Kernicterus is a life-long neurological syndrome caused by severe and untreated hyperbilirubinemia during the infant's neonatal period. Placing the severely jaundiced babies, one of the early symptoms, under a special lamp, easily prevents the disease. In some severe cases an exchange transfusion may be necessary. But one of the maddening things about kernicterus, say people from the Centers for Disease Control and Prevention, is there has been no surveillance and there is no data on how widespread the problem really is other than word of mouth and anecdotal information. These parents are more than a little upset and determined because the disease that's hit their babies and grand babies a syndrome that can include choreoathetoid cerebral palsy, mental retardation, hearing loss and other disabilities is too frequently not spotted because of a number of simple oversights. Parents of Infants and Children with Kernicterus (PICKS) coat-tailed their advocacy efforts to Capitol Hill July 25 when they were present at a 50-year celebration of CDC's Epidemic Intelligence Service (EIS). But it was more than advocacy that brought them out. They came also to support one of their own heroes, Rachel Nonkin Avchen, an EIS disease detective.

Kernicterus, a lifelong brain syndrome, can include cerebral palsy, mental retardation, and hearing loss. It's caused by hyperbilirubinemia, a severe and untreated elevation in a newborn's bilirubin concentration. Bilirubin is a natural by-product of decomposing red blood cells, but when it's present at dangerous levels, it can cause severe brain damage or even death in an infant. Jaundice is the term used to refer to a yellow or orange discoloration of the skin and whites of the eyes. Jaundice occurs because a yellow pigment called bilirubin builds up in the blood. Bilirubin is normally found in small amounts in the blood from the natural break down of old, damaged or abnormal red blood cells. Bilirubin is normally removed from the blood by the liver. The liver then converts (conjugates) the bilirubin into another form. Once conjugated, the bilirubin can be excreted as part of bile into the small intestine. From there, bile is removed from the body along with bowel movements. Anything that speeds up the rate that red blood cells are broken down or interferes with the removal of bilirubin or bile from the body can result in jaundice. Why do some newborn babies develop jaundice? Newborn jaundice is a common condition in newborn babies and, for the most part, causes no serious harm. A number of factors can contribute to the development of jaundice in a newborn. These factors include: an immature liver that can't convert or excrete bilirubin effectively.

Shapiro, M.D., Associate Professor of Neurology at VCU, has been a leader in the diagnosis and treatment of kernicterus since 1984. Dr. Shapiro has received over $4 million in continued funding from the NIH for his work, and he was recently awarded over $950,000 for work over the next three years. Dr. Shapiro also has worked extensively to implement a Virginia-based center for kernicterus treatment. Extractions: The term, hyperbilirubinemia, denotes an excess of bilirubin, a condition which is potentially neurotoxic. Kernicterus is a neurologic syndrome resulting from deposition of unconjugated bilirubin in the brain. It can occur at much lower levels of serum bilirubin in sick or premature infants than in healthy, full-term babies. While the toxic level for healthy infants is listed at or above 25 mgs/dL in current pediatric texts, levels as low or lower than l0 mgs/dL can cause brain damage in a sick or premature infant.. Other predisposing factors to kernicterus include acidosis, lowered albumin levels, hypoxia, free fatty acids (lipids), salicylates (aspirin), antibiotics, and pitocin (used in Alan's mother to induce labor). The mortality rate can exceed 50%. Early symptoms include lethargy, poor feeding, vomiting, and neurological depression, and a high-pitched cry. Even in normal newborns, serum bilirubin levels temporarily rise due to immaturity of the liver enzyme systems, including those which conjugate and excrete bilirubin, and to a lack of intestinal bacteria which aid in the processing of bilirubin. Serum albumin deficiency, when present, can contribute to the problem. If this is true in healthy newborns, it is far more so in immature and compromised infants.
