Although the causes of low back disorders are complex, substantial scientific evidence identifies some work activities and awkward postures as significantly contributing to the problem. In the United States, back disorders account for 27 percent of all nonfatal occupational injuries and illnesses involving days away from work. Prevention activities should be undertaken based on current knowledge, but important new research efforts are needed to assure that work-related low back disorders are successfully prevented and treated. For some occupations and tasks, there is a pressing need for more information about safe levels of exposure and for further validation of promising intervention approaches such as mechanical lifting devices for nursing aides. TEAM MEMBERS ADDITIONAL INFORMATION Back pain is one of the most common and significant musculoskeletal problems in the world. In 1993, back disorders accounted for 27% of all nonfatal occupational injuries and illnesses involving days away from work in the United States. The economic costs of low back disorders are staggering. In a recent study, the average cost of a workers' compensation claim for a low back disorder was $8,300, which was more than twice the average cost of $4,075 for all compensable claims combined. Estimates of the total cost of low back pain to society in 1990 were between $50 billion and $100 billion per year, with a significant share (about $11 billion) borne by the workers' compensation system. Moreover, as many as 30% of American workers are employed in jobs that routinely require them to perform activities that may increase risk of developing low back disorders.

They can prove costly for the individual, the business and health service providers. This information sheet cites epidemiological evidence regarding the prevalence and economic impact of musculoskeletal disorders of the back. Much of the information for this fact sheet derives from two surveys conducted by HSE in 19902 and 19951. It is strongly recommended that those interested in the data presented below read the two publications from these surveys that will provide further information of relevance and put the data in context. Since the publication of the results from the 1995 survey revisions have been made to the number of working days lost due to illnesses caused by work. An information sheet3 giving detailed information of the revised data can be obtained from HSE's Epidemiology and Medical Statistics Unit. Details of other sources of statistics quoted in this fact sheet can be found in the latest edition of HSE's publication: "Health and Safety Statistics"5.

In 1995, Kim and Chung measured fatigue arising from repetitive manual materials handling (MMH) tasks. They found more pronounced fatigue in asymmetric versus symmetric postures during task performance. Also, they found that muscles become fatigued more quickly during light load-high frequency tasks such as those performed at the Animal Care Facility than for heavy load-low frequency tasks. Related to this, Allread, Marras and Parnianpour (1996), reported that one-handed lifting results in back motion characteristics previously associated with lower back disorders (LDB). The weights used in this study also correspond well with those encountered at the Animal Care Facility, for instance 7.5 lbs (3.4 kg, about the weight of the shelter boxes in the pens) and 15 lbs (6.8 kg, about the weight of the an adult woodchuck and stovepipe shelter). In the breeding facility, asymmetric lifting occurs at least 100 times each day per worker. According to the research findings, one-handed lifting tasks such as, lifting woodchucks and their shelters and scooping feed from the barrels, increase the risk that employees will suffer from increased fatigue and lower back disorders. Allread, et al. also reported that in previous studies, manual materials handling was found to be the dominant factor in occupationally related LBD.

The purpose of the present study was to determine the extent to which the type of aircraft flown is associated with diagnosed back problems, and to examine differences in the prevalence of back disorders between pilots and aircrew. Methods: A case control study was conducted in which U.S. Navy pilots and aircrew members with a diagnosed back disorder on their most recent physical exam between 1991 and 1993 were compared with pilots and aircrew without such diagnoses. Data were obtained from the automated physical examination records maintained by the Naval Aerospace and Operational Medical Institute for all Naval aviation personnel. Results: Results showed that aircrew members have a higher risk of diagnosed back problems than pilots for both helicopters and fixed-wing aircraft. The study revealed that flight engineers have a higher risk of diagnosed back problems than other aircrew members.
