What is going on in the body? Certain injuries or diseases can create a hole in the esophagus. When food is swallowed, some of it can leak out of the esophagus into the chest cavity. These ulcers can eat through the wall of the esophagus, causing a hole. The esophagus is the tube that connects the mouth to the stomach. These veins may sometimes bleed. What is going on in the body? Esophageal varices usually form because of a serious liver disease called cirrhosis . Cirrhosis of the liver can interfere with blood circulation. This leads to increased pressure in the veins around the esophagus. Over time, these veins widen due to the pressure. When the walls of the veins get stretched too much, the veins can break and allow blood to enter the esophagus. What are the signs and symptoms of the condition? Symptoms often happen quite suddenly and include: coughing up or vomiting up blood black, tarry stools due to the bleeding in the gut lightheadedness from loss of blood passing out from loss of blood What are the causes and risks of the condition? Extractions: By Don Norwood Esophageal cancer is one of the deadliest cancers, largely because it is usually not detected until it has reached an advanced stage. It is also an uncommon form of cancer in Western countries, but a steady rise in the incidence of one type of esophageal cancer over the past 15 to 20 years is threatening to change that. "Adenocarcinoma of the esophagus has assumed increasing importance as a cancer in the United States because the rate of it is increasing dramatically," said Stanley R. Hamilton, M.D., head of the Division of Pathology and Laboratory Medicine at The University of Texas M. D. Anderson Cancer Center. "In some of the studies that have been done, it looks like it is perhaps the fastest-increasing tumor type in the United States." The increased incidence of adenocarcinoma has made it more prevalent than squamous cell carcinoma of the esophagus in the United States. Tobacco, alcohol, and diet have been identified as risk factors for squamous cell carcinoma, but the risk factors associated with adenocarcinoma are less clear. What investigators do know is that gastroesophageal reflux disease (GERD), a condition that is more common in obese people, is associated with the occurrence of esophageal cancer, especially adenocarcinoma.

Extractions: Last Revision Date: February 2001 A 78-year-old Caucasian woman presented because of daily episodes of shortness of breath. She described periods of wheezing that typically occurred during or immediately after her evening meal. This was not associated with coughing or chest pain. She was able to do her daily chores without dyspnea or respiratory problems. She also complained about dysphagia for liquids and solids and chest discomfort, which had started about 2 years prior to her clinic visit. She described her symptoms as burning retrosternal pain. In the past she had used H receptor blockers for presumed heartburn, which improved her symptoms slightly. Occasionally, she "choked" right after swallowing her food. She had lost about 7 kg over the last 2 years. Her prior medical history was remarkable for Parkinsons disease and three myocardial infarctions. On physical examination, she had dentures, a normal cardiovascular, pulmonary and abdominal examination. The neurological examination revealed a resting tremor and mild rigor with cogwheel phenomenon. The cranial nerves were intact and she was able to swallow some water without aspiration or regurgitation. The history described above was consistent with bronchopulmonary complications due to dysphagia with microaspiration. An elderly person with Parkinsons disease and dentures is certainly at risk for aspiration because of oro-pharyngeal dysphagia. However, the chest discomfort and significant weight loss pointed more at an esophageal etiology. To further differentiate between these two potential causes, a cine-esophagram was obtained. It demonstrated a significantly dilated esophagus without primary peristalsis

Extractions: Gastrointestinal and Hepatobiliary Surgery General Information The Gastrointestinal Surgical Service has been active within the Department of Surgery and provides comprehensive care for patients that are suffering from surgical diseases of the gastrointestinal tract. This group works in close cooperation with members of the Cancer Center, the Digestive Health Center, the Women's Center as well as other Surgical Services including the Laparoscopy Center. Specifically, patients are seen and evaluated in clinic, receive either surgical or non-surgical evaluation and are evaluated postoperatively on an acute or chronic basis. Diagnostic and therapeutic services for patients with a wide spectrum of gastrointestinal disease including benign or malignant diagnosis are seen and evaluated by the Gastrointestinal Surgical Group. Additionally, this service provides complete diagnostic and therapeutic use of laparoscopy for diverse problems such as gastrointestinal bleeding, polyps, and cancer. The Gastrointestinal Surgery Group also works closely with many members of the Colo-rectal Surgery Group. We strive to provide a full range of educational opportunities for physicians, nurses and patients in the treatment of gastrointestinal disease and are active in the recruitment of patients for up-to-date clinical protocol for evaluation of therapeutic diagnostic modalities.
