This is a discussion of congenital scoliosis, NOT idiopathic scoliosis. I was born with hemi-vertebra and vertebra which failed to segment on one side. This problem occurred in the developing fetus. That's what congenital scoliosis is. It was nothing my mother did to cause it. It was nothing I did to cause it. It just happened. The picture below is of two t-spine x-rays taken three years apart. I held the x-rays up to a window and then snapped a picture. On the left is an x-ray of 1997 when I was age 51. On the right is an x-ray of 1994 when I was age 48. The major curve is about 65-degrees in 1994 and about 85-degrees in 1997. The other compensating curves are not pictured. This is the way I have appeared for my entire life. There were no treatments for congenital scoliosis when I was a child. My scoliosis was first detected by a casual observer when I was 5 years old. It was already as bad as it appears in these x-rays. The congenital malformations are most obvious at the T4-T8 levels. The vertebra in this area are not distinctly visible on the x-ray. Notice the ribs just to the right of the spine. It appears as if these were joined as one rib which eventually split into individual ribs further out from the right side of the spine. Without these abnormalities, there is no scoliosis at all. Congenital scoliosis is about five percent of all scoliosis cases. Congenital scoliosis is really bad developmental luck. No one is to blame. I can't stress this enough. I have read e-mail from parents who thought they were to blame for their child's scoliosis. No one is to blame for this bad developmental luck. Could have been much worse luck with more to overcome. Could have been blind, could have been deaf, could have been any number of worse things in addition to scoliosis.

Everyone's spine has curves . These curves produce the normal rounding of the shoulder and the sway of the lower back. A spine with scoliosis has abnormal curves with a rotational deformity . This means that the spine turns on its axis like a corkscrew. Compare the more subtle curve of the normal spine to the severe curvature of the scoliotic spine. Scoliosis is a curvature of the spine which may have its onset in infancy but is most frequently seen in adolescence . It is more common in females by a 2:1 ratio. However, when curves in excess of 30 degrees are evaluated, females are more frequently affected by a ration of approximately 8-10:1. The cause of the most common form of scoliosis idiopathic scoliosis is unknown, but there are certainly hereditary factors that are present. Scoliosis causes shoulder, trunk and waistline asymmetry . In mild forms, the condition may be barely noticed; whereas in severe forms there is significant disfigurement, back pain and postural fatigue, and it may be associated with heart failure. Fortunately the majority of scoliosis cases need only close follow-up to watch for worsening of the curve. Some cases require more aggressive treatment which could include surgery (see below).

Scoliosis is the most common deformity of the spine. Other common spinal deformities include kyphosis and lordosis, however these deformities are much less common. The diagnosis of scoliosis is made when there are two abnormalities of the spine:
  • Lateral curvature: A lateral curvature of the spine means that the vertebral column bends from side-to-side. If looking at an individual from behind while they bend to touch their toes, one will notice an S-shaped curvature of the spine.
  • Rotation: The rotational deformity is usually subtle, but is always present in a true scoliosis deformity. The rotational aspect of scoliosis causes a twisting deformity of the vertebral bodies.

Scoliosis is, surprisingly, quite common. About three or four children per thousand of the general population will need specialist supervision, and about one child in every thousand will need hospital treatment usually for major corrective surgery with the insertion of a (permanent) metal rod, sometimes followed by a period in a plaster jacket or brace . Scoliosis can develop at any time during childhood and adolescence. It might develop in infancy, in which case there is a good chance of it resolving with growth. Progressive (worsening) curvatures that begin before the age of 5 years are potentially serious, and some of these ' early onset ' curvatures may need surgical treatment. 'Late onset' scoliosis developing after the age of five is less serious, but some will need surgery and all need to be seen by a scoliosis specialist who can keep an eye on the situation and give some idea about the future. However, at present, in the majority of cases no firm prediction can be made, so the specialist will probably want to see the child every four to six months until the end of growth at the age of 16 to 18. Severe deformity, which is uncommon nowadays because of modern treatment, can lead to damaged lung function, or disability in middle age. The lungs are usually affected only in people with untreated early onset thoracic scoliosis, when the upper (chest) part of the spine is curved. Scoliosis
patient with thoracic scoliosis, convex to the right, bridged by a Harrington rod and bone graft along the concave side of the spine If scoliosis is neglected, the curves may progress dramatically, creating significant physical deformity and even cardiopulmonary problems with especially severe curves. Currently, scoliosis is treated successfully by special braces, electrical stimulation, surgery, or by combinations of these three techniques. Generally, scoliosis is treated by orthopedic surgeons with special training in spinal problems. However, radiographic scoliosis examinations are ordered by a wide variety of other physicians, most of whom look toward the radiologist as the local musculoskeletal expert, after the orthopedist. Therefore, one must know how to read these films and how to dictate a coherent and helpful interpretation of them. A sample dictation of a typical examination is shown below: Standing AP and lateral views of the entire spine demonstrate an arcuate thoracolumbar scoliosis with a rightward convexity. No associated vertebral abnormalities are noted. Using the Cobb technique, and measuring from the top of the T9 and the bottom of the L3 vertebral bodies, this angle measures approximately 27 degrees. The apex of the curve is at the T12 vertebral body and demonstrates grade 3 out of 4 rotation to the right. Lateral bending films show persistence of the curvature with bending to the right and increased curvature with bending to the left. This indicates that the thoracolumbar curve is structural and major. The iliac apophyses are complete along the iliac crests, but have not yet fused with the ilium, indicating that the patient has not yet reached skeletal maturity.

Causes, incidence, and risk factors Return to top There are three general causes of scoliosis: congenital, usually related to a problem with the formation of vertebrae or fused ribs during prenatal development; neuromuscular (poor muscle control or muscular weakness or paralysis due to diseases like cerebral palsy muscular dystrophy , spina bifida and polio ); and idiopathic (of unknown cause), which appears in a previously straight spine.
    The idiopathic form in adolescents is the most common and may have a genetic predisposition. Most cases occur in girls and curves generally worsen during growth spurts. There are also infantile and juvenile forms that are less common and affect a similar number of boys and girls. Scoliosis may be suspected when one shoulder appears to be higher than the other, or the pelvis appears to be tilted. It is often unnoticeable to an untrained observer, however.
