Diets high in red meat linked to stomach and esophageal cancers
Dietary factors are recognized as contributing to the development of stomach and esophagus cancers. In a study published in the American Journal of Clinical Nutrition, Chen et al. examined overall dietary patterns among patients diagnosed with distal stomach cancer, esophageal cancer, or cancer-free controls. Results suggest that several dietary patterns, particularly those high in red meat, confer a higher risk of stomach or esophageal cancer than other types of diets. All participants in the study were white male or female residents of eastern Nebraska, 124 of whom had been diagnosed with stomach cancer, 124 with esophageal cancer, and 449 healthy controls. Interviewers conducted telephone dietary assessments with the case patients and control subjects or their proxies from 1992 to 1994. Food items on the questionnaire were sorted into 24 different groups that were used to establish a food-intake pattern for each participant. Of all the dietary patterns identified, the “High Meat” diet tended to be associated with a 3.6-fold higher risk of esophageal cancer and 2-fold higher risk of stomach cancer when compared with the “Healthy Diet”. The “High Milk” diet, which also included large amounts of red meat, tended to be associated with 2-fold risk of both types of cancer. These two dietary patterns were more prevalent among the cancer patients, with 33% of stomach cancer patients and 35% of the esophageal cancer patients consuming either the “High Meat” or “High Milk” diet.

What is going on in the body? Stomach cancer usually arises from cells that line the inside of the stomach. A cell will undergo changes that cause it to become cancerous. It will begin to grow and multiply uncontrollably. The cancer cells will form a tumor. The tumor will invade and destroy tissues around it as it grows. Eventually it can penetrate through the wall of the stomach and spread to structures around the stomach. Cancer cells can also break off from the tumor and enter the blood stream. These cells will form new tumors in other parts of the body, a process known as metastasis What are the signs and symptoms of the disease? Because the symptoms are vague and often ignored, stomach cancer can grow in the stomach for a long time before it is detected. The most common symptom is unexplained weight loss . People may complain of an uneasy sense of fullness, stomach heaviness, or distention. A person may also have chronic nausea and pain in the middle of the abdomen. These symptoms indicate advanced disease. Most stomach cancers are found in the advanced stage.

Irvine, Calif., Nov. 1, 2000 — Asian-American stomach cancer patients have tumors with biological traits that differ from tumors in non-Asians, which may explain why they have better survival rates from stomach cancer than non-Asians, according to a UC Irvine College of Medicine study. The findings may be the first to link differences in tumor biology to survival rates among ethnic groups and provide insight into why certain tumors affect some populations more - and more severely - than others. The findings appear in the November issue of Cancer. Hoda Anton-Culver, professor of epidemiology, and Dr. Charles Theuer, assistant professor of surgery, led a team that examined records from 3,770 Asian and non-Asian stomach cancer patients in Southern California. They found that the survival rates of Asians with stomach cancer were far higher than any other ethnic group. They also found that tumors in Asians did not invade normal tissues as aggressively as tumors in non-Asians. "The differences in cancer rates between Asians and non-Asians have been attributed in the past to varying diet, diagnostic methods and more radical surgical techniques by Asian doctors," Anton-Culver said. "But we found that Asian-Americans with the same stage of disease who received the same health care as non-Asians still exhibited superior survival rates. The variation indicates that diagnosis, screening and treatment will have to accommodate biological differences in the way cancer attacks certain ethnic groups."

Stomach Cancer Overview
Stomach cancer has decreased as the most common cause of cancer deaths in males in 1930 to one of the less common causes of cancer death today. This decrease in cancer death is almost entirely accounted for by decrease in the incidence of the disease rather than improved treatment or better detection. Stomach cancer remains common in Asia and Japan and first generation immigrants to the U.S. retain a high risk for the disease. The marked decrease in the risk of stomach cancer is unexplained since the cause of stomach cancer is unknown. Changes in diet, food preservation by refrigeration rather than salting and smoking tobacco rather than chewing it have all been cited as possibly contributing to the declining incidence. The stomach is the dilated upper part of the intestinal tube. That tube has an inner lining, a muscular coat and a thin outer layer. Cancers originate in the inner lining of the stomach and can invade into or through the muscular wall of the stomach and can spread to the lymph nodes in the area. The prognosis or likelihood of cure is related to the degree of invasion of the wall of the stomach and the presence or absence of lymph node metastases. Early detection of stomach cancer is difficult because serious symptoms such as pain and weight loss tend to occur only when the cancer is large and early symptoms such as heartburn are much more commonly associated with benign conditions. The finding of blood in the stool may lead to detection of small early cancers. There are no blood tests for early detection of stomach cancer. Only the stool guaiac test for small amounts of blood in the stool and examination of the stomach with an endoscope for heartburn unrelieved by medication will likely lead to early detection of the stomach cancer.

