    Diagnosis
    LOW BACK SLIPPED DISK (Herniated Lumbar Disk) Definition Abnormal movement of the material (disk) that helps support the bones of the lower back. Details Disks are found between the bones of the back (vertebrae). They can at times push out of their normal position (herniate) and then cause symptoms. Causes A slipped disk or herniation usually results from an twisting injury to the low back or may be due to several small sprains. Diagnosis Typically, pain and spasm begin either immediately after an injury or develop within the 24 hours. The pain is made worse by activity and usually improves with rest. The pain may start in the lower back but also can radiate to the legs. This can be felt as a sharp or shooting pain down the leg into the knee, ankle or foot. It may be accompanied by either numbness or weakness. Initially x-rays may be taken to evalute the back. An MRI Scan or CT scan may also be ordered depending on the symptoms and physical examination.

    Treatment Nonoperative:
    Initially, icing and rest coupled with anti-inflammatory medication are important to relieve pain and spasm. Other medications may also be needed. This phase should be followed by education about proper lifting techniques and a back rehabilitation program. Stretching and strengthening of back, leg and abdominal muscles are important components of this rehabilitation program.

Hazel Dinger Dear Hazel: Treatments other than rest and early activity as tolerated include physical therapy, back exercises, traction and chiropractic manipulation; no treatment has proven efficacy, and some (such as chiropractic manipulation) may occasionally be dangerous if wrongly used.

    WHAT YOU SHOULD KNOW
    A slipped diskalso known as a herniated or prolapsed diskoccurs when the ligaments that support and surround the flat, platelike cushions between the vertebrae in the spine begin to weaken. A back strain may cause one of these ligaments to break, allowing the disk material to squeeze out from between the vertebra and press on nearby nerves. Slipped disks become increasingly common with age.
    Causes
    The ligaments may give way under the stress of regular lifting, or tear open during a sudden injury. The odds of this happening increase when you are overweight.
    Signs/Symptoms
    A slipped disk in the lower back can put pressure on the sciatic (sigh-AT-ik) nerve, causing sciaticasharp, shooting pain from the buttock down the back of the leg to the foot. You may notice weakness, numbness, or loss of muscle strength in the affected leg. If a disk slips in the neck, the pain will get worse when you move your neck. It may also extend across the shoulder or down one arm. Your arm may feel weak or numb, or may lose muscle strength.

    The disks are protective shock-absorbing pads between the bones of the spine. Although they do not actually "slip," a disk may split or rupture. This can cause the disk to fail, allowing the gel to escape into the surrounding tissue. The leaking jellylike substance can place pressure on the spinal cord or on a single nerve fiber and cause pain either around the damaged disk or anywhere along the area controlled by that nerve. This condition is also known as a herniated, ruptured, prolapsed, or, more commonly, "slipped" disk. The most frequently affected areas are the low back, but any disk can rupture, including in the neck.

    Treatment: The mainstay of treatment for herniated disks is an initial period of rest with pain and anti-inflammatory medications, followed by physical therapy. Under this regimen, over 95% of people will recover and return to their normal activities. A small percentage of people do need to go on and have further treatment which may include steroid injections or surgery. MEDICATIONS
    For people with an acute herniated disk caused by some sort of trauma (like a car accident or lifting a very heavy object) and immediately followed by severe pain in the back and leg, narcotic pain relievers and non-steroidal anti-inflammatory medications (NSAIDs) will be prescribed. If there is also an element of back spasm, anti-spasm drugs, also called muscle relaxants, are usually given. On rare occasions, steroids may be administered either by pill or directly into the blood with an intervenous line (IV). Long-term pain control is usually limited to NSAIDs, but occasionally narcotics are used as well (if the pain does not respond to NSAIDs).
