Question Posted By: prough on Wednesday, October 09, 2002
My 13 year old daughter has been diagnosed with Syringomyelia- her MRI's show no Chiari malformation, she has had no spinal injury and no evidence of any tumor. She is having tremendous back pain on the left side only. Other symptons are stiffness in left arm/left leg. She also suffers with bad headaches. We have pulled her from soccer and other sports.
Her sryinx is from T6-T10. Other than Chiari or injury, what could be the cause?
Our appointment with the neurosurgeon is Nov 11th. Should she be seen sooner? Are all neurosurgeons familiar with this disorder? Do we continue to look for the cause or do we focus on treating the sryinx? Should we be limiting her activities? What should we do to releive the pain? I am just looking for some guidance and appreciate any suggestions.

Syringomyelia Fluid filled cavity within the substance of the spinal cord. Enlargement of the central canal of the spinal cord (hydromyelia or syringomyelia) can be due to: trauma, tumor, a developmental abnormality of the base of the brain called the Chiari 1 malformation , or may be idiopathic (cause unknown). The symptoms of hydromyelia/syringomyelia can include numbness and weakness. They can progress (worsen). If the enlargement is large the patient may benefit from having a shunt placed to drain it. compare hydromyelia Spine surgery Specialty Center figure: syringomyelia Return to uscneurosurgery.com Homepage

Description
Although recognized as a spinal cord disorder as early as the 1500s, the precise diagnosis of, and optimum treatment of Syringomyelia and the Chiari malformation remain elusive. During the past decade, MRI technology, intra-operative ultrasonography, clinical series and case reports, and experimental investigations have made diagnosis easier, enabling those afflicted to receive treatment during earlier stages of the disorder. Syringomyelia and the Chiari Malformation provides readers with the most recent information on the classifications, pathology and development of Syringomyelia and Chiari malformations. The editors also provide a history of these conditions. Specific surgical and non-surgical management strategies and special considerations are also covered in detail. Topics covered in Syringomyelia and the Chiari Malformation: -Classification of the Chiari malformations and Syringomyelia -Pathogenesis and development theories -The relationship between Syringomyelia and Chiari malformations -Post-traumatic Syringomyelia -Management schemes for Syringomyelia: surgical indications and nonsurgical management -Shunting procedures for Syringomyelia in Chiari malformation

Syringomyelia
Definition: Syringomyelia is damage to the spinal cord, caused by a formation of a fluid-filled cavity within the cord.
Alternative Names: Syrinx
Causes, incidence, and risk factors: Syringomyelia is caused by a formation of a fluid-filled cavity within the spinal cord. This fluid build-up may be a result of spinal cord trauma , tumors of the spinal cord, or congenital defects. The cavity most often begins in the neck area. It expands slowly, causing progressive damage to the spinal cord due to the pressure exerted by the fluid. Symptoms result from the spinal cord damage.

  • Syringomyelia is a disorder in which a cyst forms.
  • This cyst, called a syrinx, expands and elongates over time, destroying the centre of the spinal cord.
  • If not treated surgically, Syringomyelia often leads to progressive weakness in the limbs, loss of hand sensation and chronic, severe pain.
  • Each patient experiences a different combination of symptoms.

Establishing the Physiology of Syringomyelia
Number:
92-N-0226
Summary:
The brain and spinal cord are surrounded by fluid called cerebrospinal fluid (CSF). The CSF flows through channels in the brain and around the spinal cord. Occasionally, people are born with malformations of these channels. Syringomyelia is a pocket within the CSF channels that results from abnormal CSF flow. Syringomyelia is associated with problems in the nervous system. Patients with syringomyelia may be unable to detect sensations of pain and heat. If the condition is not treated it can worsen. Treatment of this condition is surgical. It requires that the flow of CSF is returns to normal. There are many different treatment options, but no one procedure has been shown to be significantly better than any other. In this study, researchers would like to learn more about how the CSF pressure and flow contribute to the progression of syringomyelia. Ultrasounds and magnetic resonance imaging (MRI) will be used to evaluate the anatomy of the brain. Researchers hope that information gathered about anatomy and measures of CSF pressure and flow can be used later to develop an optimal surgical treatment for syringomyelia.

Syringomyelia
Dr Streebig,
My name is Billie F, I am 54 years old. In 1986 I began dragging my left leg and after a period of time I went to a Neurologist, who ordered an MRI. The finding was I have Syringomyelia. Because this is very rare not much research has been done and the only thing that had been tried would probably leave me paralyzed 90 % chance and that was not a consideration. I saw numerous specialists, and they all said same thing; I would be in a wheelchair in 2 years. Usually Syringomyelia affects the arms and hands, but mine is my left leg.
In 1987, I started going to Dr. Ha an acupuncturist who told me he could not cure me but would keep me walking as long as he could. For 12 years I have gone faithfully twice a week, once a week, and then once every other week, and had to pay for it out of my pocket because he is not an M. D. As far as I am concerned he knows more than most doctors and specialists insurance companies pay for without a second thought.
Through the years I have seen many specialists, because the insurance company says I had to see a specialist to confirm my needs. To me this has been money wasted. When I needed a wheelchair I had to have a specialist say it was a medical necessity. When I needed a leg brace because the new doctor said it would help me walk, I had to see a specialist. This doctor recommended I have physical therapy so I went and the first treatment (which was my last ) the therapist hurt my knee and it still hurts.
