Genes Matter In Response To Strep A November, 20 2002 1:48 Group A streptococci have earned the popular name 'flesh-eating bacteria' for their role in an often fatal infection of soft tissues, necrotizing fasciitis. But the same type of bacteria can also cause only mild illness, such as strep throat. Why do some people walk away unscathed from Strep A infection while others succumb to horrible disease? In the December issue of Nature Medicine , Malak Kotb at the Veterans Affairs Medical Center in Memphis and colleagues begin to answer the question. They had an inkling that a particular set of genes might have something to do with susceptibility to infection. These genes, called HLA-II genes, encode proteins on the cell surface of immune cells. It’s known that these proteins can bind to certain toxins secreted by Strep A. The authors examined the HLA-II genes from 279 individuals afflicted with severe infection – either necrotizing fasciitis or streptococcal toxic shock syndrome – and compared them to the HLA-II genes of 256 healthy individuals. Certain HLA-II gene variants were present more often in individuals with severe infection, while other variants were present more frequently in healthy individuals. The authors also found that immune cells with either predisposing or protective HLA-II gene variants differed in their response to bacterial toxins. There are numerous previous reports of association between HLA variants and diseases such as multiple sclerosis, arthritis, and type 1 diabetes. This study, however, delves into the mechanism, and also presents an opportunity for further study in mice – mice can be engineered to express human HLA genes, and are also susceptible to infection by group A streptococci.

Strep throat is a contagious infection caused by the Group A beta hemolytic streptococcal bacteria. Diagnosis may be made by a throat culture. Strep throat requires a FULL TEN DAYS of antibiotic treatment.
SYMPTOMS
  • Red, sore throat
  • Fever above 100 degrees
  • May have exudate and ulceration on tonsil area
TREATMENT
  • a FULL TEN DAYS of antibiotics, taken as directed
  • Gargles with salt water (1/3 teaspoon in 8 ounces of warm water) every 2-3 hours, to help remove exudate and relieve pain
  • Drink plenty of fluids (eight or more glasses of water, juice, or koolaid daily)
  • Use over-the-counter throat lozenges or spray for temporary relief
  • You may take acetaminophen (Tylenol) or ibuprofin (Motrin, Advil) every 6-8 hours for pain and fever relief.
  • After 5 days of antibiotics, use a new toothbrush and wash any 're-used' cups or glasses that you use.
  • Return to SHS or your health care provider 24 to 48 hours after completion of antibiotics for a reculture of your throat to be sure that all the bacteria are gone.

Dear Alice What is strep throat, and what can I do to get better? Dear Reader, Strep throat is an infection caused by group A Streptococcus bacteria. Symptoms include:
  • severe sore throat, often with white patches on the tonsils fever swollen lymph nodes in the neck nausea, vomiting, and abdominal pain (common in children with the illness)
Someone can become infected with strep throat through direct contact with mucus from the nose or throat of an infected individual. This is a task most easily accomplished by breathing in infected droplets sprayed into the air when a person ill with strep throat coughs or sneezes. It's also possible to become infected by touching these secretions and then touching your mouth or nose, or through direct contact with any wounds or open sores an infected person might have.
For those not already sick with strep throat, the cold, or the flu, it is most important to talk about how to prevent infection. While taking full advantage of all that the winter wonderland has to offer, keep in mind that frequent washing of your hands can stem the spread of unwanted seasonal guests such as the common cold. Scrub up especially after coughing or sneezing and before preparing food and eating.

Last February a happy, confident 8-year-old girl went to bed and woke up the next morning having turned into someone else. She came to her mother with a series of shocking confessions. She said she had licked people's bottoms and drunk her own urine. She listed the people to whom she had shown her private parts. She asked if this made her a "bad person." The mother was horrified and baffled. She calmed the girl down and sent her to school. When her mother picked her up, the girl said she had spread her feces around the school. The mother casually checked with a teacher about the girl's behavior and was told she was fine. This went on day after day. The girl said she had blinded her brother with a fork in his eye. She said she wanted to step in front of a bus. She said she had swear words stuck in her head. At first the mother suspected sexual abuse, but the daughter said no one had touched her, and the parents could find no evidence anyone had. The pediatrician said it sounded like a case of obsessive-compulsive disorder, a condition of unknown origin, and referred the family to a psychiatrist. Surfing the Web, the mother discovered other cases just like her daughter's: normal kids who suddenly become consumed by horrible thoughts or, in some cases, begin twitching uncontrollably. Doctors at the National Institutes of Health had a startling suspect: strep throat, one of the most common illnesses of childhood. Two months before the girl's transformation, she had come down with strep throat four times.

Diagnosing Strep Throat If you have a sore, scratchy throat, you don't want to wait days to find out the cause. For patients who may have strep throat, one of the most annoying things is waiting for the results of the throat culture test for strep. With growing concerns about antibiotic resistant bacteria, doctors are more reluctant to prescribe antibiotics. If it does turn out to be strep throat, they prefer a solid diagnosis based on test results, even if that can take a couple of days. Now researchers in California say that a quicker test called the rapid antigen test can be very accurate in patients who have strep throat symptoms. These tests had been considered fairly inaccurate, missing 10 to 20 percent of the patients who had strep throat. The recent study showed, however, that the test is much more accurate in patients who have specific symptoms. There are four main symptoms of strep throat: sore throat, fever, swollen lymph nodes and no cough. In patients who had all four symptoms, the rapid antigen test was about 94 percent accurate. The researchers say this means that patients who visit the doctor with all the symptoms of strep throat may not have to wait as long to have the diagnosis confirmed so they can start treatment.
