An imbalance in the brain's chemical messengers is the most likely culprit, but its exact nature-and the reason for it-are still unclear. Stress alone will not trigger this illness, though it can make the symptoms worse. The problem is more likely to develop if you have a family member with a mood disorder.
Signs/Symptoms
The disorder usually comes on gradually. One of the most telling signs is a tendency for the victim's emotional reactions to be out of synch with the situation-or lacking entirely. Other symptoms include:

The term schizoaffective disorder refers to an uninterrupted period of illness during which the patient is diagnosed with a major mood disorder (mania, depression, or mixed affective state) while simultaneously experiencing at least two of the characteristic psychotic symptoms for schizophrenia, such as delusions, hallucinations, disorganized speech, disorganized behavior. To qualify for this diagnosis, the patient must be found to have at least one of the above psychotic symptoms for at least two weeks in the absence of mood symptoms. Patients are sometimes diagnosed as having schizoaffective disorder because they experience psychotic symptoms during an affective episode. For example, patients who are depressed may experience delusions, such as irrationally believing that they are to blame for events not related to them, or hallucinations, such as hearing voices telling them to harm themselves. Manic patients may experience grandiose delusions; they report a variety of psychotic experiences such as hearing voices telling them they're invincible or developing religious or technical insights inconsistent with the cultural norms. For the most part, psychotic experiences during an affective episode are "mood congruent," meaning that they are consistent with the underlying mood: depressed patients experience delusions or hallucinations of a depressive nature (focused on self-reproach or self-harm), while manic patients experience grandiose beliefs (that they hold the insights to Western society or are the greatest of the greatest). Less common in affective disorders are "mood-incongruent" psychotic experiences (either neutral or inconsistent with the prevailing mood) such as hearing voices commenting on one's actions. Patients who have only mood-congruent psychotic symptoms, and have those only during an affective episode, are not considered to have schizoaffective disorder. Also, 60 to 70 percent of all patients with schizophrenia have episodes of depression within a flareup of their schizophrenic illness. It is not uncommon to see these patients improperly diagnosed with schizoaffective disorder.

These are episodic disorders in which both affective and schizophrenic symptoms are prominent within the same episode of illness, preferably simultaneously, but at least within a few days of each other. Their relationship to typical mood (affective) disorders and to schizophrenic disorders is uncertain. They are given a separate category because they are too common to be ignored. Other conditions in which affective symptoms are superimposed upon or form part of a pre-existing schizophrenic illness, or in which they coexist or alternate with other types of persistent delusional disorders, are classified under the appropriate category. Mood-incongruent delusions or hallucinations in affective disorders do not by themselves justify a diagnosis of schizoaffective disorder. Patients who suffer from recurrent schizoaffective episodes, particularly those whose symptoms are of the manic rather than the depressive type, usually make a full recovery and only rarely develop a defect state.

Schizoaffective Disorder Schizoaffective disorder symptoms look like a mixture of two kinds of major mental illnesses that are usually thought to run in different families, involve different brain mechanisms, develop in different ways, and respond to different treatments: mood (affective) disorders and schizophrenia. Q. My adult son has schizoaffective disorder. He has been on almost every antipsychotic medication, but none has eliminated the voices sufficiently to permit him to function. This past year he was taken off Olanzapine and put on Clozaril. Four months later he was taken off it, complaining that the voices were too much and he was suicidal. To an outside observer, it appeared he was functioning at a higher level on the Clozaril, despite his complaints. Can you tell me what drugs are being combined to provide relief for symptoms not controlled by Clozaril and what drugs are used to combat side effects like drooling, weight gain, tremors, and excessive sleeping? DSM-IV Diagnostic Criteria Schizoaffective disorder is an illness which meets the criteria for schizophrenia and concurrently meets the criteria for a major depressive episode, manic episode, or mixed episode schizoafective The illness must also be associated with delusions or hallucinations for two weeks, without significant mood symptoms. Mood symptoms must be present for a A general medical condition or substance use is not the cause of symptoms. Symptoms of schizophrenia are present, but also associated with recurrent or chronic mood disturbances. Psychotic symptoms and mood symptoms occur independently or together. If manic or mixed symptoms occur, they must be present for one week and major depressive symptoms must be present for two weeks. Epidemiology The lifetime prevalence is under one percent. Mental Health Disorders Psychoses Schizoaffective Disorder People with schizoaffective disorder have symptoms of schizophrenia, combined with those of a major mood disorder. Hallucinations delusions , or other positive symptoms of schizophrenia must be present for a period of at least 2 weeks without a mood disorder, but the mood symptoms must be present for a "substantial" fraction of the period of illness, in order for a person to receive this diagnosis. There are two subtypes of schizoaffective disorder, bipolar and depressive; the bipolar type, as the name implies, is manic during at least part of the illness, but the depressive type is not. The risk of suicide is considerable for people with this diagnosis ; suicide is not uncommon for people diagnosed with any type of schizophrenia, with the greatest risk during the first 6 months after a person is released from the hospital, but the presence of depressive mood probably increases the risk. During the period after release from the hospital, observation and support are especially critical. The motivation for suicide is probably connected with the realization that the illness is likely to be permanent. During periods of at least partial remission the person is likely to be most acutely aware of this.

Resources Definition Schizoaffective disorder is a mental illness that shares the psychotic symptoms of schizophrenia and the mood disturbances of depression or bipolar disorder. Description The term schizoaffective disorder was first used in the 1930s to describe patients with acute psychotic symptoms such as hallucinations and delusions along with disturbed mood. These patients tended to function well before becoming psychotic; their psychotic symptoms lasted relatively briefly; and they tended to do well afterward. Over the years, however, the term schizoaffective disorder has been applied to a variety of patient groups. The current definition contained in the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders IV DSM-IV ) recognizes patients with schizoaffective disorder as those whose mood symptoms are sufficiently severe to warrant a diagnosis of depression or other full-blown mood disorder and whose mood symptoms overlap at some period with psychotic symptoms that satisfy the diagnosis of schizophrenia (e.g. hallucinations, delusions, or thought process disorder). Causes and symptoms The cause of schizoaffective disorder remains unknown and subject to continuing speculation. Some investigators believe schizoaffective disorder is associated with schizophrenia and may be caused by a similar biological predisposition. Others disagree, stressing the disorder's similarities to mood disorders such as depression and bipolar disorder (manic depression). They believe its more favorable course and less intense psychotic episodes are evidence that schizoaffective disorder and mood disorders share a similar cause.
