Peripheral neuropathy (PN), which encompasses a wide range of abnormalities that affect the peripheral nerves, causes considerable morbidity and mortality. Glucose control seems to play a role in neuropathy, and abnormalities of glucose metabolism are common in old age. Estimates of the prevalence of PN in patients with diabetes (diabetic neuropathy [DN]) range from 10% to 90%, depending on the criteria used to define neuropathy and the age of the subject (Dyck et al., 1993; Knuiman et al., 1986; Young et al., 1993). Peripheral neuropathy is associated with pain, numbness, gait abnormalities and decrements in balance (Katoulis et al., 1997; Resnick et al., 2000; Simoneau et al., 1994). These have an effect on the walking and balance that are critical for maintenance of mobility, avoidance of disability and preservation of independence in the community (Guralnik et al., 1995). Diabetes may play a role in the development of neuropathy (Resnick et al., 2000), although aging itself may be associated with impaired neurologic function, independent of diabetes.

VINIK, MD, PhD This session was devoted to a discussion and presentation of the pathogenesis of diabetic neuropathy and the scope for different forms of intervention. It has become abundantly clear that diabetic neuropathy is not a single disorder but rather, a complex heterogeneous syndrome with a variety of clinical presentations based upon differences in etiologic mechanisms. Diabetic neuropathies can be divided into the focal and diffuse forms, and the latter may be further divided into small and large fiber neuropathies. This distinction is important since the clinical presentation is so varied and the pathogenesis and approaches to treatment are dictated by the underlying pathogenic mechanism. The focal forms of neuropathy are, for the most part, entrapment syndromes or mononeuritides that respond to specific forms of treatment aimed at the pathology. These generally do not behave like the diffuse somatic and autonomic neuropathies, are self-limiting, and in certain instances treatments can be directed at specific targets. For example, nerve entrapments may require splinting, diuretics, or local anesthetic and steroid injection. The situation is quite different in the proximal neuropathies that are often inflammatory conditions, which if recognized will respond to anti-inflammatory or immune therapies. The bain of existence is, however, the distal symmetric polyneuropathies (DSPN).

The forms of neuropathy are usually divided into, by order of frequency, polyneuropathies (primarily symmetric and sensory), autonomic neuropathies, and mononeuropathies. Diabetic amyotrophy, an uncommon syndrome defined by the triad of pain, severe asymmetric muscle weakness (usually of the lower extremities), and wasting of the iliopsoas and quadriceps, falls under the heading of the polyneuropathies. There has never been complete agreement about the etiology diabetic neuropathy. The mononeuropathies are most likely ischemic in nature, as suggested by the tendency toward nerves susceptible to compression and and toward fiber subtypes with the more tenuous blood supply. The polyneuropathies are thought to be caused by a combination of ischemia and abnormal nerve metabolism. Mononeuropathies Although there is no great prevalence data, mononeuropathies are known to be more common among diabetics than non-diabetics. One half to one percent of diabetics are expected to have a mononeuropathy at some point. Symptoms are generally localized to common sites of nerve entrapment (supporting the hypothesis that nerve injury occurs when an already tenuous blood supply is further compromised by external compression or nerve swelling). The sights of most common involvement are shown in the table below. This comes from a study which enrolled the 51 patients who presented to a large diabetes with mononeuropathies over the course of 3 years.
