The same source produces the word sarcasm, which means "to cut the flesh" or the word sarcophagus, which is a box to carry flesh. Most patients do not have any complaints but some experience difficulty in breathing particularly with heavy exercise. A few have a dry cough, muscle weakness, lethargy and listlessness. Whether these symptoms are present or absent the chest x-ray will almost always show evidence of sarcoidosis. There are many causes for this x-ray shadow so the doctor will do blood tests, analyze samples of fluid from the bronchial tubes and carry out lung function breathing tests. The chest x-ray will be repeated at intervals until it has cleared. Accompanying it are severe joint pans in knees and ankles. Although the red spots may frighten the patient it is harmless and will eventually subside with the help of pain-killing tablets.

It can appear in almost any body organ, but most often starts in the lungs or lymph nodes. No one yet knows what causes sarcoidosis. The disease can appear suddenly and disappear. Or it can develop gradually and go on to produce symptoms that come and go, sometimes for a lifetime. As sarcoidosis progresses, small lumps, or granulomas, appear in the affected tissues. In the majority of cases, these granulomas clear up, either with or without treatment. In the few cases where the granulomas do not heal and disappear, the tissues tend to remain inflamed and become scarred (fibrotic). Sarcoidosis was first identified over 100 years ago by two dermatologists working independently, Dr. Jonathan Hutchinson in England and Dr. Caesar Boeck in Norway. Sarcoidosis was originally called Hutchinson's disease or Boeck's disease. Dr. Boeck went on to fashion today's name for the disease from the Greek words "sark" and "oid," meaning flesh-like. The term describes the skin eruptions that are frequently caused by the illness.

Extractions: SARCOIDOSIS Sarcoidosis is a chronic, multisystem disorder of an unknown etiology characterized by the accumulation of T-lymphocytes and mononuclear phagocytes, nonsecreting epithelial granulomas and derangements of the normal tissue architecture in affected organs. All parts of the body can be affected, but the organ most affected is the lung. Involvement of the skin, eye and lymph nodes is also common. A variety of infectious and noninfectious agents have been implicated, but there is no proof that any specific agent is responsible. However, available evidence is consistent with the concept that the disease result from an exaggerated cellular immune response (acquired, inherited or both) to a limited class of antigens or self antigens. Cases of sarcoid have been described in both sexes, almost all ages, races and geographic locations. Females appear to be slightly more susceptible than males. There is remarkable diversity of the prevalence of sarcoidosis among certain ethnic and racial groups. In the United States, the majority of patients are black 10:1 to 17:1. Blacks are often younger than whites with the disease. In Europe, however, it affects mostly whites with higher prevalence in Sweden and among Irish females. It is most common between the ages of 20-40, but it can occur in children and in the elderly. Although it is rare in children, the disease is most frequent between 9 to 15 years of age. A small cluster occurs in children under age 4 years with one half less than 1 year of age.

The onset of the disease is usually insidious but can be acute. Respiratory symptoms are most common and include cough, chest discomfort, and dyspnea. Other symptoms and signs reflect the specific organs involved by the granulomatous disease. After the thorax, the lymph nodes, skin and the eye are most often involved. Other organs much less often produce signs and symptoms despite the fact that there are granulomas on histologic examination in many organs in the majority of patients. Peripheral lymphadenopathy is common. The majority of patients have palpable non-tender lymph nodes. There is thoracic lymphadenopathy in the large majority of patients. These enlarged lymph nodes seldom cause symptoms, but rare cases of venous obstruction including the superior vena caval syndrome and bronchial obstruction producing ateletasis have been reported.

Many organs can be involved, in fact, one of the most noticeable symptoms is the peculiar skin lesions, erythema nodusum, that develop on the lower legs. Sarcoidosis is a disease of unknown etiology characterized by granuloma formation in a variety of organs. Granulomas are collections of immune cells and disease that result from granulomatous disorders. There are a variety of granulomatous disorders that closely resemble sarcoidosis, including tuberculosis. The close resemblence to tuberuculosis has led to speculation that sarcoidosis is due to an infectious organism. It is the formation of these granulomas and the resulting tissue damage caused by the cells within these granulomas that cause the symptoms of sarcoidosis.
