1. STEVENS JOHNSON SYNDROME SUPPORT NETWORK
A type of hypersensitivity (allergic) reaction that occurs in response to medications, infections, or illness. In stevensjohnson syndrome, the systemic symptoms are severe and the lesions are extensive, involving multiple body areas (especially the mucous membranes). Toxic epidermal necrolysis (TEN syndrome, or Lyell's syndrome) involves multiple large blisters (bullae) that coalesce, followed by sloughing of all or most of the skin and mucous membranes.

Legal Compensation for Arava Damages On March 28th, 2002 Public Citizen, a consumer protection group started by Ralph Nader, urged regulators to immediately ban the rheumatoid arthritis drug Arava, saying it had linked the product to 12 fatal cases of liver damage. According to the petition, there have been at least 130 cases of severe liver toxicity reported to the FDA, including 56 hospitalizations and 22 deaths. Liver toxicity induced by Arava is a suspected cause in 12 of these deaths.

Abstract
Severe skin adverse drug reactions can result in death. Toxic epidermal necrolysis (TEN) has the highest mortality (30-35%); Stevens-Johnson syndrome and transitional forms correspond to the same syndrome, but with less extensive skin detachment and a lower mortality (5-15%). Hypersensitivity syndrome, sometimes called Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS), has a mortality rate evaluated at about 10%.
Drug reactions are self-limited diseases and therefore, generally treatment is symptomatic. Prompt diagnosis, identification of, and early withdrawal of all suspect drugs are the most important preliminaries. The management of the patients must be undertaken in specialized intensive care units, with the same main types of therapy as for burns: warming of the enviromnent, correction of electrolyte disturbances, administration of a high caloric enteral intake, and prevention of sepsis. Efficacy of drugs used in some case reports is difficult to evaluate: intravenous immunoglobulins, cyclosporin, cyclophosphamide, pentoxyfilline, and thalidomide have all been tried. Corticosteroid use is debated and is probably deleterious in late forms of TEN. For DRESS, corticoids are used in cases of life-threatening systemic impairment. Specific nursing care and adequate topical management reduce associated morbidity and allow a more rapid re-epithelialization of skin lesions.

YOU have a sore throat and a few pimples, so you go to a doctor. You are given some common medicines. Suddenly, you get blisters so bad, it's like half your body has been burnt. That is the unusual ordeal that Madam Norhayati Khalid has just been through. She suffered from an allergic reaction to her medication, which left her barely conscious and in severe pain. It could also have killed her. On Dec 14, the 32-year-old security officer was admitted to hospital. Her body was 'burnt' with blisters from the waist up. The medicines had apparently triggered a rare drug allergy called Stevens-Johnson syndrome. (See report below.) She was so badly affected that she had to spend more than two weeks at the burns unit of the Singapore General Hospital (SGH). And, though much better now, she says she still looks like she has been 'stung by a nest of hornets'. Her problems started with a routine visit to the Bukit Merah polyclinic for acne, sore throat and sore eyes, on Dec 10.

Arava has been associated to at least 130 cases of severe liver problems (hepatoxicity) and 22 deaths since its arrival to the market in 1998. The rheumatoid arthritis drug has also been linked to lymphoma, high blood pressure, and Stevens-Johnson syndrome.
From September 1998, when Arava debuted in the United States, to September 2001, the FDA received 130 reports of severe liver reactions in patients taking Arava, Public Citizen said. For 12 cases that were fatal, liver damage from Arava "appears to be the most plausible explanation," the group said.
Public Citizen said it also found reports of many side effects from Arava:
  • Liver damage : The purpose of the liver is to process harmful substances, aid in the digestive process, and produce cholesterol. Liver failure, or hepatoxicity, mostly occurs when a large part of the liver has become damaged beyond repair. Prescription drugs can cause liver failure, and Arava has been associated to this serious condition. Most instances of liver failure require a liver transplant in order to avoid death.

Erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis are mucocutaneous disorders that are thought to be related, constituting a spectrum of reactions. Erythema multiforme (EM) has the most benign presentation, and is followed in severity by Stevens-Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN). Additionally, EM is generally a postinfectious process, while SJS and TEN are drug-induced reactions. ARAVA INJURIES - LEGAL ACTIONS ARAVA BACKGROUND: ARAVA , an arthritis drug has been linked to an alarmingly high number of severe liver problems, liver complications, including deaths, since it was first approved by the FDA for the US market in and later in in Canada. Public Citizen says that Arava should be taken off the market immediately. On March 28, 2002 a petition was filed by Public Citizen to the Food and Drug Administration (FDA). ARAVA, ( lenflunomide ), is an anti-inflammatory drug used for the treatment of rheumatoid arthritis. Patients with rheumatoid arthritis (RA) take the prescription medication to reduce the pain and swelling that occurs in joints, most notably in the hands or feet. Aventis, the maker of the drug, reports that there have been 1.5 million prescriptions written for ARAVA since 1998. FDA first approved Arava on September 10, 1998.

Thu Mar 28, 4:05 PM ET By LAURAN NEERGAARD, AP Medical Writer WASHINGTON - A prescription drug for rheumatoid arthritis has been linked to dozens of serious liver injuries and 12 deaths and should be banned, a consumer advocacy group told the government Thursday. Arava began selling in 1998 as a competitor to the gold-standard treatment for rheumatoid arthritis, called methotrexate. When the Food and Drug Administration (news - web sites) approved Arava, the agency noted Arava worked no better than the older drug, but said patients needed some different options. Since then, the FDA has received at least 130 reports of severe liver toxicity linked to Arava use, including 56 hospitalizations and 12 deaths, said Dr. Sidney Wolfe of the consumer advocacy group Public Citizen. Two of the deaths were people in their 20s. Since Arava and methotrexate work equally well, and methotrexate also bears a warning about possible liver damage, Wolfe compared the two. The FDA has six times more reports of liver damage among Arava users than methotrexate users — even though thousands more people use methotrexate, he said. Citing similar reactions abroad, the European Union (news - web sites) last year warned patients and doctors about Arava's toxicity, Wolfe said. He wants the FDA to go further and ban Arava's sales.
