A direct blow to the jaw or the side of the head can result in bone fracture, soft tissue bruising, or a dislocation of the temporomandibular joint itself. Arthritis. Both osteoarthritis and rheumatoid arthritis can cause TMJ. Internal derangement. Internal derangement is a condition in which the cartilage disk lies in front of its proper position. In most cases of internal derangement, the disc moves in and out of its correct location, making a clicking or popping noise as it moves. In a few cases, the disc is permanently out of position, and the patient's range of motion in the jaw is limited. Hypermobility. Hypermobility is a condition in which the ligaments that hold the jaw in place are too loose and the jaw tends to slip out of its socket.

In this article they propose that the degeneration of hyaline cartilage occurs as a consequence of activation of monocytes, macrophages and neutrophils which release oxidants that in turn attack connective tissue and the joint lubricant substance hyaluronic acid. The activation of white cells that in turn release alarm substances and oxidants occurs as a consequence of an inflammatory cascade modulated through receptor sites on the surface of white cells which are stimulated by the presence of foreign molecules.

There are more than 100 different types of arthritis. Classification is often based on the cause and progress of the arthritis. Causes include infectious agents, metabolic disorders, trauma, and immune disorders. Mild exercise retards joint degeneration and enhances mobility. Swimming and walking are recommended for people with arthritis; but running, tennis, and aerobics are not recommended. Therapy depends on the type of arthritis, but includes anti-inflammatory drugs. Current research is focusing on the possible development of antibodies against the cells that initiate the inflammatory response in the joints or against cell surface markers on those cells. Osteoarthritis (OA) is the most common type of arthritis, affecting 16 million people in the United States. OA may begin as a molecular abnormality in articular cartilage, with heredity and normal "wear and tear" of the joint important contributing factors. Slowed metabolic rates with increased age also seem to contribute to OA. Inflammation is usually secondary in this disorder. It tends to occur in the weight-bearing joints such as the knees and is more common in overweight individuals. Lyme Disease Suppurative arthritis (pus-forming) may result from number of infectious agents. These joint infections may be transferred from some other infected site in the body or may be systemic (that is, throughout the body). Usually only one joint, normally one of the larger joints, is affected, and the course of suppurative arthritis, if treated early, is transitory. However, with prolonged infection the articular surfaces may degenerate. Tuberculous arthritis can occur as a secondary infection from pulmonary tuberculosis and is more damaging than typical suppurative arthritis. It usually affects the spine or large joints and causes ulceration of the articular cartilages and even erosion of the underlying bone. Transient arthritis of multiple joints is a common symptom of rheumatic fever, but permanent damage seldom occurs in joints with this disorder.

Here is a look at some common joint problems. Dislocation A dislocation (luxation) displaces the articulating bones of a joint and usually results from a fall or other unusual body movement. The joints of the shoulders, knees, elbows, fingers, and jaw are common sites for this type of injury. A dislocation produces an obvious deformity of the joint, some loss of ability to move the articulated bones, localized pain, and swelling. Sprains Sprains result from overstretching or tearing the connective tissues, ligaments, and tendons associated with a joint, but they do not dislocate the articular bones. Usually forceful wrenching or twisting sprains the wrist or ankles. For example, excessively inverting an ankle can sprain it as the ligaments on its lateral surface are stretched. Severe injuries may pull these tissues loose from their attachments. A sprained joint is painful and swollen, restricting movement. Immediate treatment of a sprain is rest; more serious cases require medical attention. However, immobilization of a joint, even for a brief period, causes bone resorption and weakens ligaments. Consequently, exercise may help strengthen the joint.

This is mainly based on clinical research. For a long time the research of our department has concentrated on clinical research concerning fractures, posttraumatic deformity, joint disorders and bone healing. Over the last years the aim of our research programme has been to supply a scientific basis for Orthopedic Surgery. Clinical research is concentrated on prospective randomized trials to improve diagnostic and therapeutic strategies for several joint disorders. Development of instruments and operative techniques are based on anatomic studies and mechanical testing. Mechano-biological processes, and mechanical bone quality and adaptation of bone to endoprotheses are studied. This includes the role of fibrous tissues around implants. A main part of the study is directed towards bone healing. Apart from prospective trials the anchondral ossification is studied in vitro.