Stomach cancer, also known as gastric cancer, is cancer that starts in any part of the stomach. The muscles and juices of the stomach help digest food into a liquid from which nutrients can be absorbed lowel in the bowel.
Unfortunately, some of its symptoms can be confused with many common stomach complaints, such as indigestion or heartburn.
However, being alert to changes in stomach and bowel symptoms, and consulting doctors about persistent problems could lead to far earlier detection.
Dr David Cunningham, an expert in stomach cancer from the Royal Marsden Hospital in London, said: "Cancers arising at the stomach are increasing at a fairly rapid rate in most of Western civilisation - we're not entirely sure why this is taking place.
"Anyone experiencing significant indigestion or heartburn for the first time, over the age of 50 should certainly be seen by their GP."
"We now know that treating this type of cancer involves fairly major surgery and other sorts of treatment such as chemotherapy or radiotherapy."

New Study Finds Vitamin C Protective Against Stomach Cancer A new study from the Yale University of Medicine published in the October issue of Cancer Epidemiology, Biomarkers and Prevention, found a correlation between Vitamin C supplementation and a decreased risk of stomach cancer. The researchers interviewed patients throughout Connecticut, New jersey and western Washington. The nutrient intake of 1,095 people with stomach or esophageal cancer was compared to that of 687 healthy people in a control group. The results found that vitamin C supplements provided protection against one class of stomach cancer. People taking vitamin C supplements at least once a week for six months had a 40% reduced risk of cancer in the middle and lower parts of the stomach (noncardia gastric cancer). The study also indicated that in general plant based diets provide protection against these types of cancer. Diets high in animal foods were associated with an increased risk. The dietary nutrients that were found to show benefits were fiber, beta-carotene, folate, vitamin C and vitamin B6.

A researcher at The University of Nottingham has been awarded more than £200,000 to look at how some strains of a common stomach bug may increase the risk of cancer. Professor John Atherton of the Division of Gastroenterology in the School of Medical and Surgical Sciences has been awarded £212,964 from Cancer Research UK, Europe’s leading cancer charity, for the three-year project to look at the bacterium Helicobacter pylori Helicobacter pylori is a very common bacterium that lives in the stomachs of around 40 per cent of the population, usually causing no ill effects.  However, some more ‘nasty’ strains of the bacterium have been shown to increase the risk of stomach cancer. These types of strains have two features in common which researchers believe are responsible for this increased risk — they release cell-damaging toxins and inject proteins into cells, which cause inflammation in the stomach. Professor Atherton will be looking at how these two mechanisms interact to cause the increased risk of disease and why the two mechanisms are almost always found together.

Chronic inflammations caused by infection with viruses, bacteria or parasites increase the risk of certain human cancers; e.g. of the liver (hepatitis B virus), cervix (human papilloma virus), stomach ( Helicobacter pylori ) and bladder (schistosomiasis). Infected tissues can generate endogenous risk factors that may contribute to the development of cancer, e.g. reactive oxygen and nitrogen species that induce oxidative stress, and may injure cells and damage DNA.
Objectives
The Unit studies the expression and regulation of key enzymes involved in oxidative stress, enzymes that generate free radicals, or protect against the tissue damage they induce. It also studies the DNA damage induced by endogenous free radicals.

Stomach cancer (also called gastric cancer) is second only to skin cancer as the most common cancer in the world. In the U.S., stomach cancer is not nearly as common, but is still the sixth leading cause of cancer-related death. Unfortunately, early symptoms are vague and the diagnosis is often made after the cancer has spread. In the early 1900?s, stomach cancer was the most common cause of cancer death in the United States. This is thought to be due to the high incidence of using salting and smoking as methods of food preservation. As refrigeration became widely available in the U.S., stomach cancer rates decreased. Stomach cancer affects men twice as often as women and is uncommon before the age of 40. Nonwhites are affected more often than whites. Unfortunately, early stomach cancer usually causes no symptoms.
