Extractions: Canavan disease (CD) is an inherited (genetic) disease. It occurs due to a deficiency of the enzyme called Aspartoacylase, since the gene which usually tells the body to produce this enzyme is faulty. Babies are born apparently unaffected. At 2 to 4 months of age, he/she develops poor head control and seizures. Other symptoms are observed before the age of 6 months such as loss of grasping reflex, smiling and increased head circumference. Death usually occurs before 36 months. Canavan disease is a disease caused by inheriting a 'double-dose' of a faulty in a gene (called ASPA) which codes for Aspartoacylase. Every person has two copies of this gene in each cell of their body. A person will only be affected by CD if s/he has a mutation in both copies of the ASPA gene. This can only happen if one faulty copy is inherited from each parent. One in every 40 Ashkenazi Jews carries one copy of the faulty gene and one regular copy of the gene. In the general population, however, only about one in 300 people carry a faulty copy of the ASPA gene. Carriers themselves are not affected by the disease.

Canavan disease is one of a group of genetic disorders called the leukodystrophies that affect growth of the myelin sheath of the nerve fibers in the brain. The myelin sheath is the fatty covering surrounding nerve cells that acts as an insulator. Symptoms of Canavan disease, which appear in early infancy and progress rapidly, may include mental retardation, loss of previously acquired motor skills, feeding difficulties, abnormal muscle tone (i.e., floppiness or stiffness), poor head control, and megalocephaly (abnormally enlarged head). Paralysis, blindness, or hearing loss may also occur. Although Canavan disease may occur in any ethnic group, it affects persons of Eastern European Jewish ancestry more frequently. There is no cure for Canavan disease, nor is there a standard course of treatment. Treatment is symptomatic and supportive.

The underlying defect is one of aspartoacylase deficiency. A defect in N-acetyl aspartic acid metabolism (with excess in the urine) is associated with failure of myelin - a lipid that wraps around much of the neural structures so that they can work properly. By the age of three months, the Greenbergs began to worry when Jonathan was unable to control his head, make eye contact, or put his hand to his mouth. The Greenbergs sought the advice of one specialist after another until six months later Jonathan was diagnosed with Canavan disease. Similar to Tay-Sachs disease, Canavan disease occurs most frequently in families of Ashkenazi Jewish decent and leads to a degeneration of the brain. The Greenbergs learned that this rare genetic disease had no known cure and that their son would never function past the developmental level of an infant for all of his short life and slowly succumb to the disease's degenerative effects. Children affected with Canavan can smile, look around and hear, but as the illness progresses, they often lose most of their vision and are prone to seizures and infection. They develop difficulty chewing and swallowing and most eventually require tube feeding. Though the children become less responsive over time, they are never totally unresponsive or in a coma. Life expectancy varies, but most do not reach their teen years.

It is cased by a genetic abnormality which produces a deficiency of the enzyme aspartoacylase and is inherited in an autosomal recessive pattern. Like Tay-Sachs Disease, Canavan Disease occurs at a higher frequency in individuals with Ashkenazi Jewish ancestry, estimated to be 1/40. The screening test to identify individuals who are carriers is a DNA-based test that is performed by only a few specialized laboratories. If both reproductive partners have an Ashkenazi Jewish background, we recommend that carrier testing for Canavan Disease be offered before pregnancy. If only one partner is of Ashkenazi Jewish descent, she/he should be offered carrier testing and the couple should be counseled regarding the limitations and benefits of carrier testing and prenatal diagnosis for this situation.

The term leukodystrophy comes from the Greek words, 'leuko' meaning white and referring to the 'white matter' of the nervous system and 'dystrophy' meaning imperfect growth or development. In Canavan disease, there is widespread degeneration of the white matter in the brain leading to loss of sensory, motor and intellectual function. The white matter of the brain, otherwise known as myelin sheath, is a fatty covering which acts as an insulator around the nerve fibres of the brain. Myelin sheath is made up of a number of different chemicals. Canavan disease is caused by a deficiency in one of these chemicals and this affects the growth of the myelin sheath. Canavan disease is caused by changes (or mutations) in a gene on chromosome 17. There are different types of Canavan disease which include congenital, infantile, and late-onset form. Babies with Canavan disease appear normal in the first few months of life. However, in early infancy they begin to lose previously acquired skills including seeing and hearing. The head becomes progressively enlarged (megalencephaly) as the brain swells and the bones of the skull fail to fuse.
