During the past few years, the treatment of many cardiac arrhythmias has tended to move away from drug therapy and toward device-based therapy. The latter may involve pacemakers, implantable defibrillators, or catheter ablation, depending on the type of arrhythmia. When drug therapy is the preferred route, the agents used are different from those selected several years ago. The treatment changes affect the management of the most common arrhythmias seen in primary care practice, including atrial fibrillation, atrial flutter, supraventricular tachycardia, and ventricular arrhythmia. The prevalence of atrial fibrillation increases with age; this arrhythmia is most often seen in patients older than 65 years. Atrial fibrillation is typically associated with some form of cardiovascular disease, such as hypertension, coronary artery disease, or valvular heart disease, but it can also occur secondary to metabolic disorders such as thyrotoxicosis. In a minority of young patients, there is no obvious cause, a condition known as lone atrial fibrillation.

It's most likely why he passed out January 13 in the highly publicized pretzel incident. Bradycardia means that the president's heart beats more slowly than the average person's heart. It gets its name from the Greek bradu-, from bradus (slow) + kardi (heart). 'Sinus' refers to the sinus node, which is the heart's natural pacemaker. The press has reported that Bush has a slow heart rate. In August, after President Bush underwent a physical exam, the White House reported that his resting heart rate was 43 beats per minute. On Jan. 14, the day after the president's fainting episode, the Los Angeles Times reported that Bush has "a resting pulse rate of 35 to 45 beats per minute, which medical personnel consider extremely low even for a well-trained athlete." White House spokesman Ari Fleischer told reporters the day after Bush's fainting spell that the president's resting heart rate was 51 beats per minute. But up until now, the White House has not volunteered, and the media have not reported, that the president's slow heart rate is technically a heart arrhythmia called sinus bradycardia. But now the White House is confirming this.

In some instances, these episodes are symptoms of a heart disturbance. Arrhythmia an irregular heart rhythm can develop for the first time during pregnancy or become worse in women who already have an abnormal rhythm. "It is not uncommon for pregnant women to become lightheaded or to faint," said Dr. Richard Page, an associate professor of internal medicine at UT Southwestern Medical Center at Dallas. "Most of the time it has to do with other changes in the blood flow and circulation during pregnancy, but sometimes it is due to a heart-rhythm problem that needs to be addressed." Treating such a patient is filled with potential complications and complexities as well as gratification, Page said in a recent article, "Treatment of Arrhythmias During Pregnancy," published in the American Heart Journal Arrhythmia disturbances impair the heart's ability to pump blood adequately. "That's why someone might pass out, for example; there's not adequate blood flow to the brain," said Page, who is also director of the Clinical Cardiac Electrophysiology Unit at UT Southwestern and at Parkland Memorial Hospital.

A blood clot that blocked blood flow to his heart triggered it to quiver instead of beat. Medical technicians were unable to restore a normal heart rhythm, and by the time he reached a hospital, Grinkov's heart had stopped beating altogether. Grinkov suffered what is known as ventricular fibrillation. This inefficient and deadly quivering of the heart is one of several types of irregular heartbeats, or arrhythmias, that afflict the lower chambers, called the ventricles, of the heart. Not all arrhythmias are ventricular; some, for example, arise from the upper chambers of the heart. Ventricular arrhythmias often occur in people with various forms of heart disease and, according to the American Heart Association, cause most cases of sudden cardiac death. But new drugs and devices show promise in curbing the number of deaths from this condition. These new treatments, approved by the Food and Drug Administration, include a longer-acting form of an antiarrhythmia drug, a wide availability of portable and implantable electrical devices that can spark a return to normal heart rhythm, and techniques for destroying heart tissue that triggers ventricular arrhythmias. Heart of the Matter Not much bigger than a fist, the human heart beats 100,000 times each day, sending about 2,000 gallons of blood coursing through vessels, which, laid end-to-end, would be long enough to circle the earth more than twice.

In general, there are 4 types of EKG abnormalities causing irregular heart rate in these patients: 1. Vagal arrhythmia 2. Premature atrial contractions (PAC) 3. Second degree heart block, Type I (Wenckebach) 4. Premature ventricular contractions (PVC) Vagal arrhythmia refers to the normal variations in vagal tone that are associated with changes in sinus rate, or wandering pacemaker (from high right atrium to low right atrium), or junctional rhythm which self terminates. Studies have shown that up to 40% of normal children have an intermittent junctional rhythm on holter monitoring during sleep. If asymptomatic, these problems require no further workup; if there are symptoms (eg syncope) holter monitoring and exercise testing are warranted. Very rarely, these patients require pacemakers. PAC's are common up to 20% of normal children have occasional PAC's on 24 hour holter monitoring. If there are no symptoms suggestive of supraventricular tachycardia, no further workup or treatment are needed. Precipitating factors of frequent PAC's may include hyperthyroidism, myocarditis, and sympathomimetic medications.
