Self Injury: Help is Available Self injury is the act of deliberately harming your own body. Self injuries are self inflicted wounds that are severe enough for tissue damage (such as scarring) to result(3). Are you hurting yourself? Do you intentionally try to harm yourself? Do you cause this harm to yourself as a way of dealing with unpleasant or overwhelming emotions, obsessive thoughts or disassociation? Do you think about harming yourself even when you are relatively calm and are not doing it at that moment(Martinson)? Do you find yourself doing these things, if so, you are not alone...help is available. Self-injury exists in many forms. According to the Bristol Crisis Service for Women, the most common form of self injury is cutting, usually minimally, but sometimes deeply. People may also burn themselves, punch themselves or hit their bodies against something, scratch, and interfere with wound healing. Some people pick their skin or pull out their hair. The Bristol Crisis Service for Women stated that self-injury is more widespread than is generally realized. All sorts of people self-injure. Although all genders self injure, self-injury is more common among women. Women from all socioeconomic classes, ethnicities, sexual orientations and ages are capable of becoming self injurers. Self injurers tend to strongly dislike themselves, are chronically angry at themselves, suppress their anger, do not plan for the future, exert depressive or suicidal behavior, and suffer chronic anxiety. According to Debra Martinson, women are not socialized to express violence externally thus women tend to vent on themselves. "Men act out. Women act out by acting in."

Peer Review Status: Internally Peer Reviewed What are the basic approaches to accessing individuals who are displaying self-injury? In terms of assessing those who display self-injury, we have learned that the reason for self-injury, what we refer to as the function, is more important than the form or the appearance of the behavior itself. Therefore, when an individual comes to our clinic, we conduct an assessment called a functional analysis. A functional analysis literally means that we are assessing the function that the behavior serves the individual. There are two major categories of functions: social functions refer to behavior that is maintained by reinforcers in the environment. The most common reinforcers are gaining attention, preferred items, and escape from demands. The second category is biologic functions, and these refer to behaviors that occur in response to biologic events such as pain. What types of clinical services do you offer for individuals who display severe behavior problems?

Who can benefit? The Self-Injury program provides treatment for individuals who repeatedly harm themselves or who have persistent suicidal feelings. The individual who lack emotion regulation and distress tolerance skills. Research suggest that the typical self-injurer is a woman in her 20’s or 30’s who impulsively cuts her wrists or arms, burns her skin or overdoses on medication. Self-injurers usually have injured themselves on many occasions. They also may have attempted suicide. It is not uncommon for patients to have co-morbid mood disorders such as depression or dysthymia. They may have other forms of mental illness, as well. Most common are anorexia or bulimia, post-traumatic stress disorder, dissociation, alcohol or substance dependence, and borderline personality disorder. As children, they may have experienced trauma such as sexually or physically abused. What is the program?

The text below is taken from a booklet produced by YPAS to highlight the issues of Self-Injury to professionals across a variety of working areas. The aim is to help bring some understanding about self-injury and to raise awareness of the male self-injury group run by YPAS. A printed copy is available (while stocks last) from YPAS. Development of the group There are a number groups set up to deal with women who self-injure. This group was set up to respond to the lack of services available to young men who self-injure. Young men maybe more reluctant to access services and explore their emotions, and therefore find themselves isolated. This group aims to address some of these issues and meet the needs of young men and explore alternative ways of coping. This booklet has been put together to reach workers and organisations across the city who may come in contact with young men who self-injure. It is hoped this will inform and assist them in considering any referrals. Due to the lack of understanding of self-injury, people who self– injure are often dismissed as nuisances, attention seeking, manipulative, a danger to others, disturbed or mad, and is misconceived as failed suicide attempts. This booklet aims to give people a greater understanding of what self-injury is. A greater awareness will hopefully inform practice, dispel the myths and enable people to offer greater acceptance and therefore be less judgemental.
