Extractions: CASE REPORT Pernicious Anemia and Gastric Cancer By David Sutin, MD Abstract An elderly man had undergone a total gastrectomy, which had revealed a gastric adenocarcinoma. When he was seen in the clinic several months later, test results were compatible with a diagnosis of vitamin B deficiency. It is likely that this patient had pernicious anemia for several years and had developed an associated gastric carcinoma. ( Annals of Long-Term Care Case Report An elderly man presented to the geriatric clinic in early September 1997. He reported that in May 1997 he had black stool. An endoscopic evaluation at the time had shown a 20 x 20-mm gastric ulcer, the initial biopsy of which had been negative for malignancy. A second endoscopy with biopsy 2 weeks later, however, was positive for adenocarcinoma, moderately differentiated. The patient then had an abdominal computerized tomography scan, which had shown no metastasis. He had undergone a total gastrectomy, which had revealed a gastric adenocarcinoma, with metastasis in the omental tissue and in one of eight lymph nodes. The patient had done well postoperatively and was then discharged and followed in the geriatric and oncology clinics. When he was seen in the geriatric clinic, he had no complaints except for frequent flatus and occasional soft stool postoperatively and occasional dyspnea on climbing up a hill. He had a history of smoking one pack of cigarettes per day until 2 months prior to the visit, denied having any allergies, and, except for occasional Mylanta, was on no medication. He also denied any previous medical history, except as above, and had not seen a physician for 50 years. The patient lives with his son, and they alternate the shopping and the cooking responsibilities.

Contact Pernicious Anemia, A Victory For Science Anemia is a medical condition in which the blood is low in red blood cells or hemoglobin. However, in the later part of the nineteenth century, a new form of anemia was gradually identified as a distinct medical condition, termed pernicious anemia. The victim commonly exhibited an inflamed, glossy tongue, slight jaundice or yellowing of the eyes and skin, and persistent tingling or numbness in the fingers or toes. Microscopic examination of the blood revealed cells with overly large nuclei called megaloblasts (today the hallmark sign of pernicious anemia). The anemia was almost always fatal. The survival after the onset of the disease was usually from one to three years (Hardisty 1982). For over seventy years since its first recognition, pernicious anemia remained an incurable disorder with an outlook of almost certain death. It was not until 1926 when two physicians from Boston, Doctors George R. Minot and William P. Murphy announced insights into the cure of the disease. In 1925, Minot was 40 years old and an assistant professor of medicine at Harvard University. He also had a busy practice in hematology. He recruited Murphy from Peter Bent Brigham Hospital to help him with his busy practice. Minot wanted all of his office associates to have some research experience and to have published at least one paper before starting his practice. So Minot suggested to Murphy to search for some sort of dietary supplement for patients with anemia. Minot had been interested in the dietary habits in pernicious anemia patients since he graduated from Harvard Medical School. He had noticed that the victims disliked the taste of meat and had poor eating habits (Krantz 1974).

Most healthy people (except for strict vegetarians) have a 3-5-year reserve of vitamin B12, which is easily absorbed from meats, in their livers. Vitamin B12 deficiency occurs when the stomach doesn't secrete a vital substance called "intrinsic factor," which is required for B12 absorption in the ileum (part of the small intestine). It also can result after the surgical removal of the stomach or ileum. Pernicious anemia often causes decreased appetite, weight loss, constipation and diarrhea, abdominal pain, swollen or painful tongue, tingling in the hands and feet, and lack of balance and coordination. Treatment usually starts with intramuscular B12 injections several times, and then less often as reserves accumulate. Therefore, we analyzed the data from 156 documented cases: there were 73 patients of "European" origin, 52 black patients and 31 Latin-American patients. The mean age (+/- 1 S.D.) at presentation among black women, 53 +/- 16 years, was lower than that of all the others (P less than 0.001 in most comparisons), and seven of the 33 black women were less than 40 years old. In addition, 23 of the 24 black women tested had circulating antibody to intrinsic factor. A similar though less striking antibody prevalence (85 per cent) and age pattern (60 +/- 13 years) in Latin-American women did not reach statistical significance. No other group exceeded the usual 55 to 70 per cent prevalence of antibody. These finding suggest a different form of or a different response to the disease in black women and perhaps in Latin-American women.

Megaloblastic (Greek: megalo=giant; blast=germ cell) anemias are those anemias resulting from pathologic slowing, by any cause, of the doubling of the nuclear DNA necessary for bone marrow nucleated cells to divide, with only minor slowing of the synthesis of cytoplasmic RNA. This nuclear-cytoplasmic dissociation becomes greater with each daughter germ cell division, until either the cells die or omit terminal division, surviving in the marrow as end stage giant red cells, white cells, and platelets. These are extruded into the peripheral blood as macroovalocytes, hypersegmented neutrophils, and giant platelets, all with a shortened life span, hence the pancytopenia (anemia, leucopenia, and thrombopenia), characteristic of full-blown megaloblastic anemia. The most common causes of megaloblastic anemia are deficiencies of vitamin B (cobalamin) and/or folate (folic acid). As with any nutrient deficiency, these two B-vitamin deficiencies may arise from any of 3 inadequacies (inadequate ingestion, absorption, or utilization), or any of 3 excesses (excessive requirement, destruction, or excretion).

Pernicious anemia results from a lack of intrinsic factor in gastric secretions which, in turn, causes inadequate Vitamin B12 absorption. Vitamin B12 deficiency results. Vitamin B12 deficiency causes a form of anemia specifically referred to as megaloblastic anemia Other causes of reductions in the level of intrinsic factor (and thus of pernicious anemia) include atrophic gastric mucosa, autoimmunity against gastric parietal cells, and autoimmunity against intrinsic factor. Absence of intrinsic factor itself is the most common cause of Vitamin B12 deficiency. Intrinsic factor is produced by cells within the stomach. In adults, the inability to make intrinsic factor can be the result of chronic gastritis or the result of gastrectomy. The onset of the disease is slow and may span decades.

Extractions: Pernicious Anemia Pernicious anemia (PA) is an autoimmune disease that occurs as an end stage of autoimmune gastritis which results in the destruction of gastric mucosa (moist tissue that lines the stomach). It follows long-term autoimmune gastritis (inflammation of the mucosal lining of the stomach). The autoimmune process is limited to the body of the stomach and causes gastric atrophy (wasting of tissue). The auto-reaction is against the gastric intrinsic factor (substance that makes the absorption of vitamin B-12 possible) and complement-fixing antibodies to gastric parietal cells (the binding site for vitamin B-12). Long-term destruction of the parietal cells of the gastric mucosa results in atrophy leading to the inability of the stomach to absorb and utilize vitamin B-12. Thus, PA is the end result of autoimmune gastritis. The gastric atrophy is caused by chronic inflammation from the autoimmune attack on the lining of the stomach and precedes the development of PA by many years.
