NEUROLOGY CARLOS EDUARDO REIS Medstudents' Homepage Neurofibromatosis Neurofibromatosis(NF) is an autosomal dominant disease characterized by disordered growth of ectodermal tissues, and is part of a group of disorders called Phakomatoses (neurocutaneous syndrome). NF is divided in two Types: Neurofibromatosis 1(Von Recklinghausen's disease) Neurofibromatosis 1(NF1) is characterized by spots of increased skin pigmentation(café au lait spots), combined with peripheral nerve tumors and a variety of others dysplastic abnormalities of the skin, nervous system, bones, endocrine organs and blood vessels. Etiology The reponsible gene is located on the long arm of chromosome 17 Epidemiology Its incidence is 1 per 3.000 births and present in about 30 persons per 10.000 population. It is inherited as an autosomal dominant trait, but about 50 percent of cases arise as mutations. Pathology The peripheral nerve tumors of two types, schwannomas and neurofibromas. Both types of tumor occasionally become malignant. Clinical Manifestations The pigmented spots are irregular in shape with relatively even borders, vary in size, and are of brownish coffe color(café au lait). They are most proeminent over the trunk, in the axilla(axillary freckles), and about the pelve.

VRN, which is characterized by a variety of clinical features, is the most commonly encountered neurofibromatosis, affecting one in 3000 persons. To increase awareness of clinicians about the various pathology that can be associated with neurofibromatosis, the authors report a unique case of a VRN patient who had cranio-orbital anomalies which simulated a tumor. Case Report A 49-year-old Sri Lankan male, a known case of VRN, presented with a five-month history of diplopia and recurrent episodes of occipital headache associated with blurring of vision. He was investigated elsewhere and referred as a case of an intraorbital tumor. On examination, the patient had multiple cafe-au-lait spots all over the trunk and multiple skin nodules. He had a normal visual acuity bilaterally; however, there was limitation of lateral movement in the right eye. Fundoscopy was normal and there were no Lisch nodules detected in the iris. The visual evoked response showed a P100 latency of 105 mSec in the right eye and 96 mSec in the left eye. Computed tomography (CT) scan showed an enlarged empty sella (Figure 1). The right sphenoid wing was absent (Figure 2). There was also evidence of brain tissue occupying the posterior aspects of the right orbit and the intracavernous portion of the right carotid artery appeared ectatic (Figure 3). Carotid angiography demonstrated the tortuous and ectatic right internal carotid artery (Figure 4). Full endocrinological assessment was normal. The patient is being followed up.

Case Studies Post-Laminectomy Cervical-Thoracic Kyphosis in a a Patient with Von Recklinghausen's Disease J.F. Mallet, MD; C. Bronfen, MD Case History The patient is a female with a long history of von Recklinghausen's disease. She was initially evaluated by us at age 13 with severe neck pain centered at the cervical thoracic junction. Radiologic evaluation at that time revealed a large extramedullary tumor consistent with a neurofibroma in the low cervical area (Figs 1a-b). The initial surgical management approach was a CS-T1 laminectomy with partial debulking of the rumor. Complete removal was abandoned because of severe bleeding. A second operative procedure was performed 15 days later to remove the lateral portion of the tumor by a right oblique cervicotomy. Pathologic diagnosis of the specimen was hamartoma. Following the initial two-stage surgery, the patient developed a stiff cervicalthoracic kyphosis and had extreme pain with minimal movement. Physical examination revealed numerous skin lesions consistent with the stigmata of von Recklinghausen's disease. She had diminished right upper extremity strength related to her previous surgery. However, there was no evidence of abnormalities in the legs suggestive of spinal cord compression. Fig. 1a

Les anomalies squelettiques survenant lors de la maladie de von Recklinghausen sont présentes dans 1/3 des observations environ. Elles révèlent la maladie ou sont présentes au départ. Ce sont essentiellement les déviations de la colonne vertébrale (cyphose, scoliose) ou les inégalités de longueur des membres. 1) Les anomalies touchant la colonne vertébrale sont les plus importantes. Elles se traduisent par une scoliose évolutive : déformation latérale en forme de courbe de la colonne vertébrale, qui touche dans la moitié des cas les vertèbres dorsales situées avant le rachis lombaire (9ème, 10ème, 11ème et 12ème vertèbres dorsales). Généralement il existe une cyphose associée (déviation de la colonne vertébrale dont la convexité est postérieure). Les corps des vertèbres ont tendance à se déformer précocement et rapidement en prenant une forme de diabolo. Il peut s'associer d'autres troubles touchant d'autres parties de la vertèbre (canal rachidien, trou de conjugaison, pédicule). Il faut savoir penser à une tumeur la moelle épinière ou à la présence de neurofibromes. Une cyphose du rachis cervical peut également être observée de façon isolée. D'apparition précoce, elle est également très évolutive et pose les mêmes problèmes de complications que les cyphoses et les scolioses touchant le rachis dorsal.

Sie hat derzeit etwa 1.300 Mitglieder, die in über zwanzig regionalen Selbsthilfegruppen organisiert sind. Die Neurofibromatose ist eine der am häufigsten auftretenden Erbkrankheiten, von denen in Deutschland ca. 35.000 bis 40.000 Menschen betroffen sind. Hauptmerkmale dieser Krankheit sind hellbraune Hautflecken-sogenannte “Cafe-au-lait”-Flecken und Neurofibrome. Bei Neurofibromatose handelt es sich um gutartige Geschwülste bestimmter Nerven- und Bindegewebszellen. Sie können auf der Haut auftreten und zu schweren Entstellungen im Gesicht und am ganzen Körper führen. Andere Krankheitsformen bestehen in wiederkehrenden Tumoren im Gehirn, Rückenmark sowie an den Nerven von Augen und Ohren mit den entsprechenden gravierenden Folgen wie z. B. Querschnittlähmungen, Erblindung oder Ertaubung. Bei Kindern treten psychomotorische Entwicklungsverzögerungen sowie Lern-, Leistungs- und Verhaltensstörungen bei etwa 40% bis 50% auf.
