Before digging into those reports, let's cover the basics: The Nelson Textbook of Pediatrics says " Rickets is the term signifying a failure in mineralization of growing bone or osteoid tissue....During the first third of this century, the predominant cause of rickets was nutritional deficiency of vitamin D due either to inadequate direct exposure to ultraviolet rays in sunlight (296-310 nm; these rays do not pass through ordinary window glass) or to inadequate intake of vitamin D, or both. Vitamin D deficiency rickets is rare among infants and children in the industrialized countries. Deficiency may occur in unsupplemented dark-skinned infants or in breast-fed infants of mothers unexposed to sunlight." (p. 184) Vitamin D does not itself deposit into bone, but it's a crucial part of the metabolic pathway that allows new bone cells to take up calcium and so harden ("mineralize"). Cow's milk and infant formula these days are routinely fortified with vitamin D. Previously, cod liver oil was one of the better sources, which accounts for it being given to children. The precursor of vitamin D is naturally available in human skin and automatically converted to vitamin D when the skin is exposed to sunlight, so you can do without the supplement if you're exposed to sunlight see, that stuff about getting out and playing in the fresh air is not just your mother trying to get rid of you.

Extractions: Welcome ! My name is Lucy Rickets and I am a UK based textile artist producing contemporary embroidered textiles. I graduated from Manchester Metropolitan University in 1991 with a BA (Hons) in textiles with embroidery, and I now make small, unique and colourful embroidered collages which I exhibit and sell. Inspiration My embroideries primarily focus on still life scenes, both interior and exterior. I have a passion for colour and decoration, particularly in the home environment, and the balancing of colour is a central concern in my work. I often use ceramics, flowers, fruit, cloth and other collectable objects to provide me with a rich source of colour, pattern and design. Each of my embroidered pieces is a one-off creation which evolves from my small sketches and paintings. Technique I use commercially dyed cotton fabrics which give me a strong palette of flat colour to work with. In addition to this I quite often hand paint areas to create a more painterly effect. Both appliqué and embroidery techniques feature in my work, and I begin by piecing the central image. Each piece of fabric is carefully cut out, pieced and stitched by either hand or machine. Sometimes this involves a painstaking procedure of cutting tiny petals, leaves or spots which are then applied and stitched into place. The machine embroidery is done using a darning foot and free machining without the feed. I have a very old sewing machine which has no set pattern facilities, so all the machine embroidery you see is created by my own careful control of the fabric under the needle. I keep my hand embroidery simple and use just a handful of stitches to secure and embellish the fabric.

Rickets, a debilitating disease affecting bone growth and resulting in gross deformities, is usually associated with a lack of sunlight. However, in this case the Cornell scientists believe that the calcium deficiency being seen in that region of Bangladesh among children is somehow exacerbated by either soil or water conditions, or poor nutrition, or all of the above. Characterized by flaring of lower ribs, pigeon-shaped chest, delayed teeth, widening wrists, malformed ankles or knees, the disease also can be recognized as growth retardation and laterally deformed arms and legs. The disease's wide prevalence in the Chakaria region of the country has caught the attention of the Cornell scientists. Rickets is a disease unexpected in tropical or semitropical regions of the world like Bangladesh, according to Gerald Combs, professor of nutrition in Cornell's College of Human Ecology. There is enough exposure to sunlight, to give the children's skin ample ability to produce enough vitamin D to stave off rickets, he explained.

One of the classic physical manifestations of rickets is frontal bossing (enlarged forehead) and box-like shape of the head. Rickets is a prevalent health problem in children all over the world, not just for children living in orphanages. It is one of the top five medical issues in children living in China (malnutrition, rickets, anemia, lead poisoning, and asthma).3 Children living in orphanages do not get enough calcium and vitamin D in their diet and they lack exposure to sunlight, decreasing the skin's ability to produce adequate vitamin D.4, 5 The muscle weakness (hypotonia) due to the lack of calcium can cause the child to appear floppy. When the child arrives in the U.S., the child may appear developmentally delayed as a result of the decreased muscle tone and weakness of the bones due to rickets. Lack of stimulation and lack of exercise can also contribute to muscle weakness, especially muscles of the abdomen (belly). When a baby lies in the supine position (lying on the back) chronically, the abdominal (belly) muscles are weak and malnutrition can cause muscle wasting. Positioning on the belly improves this condition swiftly. Improved nutrition with vitamin D and calcium supplemented foods (formula, milk, milk products, green leafy vegetables, calcium supplemented juices) and vitamins, exposure to sunlight, and exercise rapidly improves the child's muscle tone and bone strength.

It is probably present in 100% of all children adopted from China, even when it is not apparent to the observer. The predominant cause of rickets in orphanages abroad is nutritional deficiency of vitamin D due either to inadequate direct exposure to ultraviolet rays in sunlight or to inadequate intake of vitamin D, or both. Sunlight is necessary for the formation of vitamin D in the human body and vitamin D is necessary to make bone in the human body. Inadequate calcium and phosphorus in a child's diet also contributes to the development of rickets. When a child first arrives from China, rickets can be diagnosed by a skilled pediatrician who performs a thorough physical exam and orders a blood test called a "rickets screen". The rickets screen consists of alkaline phosphatase, calcium, and phosphorus. The calcium, phosphorus, and alkaline phosphatase levels in the blood may help support the diagnosis of rickets. The child with mild rickets may have no abnormalities in the rickets screen and a normal diet will effectively reverse any weakness in bones and muscles. If the child has bow legs, raised bony bumps along the ribs (rachitic rosaries), frontal bossing of the skull, and other deformities of the bones, rickets might be the cause. Some children have no obvious bony deformities, but still may have weak bones and muscles from a mild form of rickets. Eruption of the baby teeth may be delayed and there may be defects of the enamel and extensive decay of the teeth. Permanent teeth may also inevitably be affected. Muscles are poorly developed and decreased tone is common in rickets. Vitamin D is essential for the absorption of calcium and calcium is necessary for muscle function. Standing and walking are often delayed because of this decreased muscle tone. These are all reversible conditions with mild or moderate rickets. The treatment of rickets is vitamin D containing foods such as milk, soy products fortified with calcium and vitamin D, or formula which is fortified with vitamin D. Exposure to natural and artificial light is necessary. Some infants and children may require vitamin D supplements for weeks, months, or years.
