Extractions: SOUTHWEST GEORGIA REGIONAL SPINE CENTER DISC DISORDERS OF THE Intervertebral discs are the "cushions" of cartilage between the bony vertebral bodies in the cervical, thoracic, and lumbar spine. There existence allows the body to accept and dissipate load across multiple levels in the spine and still allow for the flexibility required for performing normal activities of daily living. As the body twists, bends, flexes and extends; the intervertebral discs are constantly changing their shape. The discs themselves are composed of a tough, outer fibrous layer (the annulus fibrosis) and a soft, almost gelatinous inner layer (the nucleus pulposis). As a part of the aging process, the discs begin to lose their high water content and their ability to dissipate force as efficiently (SO CALLED DEGENERATIVE DISC DISEASE). By the age of 35, approximately 30% of people will show evidence of disc degeneration at one or more levels. By the age of 60, greater than 90% of people will show evidence of disc degeneration at one or more levels on MRI. In some patients, this disc degeneration can be nearly asymptomatic; in others, disc degeneration can lead to intractable back pain. As the discs deteriorate, the spine can initially become less stable. Bony spurs can develop as a result of this instability and can cause pressure on nearby nerves leading to leg or arm pain. Narrowing of the neural canal by these bony spurs is known as degenerative spinal stenosis. The outer layer of the discs themselves can also tear. When this occurs, the inner, gelatinous layer can herniate out ( A "HERNIATED" OR "RUPTURED" DISC) and also cause pressure on an adjacent nerve. If the herniation occurs in the neck and causes pressure there, this can cause pain that radiates into the shoulder and arm; if it occurs in the back, the pain produced can radiate down into the hip and leg.

TREATMENT COMMON SYMPTOMS OF HERNIATED DISCS A patient with a herniated or "slipped" disc can have leg and/or back pain. The most common presenting complaint is that of shooting pain down one or, although uncommon, both legs. The pain will typically travel from the back or buttocks to below the knee, following the path of a specific nerve root. The following are drawings of typical nerve root distributions. The pain does not always fit neatly into one of these patterns, but it is helpful in determining which disc is herniated. Additionally, a patient may experience numbness or weakness with a herniated disc. The numbness is most frequently reported to be in the calf, the sole of the foot or the big toe. The weakness is often experienced by the patient as an inability to walk up the stairs normally or to walk on the toes. The weakness might present as an inability to lift the foot while walking (a "foot-drop"), or weakness in flexing or extending the knee, foot, or leg.

Extractions: Low-back pain (LBP) is common in the general population: lifetime prevalence has been estimated at nearly 70% for industrialized countries; sciatic conditions may occur in one quarter of those experiencing back problems [Andersson 1981]. Studies of workers' compensation data have suggested that LBP represents a significant portion of morbidity in working populations: data from a national insurer indicate that back claims account for 16% of all workers' compensation claims and 33% of total claims costs [Snook 1982; Webster and Snook 1994b]. Studies have demonstrated that back disorder rates vary substantially by industry, occupation, and by job within given industries or facilities [see Bigos et al. 1986a; Riihimaki et al. 1989a; Schibye et al. 1995; Skovron et al. 1994]. Back disorder is multifactorial in origin and may be associated with both occupational and nonwork-related factors and characteristics. The latter may include age, gender, cigarette smoking status, physical fitness level, anthropometric measures, lumbar mobility, strength, medical history, and structural abnormalities [Garg and Moore 1992]. Psychosocial factors, both work- and nonwork-related, have been associated with back disorders. These relationships are discussed at length in Chapter 7 and Appendix B.

ANXIETY DISORDERS Everybody knows what it's like to feel anxious the butterflies in your stomach before a first date, the tension you feel when boss is angry, the way your heart pounds if you are in danger. Anxiety rouses you to action. It gears you up to threatening situations. It makes you study harder for that exam, and keeps you on your toes when you're making a speech. In general, it helps you cope. But if you have an anxiety disorder , this normally helpful emotion can do just the oppositeit can keep you from coping and can disrupt your daily life. Anxiety disorders aren't just a case of "nerves." They are illnesses, often related to the biological make-up and life experiences of the individual, and they frequently occur in families. There are several types of anxiety disorders, each with it's own distinct features. An anxiety disorder may make you feel anxious most of the time, without any apparent reason. Or the anxious feelings can be so uncomfortable that to avoid them you may stop everyday activities, or you may have occasional bouts of anxiety so intense they are terrifying and they immobilize you. Anxiety disorders are the most common of all mental disorders, at the National Institute of Mental Health (NIMH), the Federal Agency that conducts and supports research related to mental disorders, mental health, and the brain, scientists are learning more and more about the notoriety of anxiety disorders, their causes, and how to alleviate them. While there are many different disorders that can produce mechanical back pain, the following pages explain some of the more common causes of this type of pain. Degenerative Disc Disease Just like the hip and the knee, the facet joints can also become arthritic and painful, and they can be a source of back pain. The pain and discomfort that is caused by degeneration and arthritis of this part of the spine is called facet arthropathy, which simply means a disease or abnormality of the facet joints.
