A person may also self-infect by having an area of infected skin or tissue touch another area on the person's own body. HPV affects both sexes and all races equally, though it is most commonly found in those aged 15 to 40 years old. Anyone who is sexually active, especially anyone with multiple sexual partners, is at risk of exposure to HPV. Persons whose immune systems are not working well are more likely to become infected and to have a more serious infection. Although the appearance of genital warts may be what most people dislike, the truly serious issue related to HPV infection is the strong association between certain types of HPV and conditions of dysplasia (a pre-cancerous cell growth) and squamous cell cancer - especially of the cervix in women. Some types can cause warts on the hands or feet, while others can result in genital warts (condyloma). A person can be infected simultaneously with multiple HPV types. WHAT DO GENITAL WARTS LOOK LIKE? Genital warts are flesh colored, firm growths or bumps that appear on the vulva, in or around the vagina or anus, on the cervix, and on the penis, scrotum, groin, or thigh. The warts may be raised or flat, single or multiple, small or large, and some may cluster together to form a cauliflower-like shape. HOW DO YOU GET HPV AND WHEN DO WARTS APPEAR? HPV infection is usually spread by direct skin-to-skin contact during vaginal, oral, or anal sex with an infected individual. Very rarely, genital HPV will be spread through oral sex to mucous membranes of the mouth. After exposure, warts may begin to appear within several weeks; or they may take months or even years to appear; or they never appear at all. Most sex partners of infected persons probably are infected subclinically with HPV. From 60-90% of male partners of women with condyloma have HPV infection on the penis, although infection may not be visible to the naked eye. Whether partners who have subclinical HPV infection are as contagious as persons who have visible warts is not known. Not everyone who has come in contact with the virus will be infected.

Genital warts Common warts usually grow around the nails, on the fingers and on the backs of the hands. They are more common where skin has been broken, for example where fingernails are bitten or hangnails picked. Foot warts are usually on the soles of the feet and are called plantar warts. When plantar warts grow in clusters they are known as mosaic warts. Most plantar warts do not stick up because the pressure of walking flattens them and pushes them back into the skin. These warts often have black dots that are actually blood vessels. Plantar warts have a bad reputation because they can be painful, feeling like a stone in the shoe, and are often difficult to resolve. Flat warts are smaller and smoother than other warts. They tend to grow in great numbers - 20 to 100 at any one time. They can occur anywhere, but in children they are most common on the face. In adults they are often found in the beard area in men and on the legs in women. Irritation from shaving probably accounts for this.

Only a small percentage of people infected with HPV will develop genital warts. Many persons are "carriers" of HPV and may never develop warts but may still be able to pass HPV to their sexual partners. Although the incubation period from contacting HPV until development of warts may be several months, some people may not develop warts for years after contact with HPV.

Genital warts (condylomata acuminata), are caused by the human papillomavirus (HPV) and usually occur in the genital and rectal areas; they can also occur in the mouth or other areas. Common skin warts of the hands or feet are caused by a similar virus, but they do not cause genital warts. HPV is a virus consisting of more than 70 different types. Certain types cause genital warts. Sometimes HPV will not cause any symptoms at all but is still capable of being transmitted because the virus (HPV) is present in the tissue of the genitals or rectum. This is called an "asymptomatic infection" . HPV can also cause warts that appear flat on the surface of the skin or in areas where they are difficult to be seen (i.e. inside the penis or vagina or on the cervix); this is called a "subclinical infection" Genital warts may appear several weeks after exposure to HPV. Some may take months to appear or they may never appear. When they are visible, the warts appear singly or in clusters. You may have spotted them yourself, or they may have been noticed by a doctor, nurse or your partner. Usually they are not painful although they can sometimes be irritable. A number of skin conditions are seen at GUM clinics and genital warts are seen more frequently than any other. Warts can be passed on during sexual intercourse and close body contact. It is therefore important that your sexual partner is checked at a clinic for warts/ and other common infections which can encourage the growth of warts. It is possible to develop them in or around the mouth after oral sex with someone who has genital warts though this is uncommon. It is not unknown for them to be passed from the fingers to the genital area. It is probably pointless to avoid sexual intercourse until the warts have been cleared. Condoms are usually ineffectual in preventing the spread of warts. However they are usually a very effective means of reducing the risk of other sexual infections and therefore should be considered even after the warts have disappeared. Remember however that warts only need skin to skin contact to be passed on. Current partners may benefit from a check-up too.

Lesions are pedunculated or borad based with pink to gray soft excrescences, occuring in clusters or individually. External warts seldom require biopsy for diagnosis. The differential diagnosis of anogenital warts includes molluscum cantagiosum, verruca vulgaris (common non-genital wart), secondary syphilis (condyloma lata), hypertrophic vulvar dystrophies and vulvar intraepithelial and invasive neoplasias. Consider atypical, pigmented, intravaginal cervical and persistant warts for referral for biopsy. Prescribe podofilox 0.5% solution (Condylox) 3.5mL for self-treatment. Patients may apply podofilox with a cotton swab to warts twice daily for three days, followed by 4 days of no treatment. This cycle may be repeated as necessary for a total of 4 cycles. Total wart area treated should not exceed ten square centimeters and total volume of podofilox should not exceed 0.5mL per day. If possible, you should apply the initial treatment to demonstrate the proper application technique and identify which warts should be treated. Alternatively, apply 25% podophyllin in tincture of benzoin (Podocon-25) 15mL using the above application technique and with the same dosage restrictions. Have the patient thoroughly wash off in 1-4 hours. This may be repeated weekly if necessary but if warts persist after six applications the patient should be referred for alternative therapy.
