The End-Stage Renal Disease (ESRD) Workgroup worked to improve supportive care and quality of life for dialysis patients from Spring 2000 Spring 2002. Chaired by Alvin Moss, MD (Director of the West Virginia University Center for Health Ethics and Law), this workgroup consisted of 23 members including nurses, social workers, nephrologists, and kidney transplant recipients. To accomplish its goals, the workgroup divided into three subgroups Education, Quality of Life, and Quality of Dying. The ESRD Workgroup made considerable progress in bringing palliative care issues to the attention of the nephrology community, including an invitation from the Chair of the American Board of Internal Medicine Subspecialty Board on Nephrology to submit questions on end-of-life care for the board certification examination. In addition, the Renal Physicians Association (RPA) and the American Society of Nephrology's (ASN) joint Quality Patient Care Committee meeting revised their position statement on Quality Care at the End-of-Life to incorporate workgroup findings in the Fall of 2001. This revised position statement has been widely circulated. Extractions: Pathogenesis of renal osteodystrophy Ari Kostadaras, M.D. Changes in mineral metabolism and bone structure are an almost universal concomitant of progressive renal failure. There are two classic types of renal bone disease, with many patients showing evidence of both disorders (called mixed osteodystrophy): Osteitis fibrosa, in which bone turnover is increased due to secondary hyperparathyroidism. Osteomalacia, in which bone turnover is reduced with an increased volume of unmineralized bone (osteoid); this problem is in most, but not all patients due primarily to aluminum deposition in bone. Symptoms due to these disorders, such as fractures and bone pain with osteomalacia or bone pain in osteitis fibrosa, generally do not occur until the patient is already on maintenance dialysis. However, subclinical disease begins early, particularly with osteitis fibrosa. SECONDARY HYPERPARATHYROIDISM AND OSTEITIS FIBROSA A tendency to phosphate retention, beginning early in the disease as the fall in glomerular filtration rate (GFR) decreases the filtered phosphate load, plays a central role in the development of secondary hyperparathyroidism . Over the long-term, however, the hyperparathyroidism is maladaptive, producing bone disease and contributing to dysfunction in other organs. Furthermore, there is a changing effect of PTH on phosphate balance in advanced renal failure.
