If the spine is very crooked, the ribs or hips may stick out more on one side than the other side. Also, one shoulder may be lower than the other. Scoliosis may begin in childhood but often is not noticed until the teenage years. In most cases, the exact cause of scoliosis isn't known. It seems to run in some families. Scoliosis is more common in girls than in boys. How can I tell if my child has scoliosis? Look at your child's spine to see if it curves or if it is straight. Also check to see if one shoulder is lower than the other. Many public schools check for scoliosis in the 5 th or 6 th grade. Your doctor may also examine your child for scoliosis at a regular check-up. Your doctor may be able to tell if your child has scoliosis just by looking at your child's back when it is in different positions. Your doctor may have your child stand and bend over to touch the toes. Sometimes x-rays help show the curve in the spine. Does scoliosis cause any problems?

It usually starts in the early teens or pre-teens and may gradually progress as rapid growth occurs. Once rapid growth (puberty) is over then mild curves often do not change while severe curves nearly always progress.

    Extractions: There are many causes of a structural scoliosis, but by far the most common (80 to 90 per cent) is the unknown, or idiopathic. This idiopathic scoliosis develops usually as the spine is growing rapidly. The earliest form, or infantile form, occurs in the first three years of life, and it usually resolves with time. The juvenile form occurs up to the age of nine, and has a high familial relationship. It can often be a progressive scoliosis. The adolescent form is the most common, and occurs from nine to fourteen years, and the most severe cases involve females. It is not known for sure why some children get a scoliosis. It appears that genetic inheritance is a major contributor to a scoliosis. The most important part of the management of scoliosis is the early detection of its presence, as early management may prevent the need for surgery. The Spine Society of Australia recommends that screening for scoliosis should be done on Year 7 and Year 9 schoolgirls. The management of a scoliosis is determined by the extent of the scoliosis. A number of methods are used to decide upon the most appropriate treatment. In most instances a mild scoliosis requires no specific treatment. Advice in regard to posture and exercises may be offered. If the scoliosis is more severe it must be treated.

Congenital Scoliosis is therefore a lateral curvature of the spine due to congenital anomalies of the vertebrae. These are usually easily seen on standard x-rays, but on rare occasions are discovered only on MRI or at the time of surgery. CAUSATION Why are some children born with these abnormal vertebrae? We are seldom able to pinpoint a specific reason - a very similar situation to children born with congenital heart defects. Even in the situation of identical twins, usually only one twin has the problem, thus indicating a very low genetic tendency. IN the nearly 2,000 patients in our files with congenital scoliosis, only 15 have had a sibling or parent with the problem. NATURAL HISTORY There have been several excellent studies of the natural history of congenital scoliosis, and fortunately they all agree with one another. About 25 percent of patients show no progression at all. Most of these require no active treatments, but do require periodic checkups until growth is complete.
