The suggestion that even a single form of cancer might actually be eliminated seems at first like nothing more than a cruel exaggeration. The study is ongoing, but the early results reverberated through the medical world. "Everybody is amazed at these results, because of the 100 percent protection," says Thomas Wright, a gynecologic pathologist at the College of Physicians and Surgeons of Columbia University in New York. "The potential for this is just fabulous." Although most HPV infections simply go away on their own, in some women, the virus leads to cell abnormalities, a precancerous condition called dysplasia, and, in time, to cervical cancer. The research path to such a vaccine began about 20 years ago, when scientists established that several types of sexually transmitted HPV are responsible for cervical cancer. This medical advance meant that cervical cancer could be thought of as a sexually transmitted disease and that women could avoid the deadly disease by protecting against the virus.

Extractions: New research suggests that the combination of herpes and HPV, another common sexually transmitted virus, may raise the risk that infected women will develop cervical cancer. The findings may explain why some women are more prone to cervical cancer. "We know that HPV (human papillomavirus) causes cancer, but a lot of people have HPV and never get cancer," explains Stephen E. Hawes, an assistant professor of epidemiology at the University of Washington at Seattle and author of a commentary on the new study. Researchers think at least 90 percent of cervical cancer cases are caused by HPV, which may be the most common sexually transmitted disease. HPV causes inflammation, which makes the cervix more hospitable to cancerous cells by preventing them from dying naturally, Hawes explains. "This process can take years to develop20 to 30 years after you have your initial HPV infection," he says. An estimated 13,000 American women fall victim to cervical cancer each year and 4,100 will die, according to the American Cancer Society . However, tests known as Pap smears (also called Pap tests), conducted regularly from the time a woman becomes sexually active to well past menopause, allow doctors to diagnose cervical cancer earlier, before it becomes serious. Deaths from the cancer dropped by 74 percent from 1955 to 1992, after the tests became widely used.

Extractions: The best test to detect the earliest stages of cervical cancer is extremely effective. The pap test or the pap smear is still the gold standard. It involves taking a swab of cell samples from the cervix and testing them for precancerous changes that might signal the early development of cancer. If cervical cancer is caught early, it can be treated very successfully in most cases. The importance of screening is highlighted by the statistics. Cervical cancer kills more than 4,000 women in the U.S. each year. And 13,000 new cases are diagnosed every year. Of those newly-diagnosed women, 50 percent have never had a pap test, and another 10 percent haven't had a pap test within five years of their diagnosis.

However, many of these cases could have been avoided if women had access to screening and treatment programs. In most cases precancerous lesions go undetected simply because women do not have access to screening tests such as Pap smears. To address this problem, JHPIEGO began exploring the feasibility of several low-cost alternatives to Pap smears, including unmagnified (naked eye) visual inspection of the cervix using a dilute solution of acetic acid (VIA), that could be implemented in low-resource settings.
