ABSTRACT There is considerable evidence indicating that, as in Alzheimer's disease, the central cholinergic system is impaired in vascular dementia (VaD). Using lessons learned from Alzheimer's disease research, it has been proposed that enhancement of the cholinergic system is a rational approach to treating the symptoms of VaD. Galantamine's dual mode of action may provide a greater chance of success in treating patients with Alzheimer's disease through enhanced efficacy on the cognitive, functional and behavioural aspects of dementia. Trials are currently underway to see if this broad spectrum of efficacy extends to patients with Alzheimer's disease with cerebrovascular disease ('mixed' dementia) or VaD, as well as other conditions, and the results are eagerly awaited. Treating Vascular Dementia With Ginkgo Biloba* What is Vascular Dementia?
Dementia is loss of cognitive and emotional abilities severe enough to interfere with daily functioning and the quality of life. Vascular dementia is dementia that results from insufficient blood flow to the brain, called cerebral insufficiency. Vascular dementia is the second leading cause of dementia, just below Alzheimer's disease. There is a strong body of evidence that Ginkgo Biloba helps alleviate cerebral insufficiency by increasing the blood flow to the brain. This would tend to have a positive influence on vascular dementia. Ginkgo Biloba Clinical Proof
A massive body of scientific evidence supports a variety of medicinal uses of Ginkgo Biloba. In fact, the European medical community has recognized Ginkgo Biloba as an effective compound in the treatment of cerebral insufficiency (poor blood flow to the brain) resulting in vascular dementia. Recent studies in which Ginkgo Biloba demonstrated therapeutic effects in the treatment of dementia have even earned Ginkgo Biloba the approval of the German BGA for use in the treatment of dementia, as well as memory deficits, impaired concentration, depression, dizziness, tinnitus (ringing in the ears) and headache; particularly when these are due to cerebral insufficiency. A study compiling the results of using Gingko Biloba to treat 202 patient's diagnosed with Alzheimer's disease or dementia came to the conclusion that Gingko Biloba was "safe and appears capable of stabilizing and, in a substantial number of cases, improving the cognitive performance and the social functioning of demented patients for 6 months to 1 year."

Aricept Efficacious for Vascular Dementia

Data presented Wednesday at the American Academy of Neurology (AAN) Annual Meeting showed that Pfizer (NYSE: PFE) and Eisai's Aricept (donepezil hydrochloride tablets) was both efficacious and well tolerated in patients with vascular dementia. The results are a significant step towards increased use and label expansion. The study reproduced observations from earlier investigation, solidifying their resultant conclusions. The cognitive decline in vascular dementia, characterized by confusion, memory loss, impaired orientation, and impaired judgment, is initiated by a stroke or series of strokes, and is second only to Alzheimer disease (AD) as a cause of dementia.
This confirmatory randomized, double-blind, placebo-controlled, 24-week trial evaluated 603 patients with vascular dementia without a prior AD diagnosis. Study subjects received high or low dose Aricept or placebo daily, and were assessed with various well-verified tools used in the clinical evaluation of cognitive decline.
High and low dose Aricept recipients achieved statistically significant advantages in terms of cognitive function as measured by the Alzheimer's Disease Assessment Scale (ADAS-cog), cognitive function as measured by the Mini-Mental State Examination (MMSE), and slowing of functional deterioration as measured by the Alzheimer's Disease Functional Assessment and Change Scale (ADFACS).

Besides general history clinical diagnosis of dementia is based on a cognitive evaluation, a behavioural assessment and a neurological examination: 1. In all patients, a short mental status test should be part of the clinical examination. The level of intellectual performance such as memory, language, praxis, visuospacial function, temporal orientation, executive functions can be measured by several tools, Mini-Metal Scale Examination (MMSE) of which is most widely used. This test is a simple screening and easy to administer in ten minutes. A cognitive decline can be roughly quantified in the range from to 30 points. It is relatively insensitive to mild deficits and is influenced by education and age. A variety of vascular pathologies are related to VD syndrome. Large-vessel arteriopathies can lead to single or multiple infarcts, whereas small-vessel arteriopathies are the cause of subcortical infarcts. Other factors associated with VD are diffuse white matter lesions, cardiac embolic strokes, hemodynamic changes, hemorrhages and hematology abnormalities. The association between the type, extent, side, site, and tempo of vascular lesions in the brain and different types of vascular cognitive impairment has not been clearly established. Studies on VaD, which have been based on the multiinfarct concept, indicate that bilateral ishemic lesions have a significant role. The locations that are most important are not known precisly.
