My husband Bill has Shy Drager Syndrome/Multiple Systems Atrophy. For the past year, he has been troubled with a dry cough, mostly at night. Lately he has been coughing during the day also. The doctor has checked him and says his lungs are "clear as a bell". We thought his arthritis medicine Daypro might be causing it, because that is listed as a possible side effect; however, after discontinuing it, he still has the cough. It is not only annoying at night but causes severe problems during the day.

The ThreeTypes of Multi systems Atrophy (MSA) Parkinson's-type
, which may include symptoms of Parkinson's disease such as slow movement, stiff muscles and mild tremors. Cerebellar type , characterized by loss of balance and tendency to fall, and Combination type which may include symptoms of both types (1). Current Western Medicine methods of treatments are limited to controlling symptoms.
Anti-Parkinson medication such as L dopa, may be helpful but it can also lower blood pressure even more. To relieve low blood pressure high level of salt and fluids is recommended. Prognosis: Patients usually die within 7 to 10 years after the onset of symptoms. Pneumonia is the most common cause of death, although irregular heartbeat may be responsible for death in some patients.
Case Study
In 1996, the drug prescribed for him was the combination of Levo-dopa 25mg, Carbidopa 100 mg. and Fludrocortisone. He developed severe hallucinations as a side effect of these drugs within three weeks, yet he was kept on this regimen for three years. By the time I started treatments on him these hallucinations were so frequents and severe that it became one of the most persistent and debilitating problems.

Drager, born 1917, American neurologist), (SDS), consists of Parkinson's disease combined with autonomic nervous system failure (orthostatic hypotension), urinary incontinence, and inability to sweat). In some cases, the development of dementia and a motor neurone disease component may further complicate the condition. Neuronal loss in the substantia nigra and the intermediolateral cell column of the spinal cord, accompanied by gliosis, commonly occurs. MR may be negative or show putaminal atrophy and signal changes when associated to striatonigral degeneration in multiple system atrophy

The Shy – Drager syndrome is a degenerative neuralgic disorder involving the cerebellum and brain stem. The disease is characterized by autonomic dysfunction, and most patients have symptoms of urinary incontinence related to detrusor hyperreflexia and external sphincter incompetence. Imaging is rarely requested, and when it is, it is mostly to look at the complication related to the bladder dysfunction. The finding to look for is vesicle urethral reflux and the ability of the bladder to void the post-void residual. Cystography is considered the most appropriate choice of examination in the evaluation of Shy – Drager syndrome of the bladder. The best imaging modality for the evaluation of the extent of cancer of the cervix, including local regional extent, as well as presence of lymph node metastases, is MRI. Magnetic resonance is statistically significantly better than CT in the evaluation of parametrial invasion, and helps in treatment decisions between surgery and radiation therapy.

In the spring of this year there were only two items of pressing concern to Johnny Cash fans: his health and our prospects for more of his music. He addressed both issues by telephone from Los Angeles, where he and producer Rick Rubin were working on a new album.
"I'm doing better," he said. His speaking voice sounded reasonably strong. "I haven't been in the hospital now since, um … " an ironic chortle " … October.
"I had pneumonia about twice since I last saw you [in 1999], and it's pretty much just devastated my lungs. I'm having a really hard time getting any keeper vocals on my record. But I've got a whole week to work on it, and maybe it'll open up just a little bit. Yeah, pneumonia just about shut my lungs down. I'm doing all right now, though. I've been feeling great the last three or four months."

Question : My father was diagnosed with Parkinson's about 3 years ago. All Parkinson's medications made him feel worse instead of better. I attributed his autonomic symptoms (hypotension, dizziness, flushed feeling, anxiety, etc.) as expected with mitral valve prolapse's dysautonomic symptoms. This week his neurologists changed his diagnosis from Parkinson's to Shy-Drager Syndrome. The men in his family have a predominance of neurological problems. My questions are: 1. Is there a relationship between Shy-Drager Syndrome and Mitral Valve Prolapse (Dysautonomial)? 2. Does Shy-Drager run in families, as does Mitral Valve Prolapse? 3. What is the prognosis for Shy-Drager? Dr. Chelimsky : Your question is an excellent one. The usual thought is that Shy-Drager (SD) and mitral valve prolapse (MVP), while both autonomic disorders in their own right, do not have much to do with one another. SD is a progressive degeneration of the autonomic and motor neurons, and autonomic testing will demonstrate LOSS of most autonomic reflexes as time goes on. On the other hand, MVP, although sometimes associated with similar symptoms such as dizziness, fainting and bladder and bowel problems, generally demonstrates EXAGGERATION of the autonomic reflexes. Thus, the two should be relatively easy to distinguish. This is more than just an academic issue, because MVP is usually non-progressive, while SD progresses. Also the treatment strategies differ significantly.
