F or years, social anxiety disorder, also known as social phobia, has been underrecognized and undertreated. That situation is beginning to change, however, because recent research has shown that the disorder is highly prevalent, chronic in its untreated course, often associated with comorbid mental and substance-related problems, and capable of disabling those who have it. We now know more about recognizing social phobia and the types of interventions to which it is responsive. Clinical Features Social phobia is an intense, irrational and persistent fear of being scrutinized or negatively evaluated by others. For these people, their excessive anxiety can dramatically affect quality of life. This is especially so during the holidays, when added stresses can trigger an increase in panic attacks or acute social anxiety. Mental health specialists with the University of Michigan Health System encourage all of us to slow down and enjoy the holiday season, and for those individuals suffering from anxiety disorders to seek professional help. Dear Alice, Dear Reader About 5 million Americans have some form of social anxiety disorder, also known as social phobia. While everyone experiences nervousness or fear in social situations to a certain extent, those with social anxiety disorder are debilitated by their fear. They may have a constant, intense, and chronic fear of being watched and judged by others, or be terribly afraid of being embarrassed or humiliated by ways in which they behave. As you have described, this fear can prevent people with social anxiety disorder from doing everyday activities, such as going to school or work, and can become so much of a focus that they severely limit their socializing.
In the meantime, it might make sense for you to start by seeking assistance from people and places that will allow you to do so without your parents knowing. Depending on your age and school situation, you can explore some of the following:
  • speaking with a school nurse or guidance counselor reaching out to a dean or another school administrator who knows you or works closely with students consulting with your pediatrician or another health care provider or clinic staff member who has known you for a while
It may not feel like it, but you are by no means the only person who struggles with these issues. While it may feel embarrassing to ask for help, health care and school personnel are trained to help you find the support and resources you need, so that you can figure out what's going on and start feeling better.

This site provides an electronic format of the Cognitive-Behavioral Therapy (CBT) tape series produced by the Social Anxiety Institute. Their web site can be found here: www.socialanxietyinstitute.org Currently only tapes 1 and 2 are covered, and the relevant handout material for those tapes. (There are 20 tapes in the series). If you like what you find here please go to their site and purchase the full set. You'll get all 20 tapes and a professionally bound handout booklet all contained in a nice folder. The links furthur down the page are audio files in MP3 format. There are many freely available software products available for playing these files. Later versions of windows have software to do this as part of their distribution and you probably already have one installed. (Windows XP for example has this.) If you don't already have one here is a small list of popular freely available players:

The files are highly compressed and hence are of lower quality than you would get from the original tapes. The advantage of this is that they can be downloaded in a reasonable time and in fact are so higly compressed that most users with a standard modem can play the files immediately without having to download the whole file first. If you already have an MP3 player installed you can probably simply click on the links below and the files will play almost immediately. If you wish to save the file for later use then right-click on the link and choose "save" or something similar.

This fear is recognized as irrational by the individual; nevertheless, the fear of judgment in social situations persists. People who have social phobia usually find it difficult to be introduced to other people ("I wonder what they're thinking about me; I'll bet they don't like me"), attracting attention to the self or being the center of attention, being watched while doing something ("They don't like what I'm doing; I must be making a fool of myself"), and feeling insecure and self-conscious in most public settings. What is Social Anxiety Disorder? Social anxiety disorder or social phobia (SD) is fear of social situations. Typically people are afraid they will be embarrassed or humiliated by showing signs of anxiety, saying or doing something which other people will react negatively to, or not knowing what to say or do. Common signs of anxiety are blushing, trembling or shaking, or losing track of conversation (feeling that your mind goes blank). People with social anxiety disorder often worry about social interactions and may go to considerable trouble to avoid them. Situations which are commonly feared include talking in a group, speaking in front of a large or small group, having a conversation (especially with someone who is not well known to them), meeting someone new, speaking to a person in authority, and dating situations. Some people experience difficulty using the telephone, eating or drinking when other people are watching, or participating in activities where there may be an audience such as sports, music, or acting. A person has social anxiety disorder when it interferes with the person's schooling, work, family functioning, or social life or if it causes significant distress. Some people may be described as very shy while others appear to be comfortable in social situations even though they feel very anxious.

VOLUNTEERS NEEDED: STANFORD BEGINS TRIAL OF MEDICATIONS TO TREAT SOCIAL ANXIETY DISORDER STANFORD – Delivering an important presentation can make most people a bit nervous – even seasoned public speakers. Sweaty palms, heart palpitations or dry mouth are typical symptoms before "The Big Talk." But for a person with social anxiety disorder, the phrase "Never let them see you sweat" becomes painfully amplified: The speaker senses the audience noticing how his hands are trembling furiously and how he’s sweating profusely. With each successive stammer and each fumbled phrase, he’s painfully aware that his expertise is being questioned. Suddenly, his mind goes blank, and he finds himself caught in a roomful of people waiting impatiently for the next word. He begins to feel lightheaded and fears the further embarrassment of passing out before he can come up with another word. "People who suffer from this disorder are extremely afraid of potential scrutiny by others in a social situation; so much so that they either avoid particular social situations or, in extreme cases, all social situations," said Hayward.
