They do not form spores or branch as do the actinomycetes, but they have the characteristic of forming irregular shaped, club-shaped or V-shaped arrangements in normal growth. Virulent strains of C. diphtheriae produce a toxin which can damage heart and nervous tissues, although non-toxigenic strains can cause a localised infection. A diphtheria-like illness is sometimes caused by another toxin-producing bacterium Corynebacterium ulcerans . Immunisation against diphtheria was introduced on a national scale throughout the United Kingdom in the early 1940s and resulted in a dramatic drop in the number of cases and deaths from the disease. For example, in 1940, 46,281 cases and 2,480 deaths were notified, compared with 37 cases and six deaths in 1957. Diphtheria is now rare in England and Wales, nearly all new cases are acquired abroad, and only two deaths due to diphtheria have been recorded since 1972. Diphtheria is an infectious disease spreading from person to person by respiratory droplets from the throat through coughing and sneezing. The disease normally breaks out 2 to 5 days after infection. Diphtheria usually affects the tonsils, pharynx, larynx and occasionally the skin. Symptoms range from a moderately sore throat to toxic life-threatening diphtheria of the larynx or of the lower and upper respiratory tracts. Diphtheria is often complicated by diphtheric myocarditis (toxic damage to heart muscles) and neuritis (toxic damage to peripheral nerves). The disease can be fatal - between 5% and 10% of diphtheria patients die, even if properly treated. Untreated, the disease claims even more lives. Untreated patients are infectious for 2 to 3 weeks. Treatment consists of immediate administration of diphtheria antitoxin and antibiotics. Antibiotic treatment usually renders patients non-infectious within 24 hours. Unless immunized, children and adults may repeatedly be infected with the disease.
