Always refer to the full paper to resolve any points of confusion Sarcoidosis has been one of the few remaining "mystery diseases" of modern medicine. First discovered in 1877 by Jonathan Hutchinson, it was the work of Guy Scadding in the late 1940s and early 50s that really defined todays view of this disorder as being characterized by "non-caseating granuloma". 'Granuloma' are a special type of inflammation. They are an aggregation of lymphocytes monocytes macrophages and ... epithelioid giant cells . They form in soft tissues throughout the body. Although they are often found in the liver, kidney and brain, most commonly they are found in the interstitial tissue of the lungs. Since the lungs use the interstitial tissue to transfer air and other gasses to and from the bloodstream, the granuloma stop the lungs from functioning properly. The word "non-caseating" means that the granuloma remain healthy, even while they are creating the inflammatory biochemicals (Cytokines) that are designed to help our immune systems attack and kill invading, foreign, tissue and organisms (viruses, bacteria and fungi). Earlier this year, a group of Swedish Scientists published pictures of Bacteria from a tick-borne disease that were living and replicating in the granuloma of 26 Sarcoidosis patients. The organisms were from the genus "Rickettsia". In the USA, Rickettsia organisms give rise to "Rocky Mountains Spotted Fever," while in Asia they cause "Scrub Typhus". Using an Electron Microscope, with a magnification of 84 000, they photographed two Rickettsia cells which were in the process of replicating by dividing into two. The granuloma are supposed to kill such bacteria, yet here was evidence that bacteria can live and replicate within those granuloma. Other scientists have since found different types of bacteria which also seem to be involved in sarcoid inflammation.

Sarcoidosis is characterized by the presence of small areas of inflammed cells called granulomas. Granulomas can be found inside the body, for example, on the walls of the bronchi and bronchioles (breathing tubes) in the lungs. They can also appear as sores on the skin. What are the symptoms of sarcoidosis? Sarcoidosis causes a variety of symptoms or no symptoms at all. Extractions: Send e-mail Background For many years, thalidomide has been used to clear a severe skin reaction associated with leprosy treatment. More recently, thalidomide has shown efficacy in chronic inflammatory skin and mucous membrane disorders such as discoid lupus, HIV-associated oral ulcers, graft-versus-host disease, and inflammatory bowel disease. Recent work has suggested that thalidomide may also attenuate weight loss in patients with tuberculosis and HIV infections. The exact mechanism by which thalidomide exerts its beneficial action in these diseases is still an object of intense research. Thalidomide can decrease levels of an inflammation causing element in the blood called tumor necrosis factor alpha (TNF- a ), an important factor in granuloma formation. Studies have also suggested that thalidomide may alter new blood vessel formation and enhance certain forms of the body's immune response. Although corticosteroid treatment has been generally successful at arresting or reversing sarcoidosis, the prominent steroid side effects and potential for developing disease resistance to steroids are constant concerns. New and more specific treatments need to be developed to control sarcoidosis. Although a few anecdotal reports suggest thalidomide may be effective in sarcoid patients, no organized evaluation of thalidomide use in this disease exists. We have recently completed an open pilot study to evaluate thalidomide's effect in 10 sarcoidosis patients. Although the main focus of the study was on safety, we also observed the clinical and immune effects of the drug on these patients. We are currently preparing a second study to answer specific questions posed by this initial study. When this study is open for enrollment, we will post the information on this website.

Since the lungs are affected in more than 90% of patients with sarcoid, pulmonologists are usually called upon to take care of patients with this disease. Sarcoid can occur silently and become detected incidentally when a CXR is obtained for other reasons, or can produce symptoms such as cough or shortness of breath. Since sarcoid often resolves spontaneously, treatment is not always necessary. Sarcoid can also affect the skin and the eyes, and rarely other organs of the body.

Harvard Medical School Boston, MA Sarcoidosis Sarcoidosis is a multisystem granulomatous disease which was first described by Jonathan Hutchinson in 1878. Its clinical manifestations and course can be variable in different ethnic groups. The organs affected more often are the lungs, skin and eyes. The frequency of ocular involvement ranges from 26% to 50%. The characteristics of ocular involvement are (1) when present, is seen generally early in the course of the disease (2) may co-exist with asymptomatic systemic disease and (3) can precede systemic involvement by several years. Most patients present between the ages of 20 to 40 years; however, children and the elderly can be affected. Cases of familial sarcoidosis, including monozygotic twins, and husband-wife pairs have also been reported. Diagnosis relies on demonstration of non caseating granuloma by tissue biopsy. In cases of suspected sarcoidosis where no affected tissue amenable to biopsy is identifiable, supportive evidence of the diagnosis can be obtained through non-invasive investigations including measurement of serum angiotensin converting enzyme (ACE) and lysozyme, chest x-ray, chest computerized tomography (C-T), gallium scintillography, pulmonary function tests, bronchoalveolar lavage, and measurement of serum and urinary calcium. Ocular manifestations Anterior segment Conjunctival involvement has been reported in 6.9%-70% of patients with ocular sarcoidosis (Figure 1).

The cause of the disease is unknown. Tissue samples from involved organs show granulomas, which are clusters of macrophages, lymphocytes, and cells called multinucleated giant cells. Possible causes include a hypersensitivity response, a genetic predisposition, infection, or chemicals. The incidence varies widely according to race and sex.
