  • Somatization is extremely common in internal medicine practice. In its mildest form it may consist of a single transient symptom in response to stress in an otherwise psychologically healthy person; in its severe form it may be completely disabling, as in a patient with pain syndrome. Classic somatoform disorders are infrequent in primary care but less well differentiated somatization is common.. The Katon article summarizes research that demonstrates the relation of symptom formation to anxiety and depression and to childhood and adult trauma.. Increased symptomatology, not surprisingly, leads to higher health care expenditures. As noted above, somatization symptoms are not under the patient’s conscious control (although a patient may take protective action which seems volitional, such as not allowing an arm raised above his head to drop onto his face) Symptoms which are produced with an intent to deceive are

Background: Somatoform disorders represent a group of disorders characterized by physical symptoms suggesting a medical disorder. However, somatoform disorders represent a psychiatric condition because the physical symptoms present in the disorder cannot be fully explained by a medical disorder, substance use, or another mental disorder. These somatoform disorder physical complaints challenge medical providers who must distinguish between a physical and psychiatric source for the patient’s complaints. Often, the medical symptoms patients experience may be from both medical and a psychiatric illnesses. Anxiety disorders and mood disorders commonly produce physical symptoms. These physical symptoms can dramatically improve with successful treatment of the anxiety or mood disorder. The Diagnostic and Statistical Manual for Mental Disorders, Fourth Edition (DSM-IV)

The Somatoform Disorders take on the form of the soma, or body. All are characterized by somatic complaints without physical basis. In contrast to malingering and other forms of faking, for these persons the illnesses, pains, and physical complaints are believed rather than feigned.
    Somatization Disorder
Somatization Disorder (Briquet's) was originally described by Briquet. Person with this disorder present with multiple complaints of physical ailments over a long period. These begin before age 30, are inadequately explained by independent physical findings and that lead to medical treatment or significant life impairment. The complaints are are typically dramatic and significant; hence, they often lead to major medical intervention, such as surgeries. When performed these interventions tend to lead to a conclusion that no physical basis can be found. When doctors refuse treatment these person tend to "doctor shop" until they find someone new who will listen to their complaints. Diagnosis requires reporting of symptoms in four categories:
  • Pain
  • Gastrointestinal
  • Sexual
  • Pseudoneurological Caution should be used in assuming that no physical basis underlies complaints for these persons. Follow-up studies have shown that for a significant number of Briquet's patients some basis for a portion of their complaints may later be found.

Case Presentation Discussion Somatization disorder is one of the most poorly understood psychiatric disorders, but it is very common in primary care settings. Patients with this disorder are among the most frustrating for providers because they are high utilizers of medical services, diagnostic tests, and procedures, with typically no conclusive diagnosis or identifiable physical cause of their symptoms. Somatization disorder is one of a group of somatoform disorders that also includes hypochondriasis, conversion disorder, and psychogenic pain disorder. Early terms for the condition include hysteria and Briquet’s syndrome, named for the 19th-century French physician who published detailed reports of hundreds of cases, almost exclusively women, who suffered from multiple physical symptoms with no explainable medical diagnosis. In patients with somatization disorder, symptoms often begin in young adulthood, before the age of 30, and persist for decades into late life.

In 1990, I was diagnosed with EBV, or what is now called Chronic Fatigue Syndrome or Myalgic Encephalomyelitis. Three different doctor's went as far as pronouncing that I had AID'S, that's how server my symptoms were. Fortunately after a year of repeated testing and a HIV by PCR was done, all test came out negative, so I didn't have AID'S, but I did have a extremely high EBV titer at the time. (but that's before they found that the EBV titers were unreliable, because it fluctuates from high to low regardless of how you feel.) In 1991 I have seen several many various Good

    Causes and Risks:
In somatization disorders, there tends to be a pattern of multiple and recurrent symptoms and complaints that can involve any body system and often persist for years. Most frequently the complaints involve the gastrointestinal system, the nervous system, the cardiopulmonary (heart and lungs) system, or the reproductive system. The symptoms begin before the age of 30 and occur more often in females. The symptoms are severe enough to lead to visiting health professionals, taking medication, or interfering with the person's life. The cause is not specific but symptoms begin or worsen after losses (for example, job, close relative, or friend). A lifelong history of sickliness is often present, and often people with this disorder relate to others only through their symptoms. A greater intensity of symptoms often occurs with stress
    Prevention:
Counseling or other psychological intervention may help people who are prone to somatization to learn other ways of dealing with stresses. This may help reduce the intensity of symptoms.

Yet, some of them have received only limited empirical investigation. This book continues that rich tradition by offering a broad and scholarly synthesis of the current knowledge-and controversies-about somatoform and factitious disorders. Here you'll find up-to-date, clinically focused overviews of these intriguing and often difficult-to-treat disorders. These people are not "lying" about or making up their illness; they believe that they are truly sick. Causes and Risks There is no specific cause, but stress or conflict may underlie the illness. The pain may have a special significance or may serve as a way to receive attention. People with this disorder may receive attention, sympathy or relief from responsibilities from others, or financial compensation from disability that may sustain the pain and behavior.
Usually there is an anatomical change within the body as a result of disease or injury that is irreversible because multiple interventions to relieve the pain or abnormality have been unsuccessful. Chronic anxiety and depression may develop, with a corresponding decrease in the pain threshold.

SOMATOFORM DISORDER: FROM A LEGAL PERSPECTIVE
Somatoform Disorders are a broad category of psychological disorders which are characterized by diverse physical complaints with no detectable organic causes. Somatization Disorder, a specific type of Somatoform Disorder. Somatization Disorder is a specific psychological disorder that begins before age 30, and is characterized by a combination of pain, gastrointestinal, sexual and pseudoneurological complaints. The diagnostic criteria for Somatization Disorder is quite rigid. There are four requirements that need to be met. To make the diagnosis, the American Psychiatric Association's current manual (DSM - IV) first requirement is the presence of at least four physical symptoms that would normally be suggestive of a general medical condition. The individual must suffer from at least four types of pain (headaches, joint pain, lower back pain), two gastrointestinal symptoms (as nausea, diarrhea or food intolerance), one sexual symptom (sexual indifference), and one pseudoneurological symptom (such as impaired coordination, paralysis or weakness). The second diagnostic requirement is that there must be a history of physical complaints that begin prior to age 30 and last over several years. The individual must have sought medical or health related treatment for these complaints and as a result of their physical conditions there has been an impairment in the individual's social or occupational functioning.
