I knew Gloria Fallows for about two years before she was admitted to Intensive Care. I first saw her as an outpatient in 1992 when she was 63. Even then she was enormous 275 pounds, five feet two inches and had trouble breathing. Her chief complaint was "shortness of my breath." "Oh Dr. Martin!" she exclaimed back then. But, in fact, habitual loud snoring is the most common symptom of breathing disorders that occur during sleep. The person who snores not only sleeps restlessly, but also is at risk for serious disorders of the heart and lungs. Snoring can therefore be lifethreatening because it can lead to high blood pressure, irregular heart beats, heart attacks, and sudden death. Normal breathing must continue at all times whether awake or asleep. The act of breathing is an automatic, highly regulated mechanical function of the body. In healthy sleeping individuals, most muscular and neural activities will slow or even shut down but respiration goes on under a neuromuscular "auto pilot." However, if something goes wrong with the auto pilot during sleep, breathing may become erratic and inefficient. Understanding Sleep Sleep is a complex neurological state. Its primary function is rest and restoring the body's energy levels. Repeated interruption of sleep by breathing abnormalities such as cessation of breathing (apnea) or heavy snoring, leads to fragmented sleep and abnormal oxygen and carbon dioxide levels in the blood. Excessive daytime sleepiness and various disorders of the heart, lungs, and the nervous system result.

This year, I had the unfortunate experience of knowing what it is like to the family member of a patient in CCU. My brother, age 49, was near death and had called me to be with him. He was suffering the effects of years of morbid obesity and I had been aware of the S/Sx of sleep apnea and had tried to get him into a sleep study for years. He is the typical Pickwickian Syndrome patient. I'd like some sources of information to let him read about the syndrome, but it's not easy to locate on the net. Would you have any specific references? In layman's terms, please. He is a very inteligent man, but when it comes to paying attention to what he needs to do to change, it has to be brief, direct and to the point. Thanks.

Author's Email: farnelam@muss.mcmaster.ca Hello there, This is a message responding to your request for more information regarding the "pickwickian syndrome" which your mother is currently suffering with. I am a physiotherapy student from Ontario, Canada. I have only recently read some information on this syndrome. Since I am not a medical doctor, I cannot give you the details about the drugs used to treat this disorder. So, I will tell you the little I have learned from my research. My understanding is that most frequently this disorder is associated with being overweight (obese). People with this syndrome tend to stop breathing for periods during their sleep and sometimes during the day. As a result of this, they often have less oxygen in their blood. This can be damaging to various internal organs. If your mother is overweight it is very important that she try to lose some weight. The research seems to indicate that even losing 15-20kg will dramatically reduce the symptoms these people suffer (help your mother to breath more easily). One way to do this is to begin a walking program to improve fitness levels. Other treatments seem to include: oxygen therapy, the hormone-progesterone and various other medications. Your mother needs to see a doctor who can determine the cause of her pickwickian syndrome in order to find the appropriate treatment.
