PATIENTS AND METHODS: The clinical records of 50 patients (21 males and 29 females), who had been diagnosed as having scoliosis associated with syringomyelia and had been operated on for syringomyelia, scoliosis or both, were investigated. We divided these patients into three groups according to the surgical procedures. Group 1 included 14 patients who were subjected to corrective surgery for scoliosis after surgery for syringomyelia such as foramen magnum decompression or shunt procedure. Group 2 included 21 patients subjected to corrective surgery for scoliosis alone. Group 3 included 15 patients subjected to surgery for syringomyelia alone. In each group, we mainly reviewed the postoperative changes of neurologic symptoms. RESULTS: CONCLUSIONS: This study reported a high frequency (14.0%) of postoperative neurologic complications. It increased to 33.3% in the patients who had not been treated for syringomvelia. Dealing with syringomyelia previous to spinal corrective surgery is necessary to avoid not only early neurologic complications but also delayed complications, especially if the syrinx is large or tonsillar herniation is severe.

Syringomyelia, though an uncommon complication of it, is usually of late onset. Methods: We report two patients with tuberculosis meningitis who developed syringomyelia acutely. The diagnosis was supported by neuroimaging and findings at laminectomy. Results: The two patients developed syringomyelia between 11 days and 6 weeks of the onset of tuberculous meningitis. They both had cord swelling and softening. Conclusions: Acute-onset syringomyelia should be suspected in any patient being treated for tuberculosis meningitis who subsequently develops limb weakness and/or sphincteric dysfunction. Inflammatory edema and cord ischemia appeared to be the underlying mechanisms in these early onset cases rather than arachnoiditis which is important in late-onset cases. Can. J. Neurol. I can find no reports that damage from a decompression accident has been related as a cause. This condition is not mentioned in Edmonds' or Bove's texts - so one will have to use associative reasoning - as we often do about situations such as this. As the body moves in it's day to day activities the syrinx or cyst expands and elongates over time, destroying the center of the cord, and can mimic the effects of a spinal decompression accident. This damage may result in pain, weakness, and stiffness in the back, shoulders, arms or legs. Other symptoms may include headaches and loss of the ability to feel extremes of hot or cold, especially in the hands and disruption in body temperature,sweating, sexual function and bladder and bowel control. Because of the difficulties in differentiating between the residual effects of a diver's decompression accident and the effects of the syrinx - it is felt that diving would be unwise unless carefully documented and evaluated on an individual basis.

Syringomyelia An excerpt from The Spinal Cord Injury Newsletter , Spring 1996 SYRINGOMYELIA AFTER SCI There are several types of SM, including SM that occurs after spinal cord injury. Research studies have examined the location, size, shape and type of cavity in many different cases. These studies have found that a syrinx that forms after a spinal cord injury almost always appears at the site of the original injury. The syrinx may also extend several spinal segments above or below the level of injury. The shape of cavities, as shown by Magnetic Resonance Imaging (MRI), is also variable. There may be one oblong cavity or a large cavity may have several oblong compartments that are connected. Or there may be several separate cavities that have nerve tissue between them. The ends of the cavities may have a round, bulb-like shape or may stretch down to a thin strand. Currently, SM is diagnosed following physical examination, history and progression of symptoms, and MRI of the spinal cord. The fluid-filled cavity usually shows up clearly on the MRI, revealing the size and location of the syrinx. It was generally believed that SM symptoms began long after the initial SCI, but recent studies suggest that syrinx-related symptoms may begin as early as two months after injury. TREATMENT The question of when and how to treat SM is not easy to answer, and different physicians take different approaches. Research studies do agree that it is important for the person with SM or suspected SM to find a physician who is knowledgeable about SM, including a neurosurgeon who has treated multiple cases of SM. A university hospital or rehabilitation clinic with an SCI unit is a reasonable place to start a physician search.

Since the spinal cord connects the brain to nerves in the extremities, this damage results in pain, weakness, and stiffness in the back, shoulders, arms, or legs. Other symptoms may include headaches and a loss of the ability to feel extremes of hot or cold, especially in the hands. Each patient experiences a different combination of symptoms. Other, more common disorders share the early symptoms of syringomyelia. In the past, this has made diagnosis difficult. The advent of one outpatient test, however, called magnetic resonance imaging (MRI), has significantly increased the number of syringomyelia cases diagnosed in the beginning stages of the disorder. About 21,000 American men and women have syringomyelia, with symptoms usually beginning in young adulthood. Signs of the disorder tend to develop slowly, although sudden onset may occur with coughing or straining. If not treated surgically, syringomyelia often leads to progressive weakness in the arms and legs, loss of hand sensation, and chronic, severe pain.
