Extractions: is a chronic granulomatous inflammatory disease of the gastrointestinal tract of unknown cause and is a strong possibility in this case. Bloody diarrhoea is a recognised feature of Crohn's colitis. Colonic disease is associated with perianal disease in just over 30% of patients. A seronegative reactive arthritis is a recognised complication of Crohn's disease. Erythema nodosum occurs in some case. Genital ulcers are rare as is deep vein thrombosis. Urethral involvement and dysuria only occur when an inflammatory fistula develops between the colon and the ureter. Recurrent urinary tract infections due to faecaluria can cause urethral stricture.

Hughes** *Department of Internal Medicine, Shiraz University of Medical Sciences, Shiraz, Iran, and **Lupus Arthritis Research Unit, The Rayne Institute, St. Thomas Hospital, London, UK ABSTRACT To evaluate a group of patients with multiple sclerosis (MS) for antiphospholipid syndrome (APS), 39 unselected patients with MS (26 females and 13 males) were included in this study. Medical notes were retrospectively reviewed and anticardiolipin (aCL) test was performed on their stored sera. Five positive aCL-IgG (13%) were found in 39 sera, two of them with moderate titres, and three with low titres. In reviewing the medical notes we found that the two patients with moderate aCL titres had clinical features of APS, while the other three did not have any of the major clinical manifestations. It is concluded that some cases presenting with MS symptomatology should be evaluated for APS, as clinical presentations are sometimes similar in both diseases. Sanyal , M.B.B.S., M.D. a Transjugular intrahepatic portasystemic shunts (TIPS) provide a low-resistance pathway between the hepatic veins and the intrahepatic portion of the portal vein for blood to return to the heart in patients with portal hypertension thereby bypassing and decompressing the hepatic sinusoids. The ability to achieve portal venous decompression like a surgical portacaval shunt by TIPS without the attendant risks of general anesthesia and major surgery have led to a resurgence in interest in mechanical portal decompression for the treatment of portal hypertension. While several large controlled studies have examined the role of TIPS in the management of sinusoidal portal hypertension ( ), its use for postsinusoidal and especially presinusoidal portal hypertension remain to be fully evaluated. In this issue, Ganger et al ) extend the body of anecdotes and small series existing in literature ( ) by documenting the efficacy and safety of TIPS for postsinusoidal (Budd-Chiari syndrome) or presinusoidal (portal vein thrombosis) portal hypertension. The study by Ganger et al ) as well as the numerous previous reports in literature suggest that TIPS may have a beneficial effect in those with Budd-Chiari syndrome. However, when considering the therapeutic options for a given subject, one not only has to consider the efficacy and safety of TIPS in absolute terms but also relative to the other modalities available. Unfortunately, there are no randomized controlled trials to guide therapy. One must then rely on a clear understanding of the principles of management of Budd-Chiari syndrome to determine the optimal treatment for a given subject.
