In our hectic, stressful everyday lifestyles we all use the term “I was literally pulling my hair out!” but this is living reality for people with the impulse control disorder called trichotillomania.
It can be a very upsetting ordeal, having to deal with issues such as hair loss and bullying as a result. They may feel helpless and loose all self-confidence. People with trichotillomania can pull from anywhere such as the scalp or eyelashes, to leg and arm hairs.
It is a frustrating experience for sufferers as loved ones try to help but simply do not understand, they may see it as a phase in which they should get over in time, but it can get worse instead of better. The nagging feeling in the stomach will just get worse and worse until you fell the relief and gratification of pulling the hair.
I am 15, and have been pulling for 6 years now. Starting with my eyelashes it then lead onto my scalp hair. The funny stares on the bus and around town and schools are enough to drive anyone mad! I know people have it worse than me though but then I know that a lot of people haven’t been able to stop even with professional help. It scares me really that I may have it for the rest of my life but I know I will try my hardest.

How and When Does it Start?
People often start compulsive hair-pulling around the ages of 12-13; although it is not uncommon for it to start at a much younger or older age. Hair pulling works to reduce stress because the behavior and sensations of pulling hair causes a short-term distraction from other emotional distress. The concept of negative reinforcement is commonly misunderstood and confused with idea of punishment. Let me discuss the concept of negative reinforcement. Simply, is is the pleasure/reinforcement we get when "it is so nice when it stops." An example might be laying in the sun on the beach. After while you become uncomfortably hot. To cool off you get up and run into the water. Running into the water behavior is reinforced by the cool water's termination of your uncomfortable "too hot" feeling. It is reinforcing to stop the too hot feeling. TTM works the same way. Patients may be uncomfortable from anxiety, worry, depression or the like. Pulling on their hair distracts their attention from the experience of this distress. Sure, this is only a short-term reprieve from the emotional pain, but that is really what counts in operant conditioning. Our nervous systems are wired so that short-term rewards are much more powerful than long-term ones.

COGNITIVE-BEHAVIORAL TREATMENT OF TRICHOTILLOMANIA By Frederick Penzel, Ph.D.
Of all the disorders belonging to the OCD family, trichotillomania is probably one of the least understood by both sufferers and practitioners alike. It has also been, for many years, among the most frustrating and difficult to treat for all parties concerned.
The name trichotillomania (TTM) coined in 1889 by Francois Henri Hallopeau, is in itself an unfortunate and misleading one. It seems to pigeonhole the disorder with other unrelated problems such as kleptomania, pyromania, etc. In addition, it seems to imply an enjoyment and attraction on the part of sufferers to pursue their hair pulling activities. Those who have the disorder understand that this obviously is not true. The term 'mania' itself seems to suggest a serious mental illness.
The gap between what the drugs can and cannot do can be bridged by behavioral therapy. Over the years, many assorted techniques for treating TTM have been tried, with little or no success. These would include psychoanalysis, biofeedback, relaxation training, aversion therapy, etc. I have heard a few isolated reports of the successful use of hypnosis, but only a few small studies are available on its effectiveness. Although more formal scientific studies of the use of behavioral therapy for TTM remain to be done, it appears that it is the most effective treatment that we possess.
TTM is an extremely complex disorder with many potential inputs. It is my own hypothesis, and that of others, that TTM is the result of a dysfunction of a mechanism that regulates levels of stimulation within the central nervous system. People seem to pull when they are either overstimulated (stressed, anxious, or excited) or when understimulated (bored or inactive). Obviously, there are many things that can influence a person's stimulation levels. This is why there does not appear to be one single technique, approach, or device that can relieve the problem. TTM appears to respond best to an approach made of a number of different components.

The word trichotillomania is derived from the Greek thrix, hair, tillein, to pull, and mania, madness or frenzy. This name is somewhat of a misnomer in that people with trichotillomania are not "mad", "psychotic" or "crazy" as the name suggests. In psychiatry, trichotillomania, is classified as an Impulse Control Disorder as are conditions such as compulsive gambling, kleptomania (compulsive stealing) and pyromania (compulsive fire setting). Impulse control disorders are characterized by the inability to control or resist the temptation (or impulse) to do something harmful to oneself or someone else. A sufferer sometimes experiences a sense of increasing tension before performing the behavior and can feel a sense of relief or release of tension afterwards. Sometimes people even express a degree of pleasure after having performed the act. Features of trichotillomania that fit the description of an impulse control disorder include the inability to resist urges to pull out one's hair, mounting tension before pulling and feeling of relief afterward.

What is Trichotillomania?
The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association, Washington, DC, 1994) defines Trichotillomania as:
Recurrent pulling out of one's hair resulting in noticeable hair loss.
Increasing sense of tension immediately before pulling out the hair.
Pleasure, gratification, or relief when pulling out the hair.
Not better accounted for by another mental disorder and not due to a general medical condition (e.g., a dermatological disorder).
Results in substantial distress and/or interferes with functioning.
Not all patients who pull out their hair meet these strict criteria. In fact, many do not experience a growing sense of tension before pulling. Nonetheless, they frequently have a distressing problem that might very well benefit from treatment.
