In de plekken vallen (vrijwel) alle haren uit, terwijl de huid eromheen een normale haargroei toont. Het gevolg is dat er één of meer kale gebieden ontstaan. De precieze oorzaak van alopecia areata is onbekend. Als oorzaak van alopecia areata wordt gedacht aan een stoornis in het afweermechanisme van het lichaam, de immuniteit. De afweer is als het ware ontspoord en richt zich niet tegen infecties van buitenaf maar tegen het eigen lichaam, in dit geval tegen de haarwortels. Men denkt dat alopecia areata van immunologische oorsprong is. Dit wordt versterkt doordat gebleken is, dat patiënten met deze ziekte verhoudingsgewijs iets vaker lijden aan andere ziekten die gepaard gaan met stoornissen in het afweersysteem. Dit kunnen sommige schildklierziekten, vitiligo (witte plekken op de huid), allergische aandoeningen als eczeem en astma of bepaalde vormen van bloedarmoede zijn. Of stress alopecia areata kan uitlokken of verergeren is niet bekend. Waarschijnlijk spelen stressfactoren slechts een ondergeschikte rol.

A European and Scandinavian consortium of 14 clinics in 12 countries are planning a large scale study to define general alopecia areata susceptibility and resistance genes in families of alopecia areata affected individuals.
