The corner stone when treating schizphrenia is with no doubt medication with so called antipsychotic drugs, or neuroleptics. This group of medications consists of a number of chemical substances, and is divided into several subgroups. These have all shown to be very efficient in reducing the psychotic symptoms in schizophrenia, but vary in terms of the side effects they may cause. Substance Abuse Substance abuse is a common concern of the family and friends of people with schizophrenia. Since some people who abuse drugs may show symptoms similar to those of schizophrenia, people with schizophrenia may be mistaken for people "high on drugs.” While most researchers do not believe that substance abuse causes schizophrenia, people who have schizophrenia often abuse alcohol and/or drugs, and may have particularly bad reactions to certain drugs. Substance abuse can reduce the effectiveness of treatment for schizophrenia. Stimulants (such as amphetamines or cocaine) may cause major problems for patients with schizophrenia, as may PCP or marijuana. In fact, some people experience a worsening of their schizophrenic symptoms when they are taking such drugs. Substance abuse also reduces the likelihood that patients will follow the treatment plans recommended by their doctors.

Observer Sunday March 18, 2001
Tiny mutations in our ancestors' brain cells triggered mankind's takeover of the world 100,000 years ago. But these changes also cursed our species to suffer from schizophrenia and depression. Horrobin - who is medical adviser to the Schizophrenia Association of Great Britain - argues that the changes which propelled humanity to its current global ascendancy were the same as those which have left us vulnerable to mental disease. 'We became human because of small genetic changes in the chemistry of the fat in our skulls,' he says. 'These changes injected into our ancestors both the seeds of the illness of schizophrenia and the extraordinary minds which made us human.'

Schizophrenia is a mental disorder that severely impacts how 2.5 million Americans think, feel, and act. It is a disorder that makes it difficult for a person to tell the difference between real and imagined experiences, to think logically, to express normal emotional responses or to behave appropriately in social situations. Schizophrenia can be draining on both the person with schizophrenia and their families. People with schizophrenia often have difficulty functioning in society, at work and in school. Family members may have to help out financially and make sure that medication is taken as prescribed. No cure for schizophrenia has been discovered, but with proper treatment, many people with this illness can lead productive and fulfilling lives.

prevalence chart
prevalence chart

Whatever it actually is, the statistics have historically been pretty grim.
  • 1% of the population develops the illness
  • 40% attempt suicide and 10% die by suicide
  • 50% develop a substance abuse problem in their lifetime
  • 50% can't accept or don't believe they have a psychotic disorder
  • most people stop taking their medication at least once
  • 70% of those who go off their medication have to be rehospitalized within two years because they are in psychosis
  • 33% of the homeless have an untreated psychotic disorder
Essentially, the longer you are in psychosis, the more likely you will become homeless or incarcerated in jail if you are not hospitalized and/or treated with antipsychotics. Untreated schizophrenia always leads to psychosis. It's just a question of how long it takes. There is a fairly strong genetic component but in genetically identical twins, the probability of both developing schizophrenia if one does is only 50%. The genetics of schizophrenia is thought to involve multiple genes but if there is a family history the risk of developing it increases. Many psychiatrists believe schizophrenia could be four or five different illnesses that look alike. There is a lot of variation between individuals. A common differentiation is whether onset is fairly sudden or insidious (gradual). The brain is still basically an uncharted universe. At a conference I went to in 1996 the following slide, which I don't understand, was shown:

Schizophrenia Definition Return to top Schizophrenia is a serious brain disorder. It is a disease that makes it difficult for a person to tell the difference between real and unreal experiences, to think logically, to have normal emotional responses to others, and to behave normally in social situations. Causes, incidence, and risk factors Return to top Schizophrenia is a complex and puzzling illness. Even the experts in the field are not exactly sure what causes it. Some doctors think that the brain may not be able to process information correctly. Genetic factors appear to play a role, as people who have family members with schizophrenia may be more likely to get the disease themselves. Some researchers believe that events in a person's environment may trigger schizophrenia. For example, problems during intrauterine development (infection) and birth may increase the risk for developing schizophrenia later in life. Psychological and social factors may also play some role in its development. However, the level of social and familial support appears to influence the course of illness and may be protective against relapse.

    http://www.world-schizophrenia.org/
Schizophrenia strikes 1% of the world's population at some point in their lives. It affects people from all walks of life, usually young people between the ages of 15 and 30. Not everyone will experience the same symptoms, but some symptoms, such as hearing voices, are common to many. Schizophrenia alters the way people think and feel, so that perceptions may be changed and thinking can be disturbed. This is very disabling for them and very distressing for their families. Other serious mental illnesses, including bipolar disorder, schizo-affective disorder and major depression, are also very debilitating.
Fortunately, research is bringing about a new era in treatment and understanding of these disorders. While cause and cure are still elusive, substantial strides have been made in the development of medications to treat these conditions. With modern medical treatment, opportunities for rehabilitation, and the support of family and friends, these diagnoses need not be as feared as they were in the past.
The World Fellowship for Schizophrenia and Allied Disorders is the only worldwide international organization dedicated to lessening the problems faced by people with serious mental illness and their families. Our members include family support and advocacy organizations in over 60 countries. We disseminate information to people with mental illness and their families on the symptoms, the treatments and the ways of coping. We act as a meeting point for family organizations, putting people in contact with their closest self-help group. Finally, we work with mental health professionals and international organizations to advocate for families and their loved ones on the key issues affecting their lives.
