A well-balanced diet is essential to provide other elements such as folic acid , iron, and vitamin C for healthy blood cell development. Prognosis The outcome is usually excellent with treatment. Complications People with pernicious anemia may have gastric polyps and have twice the incidence of gastric cancer than the normal population.

Extractions: Pernicious Anemia Biochemistry Selective Paper Angel Lee November 2, 1999 Anemia is a broad term for a group of diseases that are characterized by a decrease in hematocrit or hemglobin concentration among their many effects on the human body. This can be a result of decreased production or increased destruction of red blood cells. Defects of stem cell proliferation, cell differentiation, DNA Synthesis, hemoglobin synthesis, or any combination would cause, among their effects, the levels of red blood cell production to be lower. Associated enzyme deficiencies, abnormally shaped hemoglobin and immune system activities will also lead to excessive destruction of red blood cells. The disease Pernicious Anemia (PA), also known as Megaloblastic Anemia, is one form with these associated systemic effects. This disease is a result of impaired absorption of Vitamin B , or Cobalamin, due to a deficiency of Intrinsic Factor and Hydrochloric Acid in gastric secretions. Generally, the disease manifests in two forms, a rare congenital form and an adult onset acquired form. Vitamin B12 is a corrinoid, which resembles the heme of hemoglobin, however uses cobalt instead of iron. It functions as a coenzyme for only two reactions in the human body, methionine synthase and L-methylmalonyl CoA mutase. Methionine synthase is involved in the synthesis of the amino acid methionine from homocysteine. Therefore methionine can not be regenerated and levels decrease. L-methylmalonyl CoA mutase is involved in the conversion of methylmalonyl coenzyme A to succinyl CoA. The end results of the Vitamin B12 deficiency includes slow and restricted DNA synthesis

Extractions: PATHOLOGICAL CONSIDERATIONS: A chronic, macrocytic anemia characterized by achlorhydria, gastro-intestinal disturbances, and neurologic syndromes. It occurs most often in Caucasians after age 50. The fundamental defect is a permanent deficiency of the intrinsic factor normally present in gastric juice. In addition to atrophy of gastric mucosa and changes in bone marrow common to all megaloblastic anemias, there is often specific neurologisc changes. Subacute combined degeneration of the spinal cord is a common finding. Degeneration may also occur in peripheral nerves, nerve plexuses of the gastro-intestinal tract, and subcortical areas in the motor region of the brain. PHYSIOLOGICAL CONSIDERATIONS: Pernicious anemia is basically a nutritional deficiency disease. Characteristic blood abnormalities can be corrected by use of folic acid. However, if there is an accompanying deficiency of vitamin B-12, an irreversible degeneration of the spinal cord may take place. The intrinsic factor normally produced by stomach mucosa is necessary in order that B-12 may be absorbed and utilized. In pernicious anemia, the stomach has become unable to produce either the intrinsic factor or hydrochloric acid. Vitamin B-6 and vitamin C increase absorption of vitamin B-12. Adequate protein is also needed as vitamin B-12 is almost always associated with animal proteins such as liver, kidney, muscle meats, etc.

This is especially relevant if you have a family history of this or are from northern European descent. It doesn't happen from a lack of B12 in diet and you need the injected form of B12 as it is a stomach absorption problem. YES it is one of the reasons why people get RLS. My partner is now sleeping nights about one week after first injection.

The anemia results from the death of parietal cells in the body of the stomach and in the absence of gastric parietal cells, a blood-sustaining vitamin, vitamin B12, is no longer transported to its site of absorption. The term 'pernicious' described the inevitable progress and fatal outcome of the anemia and this given name by Biermer in 1972 has endured despite the knowledge that it is appropriate only to the terminal phase of the disease. I.General Considerations The primary pathologic feature of PA is progressive destruction of the normal glands of the body of the stomach. This is associated with partial or complete loss of chief cells, which secrete pepsinogens, and the parietal cells, which secrete acid and intrinsic factor. When the last is complete, failure of vitamin B12 absorption occurs. Complete loss of intrinsic factor secretion may require many years or never occur. The gastric mucosa is infiltrated, homogeneously or in patchy distribution, with various numbers of polymorphonuclear and mononuclear leukocytes - predominantly the latter. Often the gastric body mucosa is partly or wholly replaced by small intestinal mucosa (metaplasia). II.Immunologic Pathogenesis

The active agent at this time was unknown, Eventually in 1948, the active agent was isolated, and characterised. The active agent, being vitamin B . Cobalamin is composed of a carbon ring, containing a cobolt atom in its centre. Cobalamin is involved in only two chemical reactions within the body (see figure 22.02.01). One involving the formation of methionine, and the other, the formation of succinate. These two reactions require modified versions of cobalamin, the methyltransferase requires a methylated cobalamin. Whereas the mutase requires an adenylated cobalamin. Defects in these two enzymes result in the symptoms of PA. Aetiology PA can be brought about by several possible defects. Cobalamin is absorbed across the gut by the secretion of a glycoprotein called "Intrinsic Factor" (IF). IF binds to cobalamin, and then facilitates cobnbalamins crossing into the blood stream through the IF receptor. Once in the blood, cobalamin is released by IF, and binds to transcobalamin II (TCII). This allows TCII, and cobalamin to be taken up by liver cells. Cobalamin is then modified by either adenylation, or methylation. Defects can arise in any of these pathways, particularly with IF, IF receptor, and TCII. A genetic defect in any of these will result in PA. A defect in the methylation/adenylation step results in much less severe symptoms.
