Extractions: 1. Open endothelial fenestrae and filtration slits between podocyte foot processes are necessary for proper glomerular filtration. To keep the filtration slits open podocytes must maintain sophisticated cytoskeletal organization, intercellular junctions between the foot processes, and adhesion to the GBM matrix. In addition the apical membrane of podocytes and endothelial cells is covered by a rich glycocalyx. Podocalyxin protein is a major component of the glycocalyx. It may have an anti-adhesive role, but its functions are not known. Our recent findings show that podocalyxin is also present in platelets and is made by megakaryocytes. Key Concept/Objective: To understand the risk of disease recurrence in patients with primary glomerular disease The recurrence rates of different primary renal diseases vary. Primary glomerular diseases frequently recur in the transplanted kidney; however, graft loss secondary to recurrence is uncommon. The patients who are at greatest risk of graft loss are those in whom renal function deteriorated rapidly and aggressively. In these patients, transplantation may be relatively contraindicated. Lupus nephritis, anti-GBM disease, and membranous nephropathy have low recurrence rates and are rarely associated with graft loss. Type II membranoproliferative disease has a high recurrence rate (80% to 90%); however, it too is associated with a low incidence of graft loss. Patients with Alport syndrome can develop anti-GBM disease in the allograft, although this is uncommon, and Alport syndrome is not a contraindication to transplantation.

Also, first in vivo studies point to an altered ECM as a relevant factor in the pathogenesis of glomerular diseases. Adhesion of MC to the ECM is mediated by receptors of the integrin family. Typically, MC in vitro and in vivo express a1b1, a5b1, a8b1 and avb3 integrins. Objective of our studies is to gain new insights into the relevance of MC-expressed integrin receptors and their interaction with ECM ligands in the pathogenesis of glomerular diseases. The long-term aim of our investigations is the development of new strategies for improved diagnostics and therapy of glomerular diseases.

Extractions: In vitro, we will study the regulation of myocilin expression in cultured podocytes and mesangial cells and the transcriptional regulation of the myocilin promoter. An important aspect of these investigations will be to analyze, if, similar to other tissues, the glomerular expression of myocilin is under the influence of dexamethasone, TGF-beta or mechanical factors. Finally, we will establish transgenic mice with sequences of the tetracycline transactivator protein under control of the nephrin promoter to establish a transgenic model of tetracycline-inducible gene expression in podocytes. We are confident that the data resulting from this project will enhance our knowledge on the role of a new family of glomerular proteins and result in novel approaches to understand glomerular diseases.
