Hello.
I am a 27 year old female who has suffered severe Trichotillomania and OCD for over 10 years. All medications and forms of therapy have been exhausted. I am now working to get placed on a list to have a Cingulotomy... My question is How successful is the surgery in treating OCD? and more importantly, what can a person expect immediately after the operation.... (w/ gamma knives)... My psychiatrist seems to think it is a REAL option at this time... I just don't know anyone else who has had one... and don't know what recovery time is....
Why isn't more written about this procedure?

TRICHOTILLOMANIA The defining characteristic of Trichotillomania is the recurrent, compulsive pulling out of one's own hair, resulting in observable hair loss. Usually, but not always, the scalp and/or face are the primary locations for hair pulling. However Trichotillomania may involve any part of the body with hair. The most common hair pulling sites are the scalp, eyebrows, and eyelashes. Less common locations for hair pulling include the pubic area, perirectal region, or any other body region. An individual with Trichotillomania may use his or her fingernails, as well as tweezers, pins or other mechanical devices. This hair pulling can result in permanent skin damage. Usually, but not always, hair pulling is preceded by a high level of tension and a strong "urge". Likewise, hair pulling is usually, but not always, followed by a sensation of relief or pleasure. Hair pulling is usually done alone, often while watching TV, reading, talking on the phone, driving or while grooming in the bathroom. An episode may be triggered by a negative mood state or occur in response to stress, but may also occur while an individual is calm and relaxed. Sometimes hair pulling is done as a conscious behavior, but it is frequently done as an unconscious habit. Individuals with Trichotillomania often attempt to camouflage the hair loss that accompanies the disorder. Common camouflaging techniques include the use of hats, scarves, and false eyelashes. Some may even resort to having false eyebrows permanently tattooed. In extreme cases, individuals with Trichotillomania may avoid social situations in an effort to prevent others from seeing the hair loss that results from hair pulling.

We thought you were a complete fool to take her on, and just look at her! She looks like my pretty angel again!" Simone sat cross legged on the floor holding a steaming cup of tea in both hands. She smiled at her father shyly, and she was indeed pretty. Pale, yes, trembling, yes, but a thin, short halo of fine baby hair clung about her head, and her eyes looked almost natural with their new, soft lashes and eyebrows. "It's very difficult, Daddy," she said, "you know me, you know how hard it is for me not to turn it back on myself, but Evan helps me." "Well yes, clearly! And you've done a wonderful job, Evan, but how?"

I attended and was one of the organizers of this years annual Trichotillomania Retreat membership conference in Massachusetts. Each year adults and children gather in a camp setting to attend educational and experiential workshops on TTM. We laugh, cry, experience, swim, share and heal. It is the most important healing experience in the Trich. community. During breakfast Sunday morning a lady came up and asked me to talk to a participant who had been crying since she registered on Saturday. This person was scared the depressed woman was going to harm herself. I introduced myself and asked if I could attend a workshop with her. We went to one workshop and she continued to cry from the moment I met her through the workshop. I asked if I could see her privately to talk with her. We went back to her cabin and talked. She told me no therapy or medications had helped. She picked her face for hours a day and was up till 3 am most nights stuck picking and staring in the mirror. Her arms were weak. She had been picking most of Saturday night since arriving at the retreat. I asked if I could try my energy balancing work. I corrected a massive psychological reversal and gave her her correction for reversal to stop picking "Even if I never stop picking for the rest of my life and am scared forever", I then helped her elicit her picking urge by staring in her pocket mirror. She got it up to a 5 and we used the CB, UE, CB, UA, CB, 9 gamut and repeat with one mini reversal. She could not elicit the urge after two treatments. She reported her depression gone. She was in shock.

Researchers estimate that 1-2% of the U.S. population has trichotillomania - a compulsion to repetitively pull or pluck one’s hair, resulting in noticeable hair loss. Many people with trichotillomania feel ashamed and embarrassed by their hair pulling, attempt to hide it from friends, co-workers and family members, and do not seek help. Many who consult their personal physician or a dermatologist because of hair loss never reveal the true cause and doctors often fail to consider this diagnosis. Although trichotillomania can begin in very young children or middle-aged adults, the most common age of onset is during early adolescence. Women seem to be affected more than men with some estimates suggesting a ratio of 3 women to every man.
Trichotillomania is currently categorized as an impulse control disorder in which the urge to pull hair is associated with an increasing sense of tension. The act of pulling itself is presumed to relieve that tension. Trichotillomania has been considered a habit, like nail biting, that can have both a soothing function and potential consequences.
While the actual cause of trichotillomania is not known for certain, several factors appear to play contributing roles:

There are many causes of patchy hair loss (alopecia). Trichotillomania is the name used when a child twists, pulls or plucks out their hair, eyebrows or eyelashes. Trichotillomania does not cause permanent hair loss. No harm is done to the hair follicles. It does not cause brain damage. When the pulling, twisting or plucking stops your child's hair will grow back normally. Trichtillomania is quite common in children. The behaviour responds well to treatment.
Babies: one month to two years
Hair pulling and thumb sucking usually go together. The baby reclines with thumb or fingers in the mouth and twists their own or their mother's hair with the other hand. It is a comforting habit. Children usually do it before falling asleep or when they are distressed.
Toddlers: two to five years
If your child has been pulling at their hair since they were a baby, they will be in the habit of doing it without thinking. At around the age of three, children can tell if you are reacting to what they do. If you get worried or upset when your child pulls at their hair, they soon learn how to get your attention with this behaviour. Hair pulling is used by your child the same as a kicking and screaming tantrum. Younger children usually twist hair and the older ones pluck at their hair.
School age: five to 12 years
Trichotillomania can be a simple habit or an angry protest. If your child has only started pulling their hair out at school age, there may be other causes for the problem. It may be school-related stress or it can be a signal or warning that something is worrying your child.
