An ulcer is a raw inflamed area in the lining of your stomach (gastric ulcer) or the duodenum - an area where the upper intestine joins the stomach, (duodenal ulcer).
How does it develop?
The digestive juices formed in your stomach contain acid and enzymes, which break down food. The stomach and the duodenum have a protective lining but if your stomach produces too much of these acidic digestive juices, the stomach lining can become broken. Why does it happen?
There are some known contributing factors to the development of duodenal ulcers.
Your stomach may be infected by a bacterium called Helicobacter pylori . This infection can significantly contribute to the development of peptic ulcers.
Men are more at risk of ulcers than women.
Aspirin and some anti-inflammatory drugs for pain or arthritis are commonly associated with ulcers. Elderly people and patients with a family history of stomach ulceration are more likely to develop ulcers.

What is a stomach or duodenal ulcer?
An ulcer is an open sore, or lesion, usually found on the skin or mucous membrane areas of the body. An ulcer in the lining of the stomach or duodenum, where hydrochloric acid and pepsin are present, is referred to as a peptic ulcer. When the peptic ulcer is located in the stomach, it is called a gastric ulcer . When the peptic ulcer is located in the duodenum, it is called a duodenal ulcer What causes gastric and duodenal ulcers? In the past, it was believed lifestyle factors, such as stress and diet caused ulcers. Later, researchers determined that stomach acids - hydrochloric acid and pepsin - contributed to ulcer formation. Today, research shows that most ulcers (80 percent of gastric ulcers and 90 percent of duodenal ulcers) develop as a result of infection with a bacterium called Helicobacter pylori (H. pylori). It is believed that, although all three of these factors - lifestyle, acid and pepsin, and H. pylori - play a role in ulcer development, H. pylori is considered to be the primary cause in most cases.

The inflammation may be precipitated by aspirin and nonsteroidal anti-inflammatory medications (NSAIDs).
Duodenal ulcers are commonly associated with the presence of the bacteria Helicobacter pylori in the stomach. Risk factors are aspirin and NSAID use, cigarette smoking , and older age. Duodenal ulcer has historically occurred more frequently in men, but more recent data suggest similar rates in both men and women. The lifetime prevalence of a peptic ulcer 5 to 10% and approaches 10 to 20% in patients who are Helicobacter pylori positive.

Ulcers i n the stomach or duodenum were attributed for years to excess stomach acid and treated with antihistamines which reduce acid secretion and antacids. In the past, milk was recommended, but dairy products can be the cause of the problem and are often contra-indicated; milk allergy may be an original cause of gastritis which leads to ulceration. A common cause of stomach ulcers is the regular use of ASA or related anti-inflammatory drugs - NSAIDs. If ulcer symptoms occur while taking these drugs, their use should be discontinued. An acute duodenal ulcer will present with pain high and central in the abdomen; the pain comes on as the stomach empties 3 or 4 hours after eating and during the night. Eating food, especially bland foods, milk or antacid tends to relieve the pain. Antihistamines ( H2 blockers - examples are tagamet, pepcid and zantac) have been prescribed for years to treat ulcers and acid reflux into the esophagus and are now available as over-the counter medications.
